Composite graft replacement after aortic valvuloplasty in Takayasu arteritis.

A 24-year-old woman had undergone valvuloplasty of the aortic valve and external reinforcement of an aneurysm of the ascending aorta during the active phase of Takayasu arteritis 1 year prior to admission to our hospital. On examination, she was diagnosed as having a large false aneurysm of the ascending aorta with annuloaortic ectasia and severe aortic regurgitation, bilateral common carotid artery aneurysms with a left internal carotid artery saccular aneurysm, and bilateral subclavian artery and right vertebral artery obstructions due to Takayasu arteritis. Because of the risk of rupture, s…

Giant cell (temporal) arteritis diagnosed following upper limb claudication.

Giant cell arteritis or temporal arteritis occurs almost exclusively in people over 50 years of age. It classically presents with new onset temporal headache, scalp tenderness and jaw claudication. Proximal muscle pain and stiffness is often present because of frequent association with polymyalgia rheumatica. In most cases, the erythrocyte sedimentation rate is markedly elevated. Uncommon presentations include systemic symptoms and symptoms related to large artery involvement. We report a case of giant cell arteritis without symptoms related to the temporal artery, diagnosed angiographically f…

Takayasu's arteritis-associated aneurysm formation without steno-occlusive lesions.

The authors report a case of a 20-year-old woman with Takayasu's arteritis (TA) presenting with recurrent erythema nodosum-like lesions, elevated acute-phase proteins and aortographic findings of multiple aneurysmal dilatations of the aorta without the coexistence of steno-occlusive lesions. This finding indicates that aneurysms could be an early manifestation of TA and not necessarily a change secondary to stenotic lesions.

Takayasu's arteritis concurrent with Marfan syndrome--a case report.

Marfan syndrome (MS) is a dominantly inherited connective tissue disorder characterized by arachnodactyly, tall stature, the presence of aortic aneurysm, and lens dislocation. Takayasu's arteritis (TA) is a chronic vasculitis that primarily affects the aorta and its branches. The authors report the first case of TA in a patient with MS. The simultaneous presence of TA and MS could be a coincidence, however; the pathogenesis of TA might be linked with autoimmunity induced by abnormal extracellular matrix protein derived from the genetic mutations in MS.

Non specific aorto-arteritis. Clinical picture, diagnosis and therapy.

This study is aimed at analyzing clinical features, angiographic findings and evolution of Takayasu's arteritis and the criteria adopted to establish the indication for non-surgical versus operative treatment. Eighteen patients affected by non specific aortarteritis were observed and treated at our Department between 1973 and 1996. All patients met the American College of Rheumatology 1990 criteria of classification of Takayasu's arteritis. Sixteen patients were young females. Two patients were males. Nine patients underwent surgical procedures. One young female underwent a PTA. All eight asym…

Takayasu's arteritis with renovascular hypertension: results of surgical treatment.

Renal artery stenosis caused by Takayasu's arteritis is an important cause of hypertension in young patients in the Far East. The role of surgery in Takayasu's arteritis is not as well-defined as in atherosclerosis or fibromuscular dysplasia. In this retrospective review, the author reports the results of 19 renal artery reconstructions in 12 young patients (median age 23.5 years, range 10-46 years) presenting with renovascular hypertension and Takayasu's arteritis, and discusses the different surgical options. The procedures performed included aortorenal bypass using vein (five), aortorenal b…

Secondary amyloidosis associated with giant cell arteritis/polymyalgia rheumatica.

Although giant cell arteritis (GCA) is characterized by chronic inflammation, secondary (AA) amyloidosis appears to be an exceptionally rare complication of this disorder. We describe an 84-year-old man with biopsy proven GCA and polymyalgia rheumatica (PMR) who was found at autopsy to have AA amyloid deposition in numerous organs, 9 years after his diagnosis of GCA. Persistent musculoskeletal symptoms, attributed to refractory PMR during the patient's life, were likely due to AA amyloidosis. This unrecognized complication of GCA/PMR confounded his therapy, leading to excessive treatment with …

Surgical treatment for aortic regurgitation caused by Takayasu's arteritis.

Aortic regurgitation (AR) caused by Takayasu's arteritis is relatively rare. We investigated the surgical results in patients with AR caused by Takayasu's arteritis. Between 1978 and December 1997, 65 patients (5 males, 60 females) with AR secondary to Takayasu's arteritis underwent surgery. The patients' ages ranged from 19 to 70 years (mean, 48 years). Takayasu's arteritis was diagnosed by clinical examination confirmed by intraoperative pathological examination of the aortic wall. The surgical procedures for AR were aortic valve replacement in 45 patients, Bentall-type operation in 19, and …

Psoriatic arthritis associated with dilated cardiomyopathy and Takayasu's arteritis.

A 40-year-old Japanese man with psoriatic arthritis (PA) involving the spine, sacroiliac and peripheral joints presented with dyspnoea and ankle oedema. Blood pressure was 180/110 and 114/80 mmHg in the right and left upper arms, respectively. Examinations showed left ventricular dilatation and diffuse hypokinesis of the left ventricle, with no involvement of the coronary arteries. Aortography detected total occlusion of the left subclavian artery and stenosis of the origin at the right renal artery. Dilated cardiomyopathy and Takayasu's arteritis associated with PA was diagnosed. A few cases …

Takayasu arteritis: ultrasonographic evaluation of the cervico-cerebral arteries.

PURPOSE: To present the ultrasonographic findings of the cervico-cerebral arteries in patients with Takayasu arteritis. MATERIALS AND METHODS: We used duplex ultrasound to study the arteries in the neck including the common carotid artery (CCA), internal carotid artery (ICA), external carotid artery (ECA) and the vertebral arteries. For the intracranial arteries, transcranial Doppler ultrasound was used to study the blood flow velocities and directions in the middle cerebral artery (MCA), anterior cerebral artery (ACA), posterior cerebral artery (PCA), ophthalmic artery, basilar and vertebral…

Cerebral malacia caused by Takayasu's arteritis--a case report.

This paper reports an unusual autopsy case, a 27-year-old man, who died from severe cerebral malacia. It is well known to our legal medical workers that cerebral malacia could be caused by injury and/ or disease. By coincidence, the deceased was subjected to head injury nine months prior to death, so differential diagnosis of cerebral malacia is very important in this case. His cerebral malacia was finally confirmed to be caused by Takayasu's arteritis not by his head injury by means of autopsy and pathological examination.

Temporal arteritis and polymyalgia rheumatica: nosographic and nosologic considerations.

Original reports on temporal arteritis and polymyalgia rheumatica were reviewed before and after the introduction of steroid therapy to prevent blindness in temporal arteritis. In some cases, the original data were reworked. There is evidence that both diseases have become more benign. As a result, the perceived risk of blindness is presently overestimated, as is the perceived benefit of steroids in reducing this risk, and neither should be used as support for an essential difference between temporal arteritis and polymyalgia rheumatica. Indeed, no qualitative differences otherwise exist and b…

[Follow-up study of Takayasu arteritis with aortic regurgitation].

The clinical courses of patients with Takayasu arteritis vary especially when aortic regurgitation is involved. The clinical features and outcomes of Takayasu arteritis were studied in 78 patients to clarify the influence of aortic regurgitation on the natural history, especially the earlier stages of aortic regurgitation after onset of Takayasu arteritis. During the average 12.7-year follow-up period, 7% (3/43) of patients without aortic regurgitation died, but 17% (6/35) of patients with aortic regurgitation died. Mortality was low (6%; 1/16) in patients with mild (grade II or less) regurgit…

Temporal arteritis with cerebral complications: report of four cases.

We report on four patients with a histologically proven diagnosis of arteritis temporalis and clinical and/or neuroradiological evidence of severe focal cerebral ischemia due to intracranial vasculitis. While one patient suffered from a transient ischemic attack, CCT and MRI scans of the other patients showed multiple lacunar infarctions, combined with territorial infarctions in two cases. Necropsy in one patient demonstrated generalized giant cell vasculitis in large and small cerebral vessels. We suppose that the cerebral involvement was provoked by insufficient steroid therapy of arteritis …

Concentric left ventricular hypertrophy in patients with Takayasu arteritis.

Although aortic regurgitation (AR) in patients with Takayasu arteritis (TA) is believed to be caused by dilatation of the aortic ring or thickening of the aortic cusps, the pattern of left ventricular hypertrophy (LVH) in these patients has not been described. To clarify the geometry of the LV in patients with TA that had been documented both echocardiographically and hemodynamically, the authors evaluated 22 patients with TA and severe AR (group 1), 10 with TA and systemic hypertension (group 2), and 17 with isolated severe AR of other etiologies (group 3). M-mode echocardiography showed that…
