We report, to the best of our knowledge, the first case of a child with typical ataxia telangiectasia (A-T) who developed juvenile idiopathic arthritis (JIA). The patient was a 15-year-old boy with A-T who presented with noninfectious polyarthritis. A-T is a rare, autosomal recessive disorder characterized by cerebellar atrophy, oculocutaneous telangiectasia, immunodeficiency, radiosensitivity, and predisposition to cancer. The gene responsible for A-T is the A-T mutated (ATM) gene. Clinical manifestations of the disorder are the result of lacking ATM protein, which is involved in DNA repair,

Immune checkpoint inhibitors (ICIs) are increasingly used in the treatment of advanced malignancies, but they can cause a wide range of adverse effects, including inflammatory arthritis. Severe ICI-induced inflammatory arthritis (ICI-IA) is rare, and its distinguishing clinical features are not defined. We present a patient with metastatic urothelial carcinoma who developed severe, polyarticular inflammatory arthritis while being treated with an ICI. Arthrocentesis of native and prosthetic joints revealed a significantly elevated white blood cell (WBC) count with a neutrophil predominance. Ant

Syphilis is a great mimicker of diseases and has a broad presentation ranging from painless chancre, constitutional symptoms, adenopathy, rash, synovitis, neurological, and ocular findings. Syphilis-related bone and joint involvement is usually found in congenital form, but it can also be seen in adults with acquired syphilis as a rare sequela of infectious syphilis. We report a case of a patient diagnosed as rheumatoid arthritis, who presented to the ophthalmology outpatient department with an acute onset of black spots in his visual field, whose clinical examination was a diagnostic dilemma,

Rheumatoid arthritis is a chronic inflammatory rheumatic disease that can cause extra-articular manifestations, in particular pleuropulmonary involvement, which is the second leading cause of death in this connective tissue disease. We report a rare pulmonary manifestation of RA. This was a 58-year-old patient being followed for RA on leflunomide with 3 excavated pulmonary nodules measuring 13 mm, 10 mm and 6 mm, for which the diagnostic hypotheses of infection, pulmonary metastases and rheumatoid nodules had been put forward. The diagnosis of excavated pulmonary nodules in rheumatoid arthriti

A questionnaire survey covered 11 females with temporal arteritis (CA) and 20 healthy controls. Gastrointestinal, joint and spinal complaints were significantly more frequent in CA patients. Clinical characteristics of joint and spinal disorders in CA patients are close to those of chronic reactive arthritis. It is suggested that Horton's disease may be lacking as a stage of the infectious process in elderly patients (chronic intestinal and/or urinary infection-reactive arthritis-CA). A case is reported where CA developed after intestinal and urinary infection. Administration of antibiotics fo

We describe 4 patients successfully treated with intraarticular osmic acid for post-infectious arthritis persisting after apparent eradication of bacteria from the infected joint. In addition the bactericidal effect of osmic acid in vitro is demonstrated. We conclude that treatment with osmic acid carries no risk of iatrogenic infection; on the contrary, an antibacterial effect seems likely.

The case of an 8-year-old boy with tuberculous arthritis of the right knee in whom patellar disease was the only radiological abnormality is presented. The patellar findings were a large lytic area with sclerotic borders containing a dense image corresponding to a sequestrum. Open biopsy was necessary for diagnosis, since the radiological abnormalities are consistent with other chronic infectious diseases.

INTRODUCTION: Biologicals are a fast expanding group of new drugs and rituximab (RTX) is one of them. Long-term efficacy and safety constantly need addressing as little is known about these factors. In rheumatoid arthritis, RTX it is used for active disease that is not responding to other therapies. Since RTX acts by depleting B-cells, concerns regarding the long-term safety of this drug have been raised.
AREAS COVERED: This review covers 10 manuscripts on RTX safety in rheumatoid arthritis published between January 2004 and July 2010.
EXPERT OPINION: In present literature RTX appears to be

OBJECTIVE: To evaluate the incidence, complications, and course of Felty's syndrome (FS) in patients treated with intramuscular (IM) gold.
METHODS: Retrospective chart review of all FS cases (1979 to 2003) was conducted in the Mary Pack Arthritis Centre (MPAC) gold clinic. FS was diagnosed if patients had rheumatoid arthritis (RA; American College of Rheumatology criteria) and persistent leukopenia [white blood cell (WBC) count < 4] in the absence of other known causes of leukopenia. Splenomegaly was not part of the inclusion criteria.
RESULTS: Thirteen patients with FS were identified in th

Habitual inflammation is defined as the persistent activation of the body's susceptible system in response to harmful events. This ongoing inflammatory process can sometimes harm normal organs & tissues. Crucially, chronic inflammation has been linked to the emergence and advancement of a variety of disorders, including cardiovascular diseases, respiratory issues, metabolic illnesses, neurological disorders, and cancer. These diseases are classified as chronic-degenerative conditions due to the sustained and persistent nature of the underlying chronic inflammatory processes. Treating patient's

BACKGROUND: Subcutaneous adalimumab is the preferred treatment for most children with juvenile idiopathic arthritis (JIA) and non-infectious uveitis, usually administered every other week. Some patients do not respond or lose responsiveness over time, leading to dose escalation to weekly administration. This study evaluated the efficacy and pharmacokinetics of weekly subcutaneous adalimumab in children with JIA and idiopathic uveitis.
METHODS: This is a retrospective study on clinical and pharmacokinetic characteristics of patients treated with subcutaneous adalimumab for psoriatic arthritis

The purpose of this investigation was to determine how specialists in paediatric infectious diseases (PIDs) manage children with suspected Lyme disease (LD) by comparing their approaches in Italian endemic and non-endemic areas. A cross-sectional survey of the PID specialists participating in the Italian Society for Pediatric Infectious Disease (SITIP) Registry of LD was carried out between 1 January and 30 April 2012. A total of 160 children (80 living in endemic areas and 80 living in non-endemic areas) were diagnosed as having LD between 1 January 2005 and 31 December 2011. The clinical man

A variety of infectious agents can be transmitted from animals to humans, either directly or indirectly. Unfortunately, the diagnosis of a zoonosis may not be considered at presentation, which can lead to a delay in initiating appropriate therapy, or is never made because the symptoms are non-specific and no exposure history is obtained. The keys to making a diagnosis are to recognize the clinical manifestations of the diseases (eg, fever of unknown origin, arthritis, rash, or lymphadenopathy), to obtain an exposure history, and to understand the local epidemiology. When a child presents with

BACKGROUND: There is accumulating evidence for an increased susceptibility to infection in patients with arthritis. We sought to understand the epidemiology of Toxoplasma gondii infection in arthritis patients in eastern China, given the paucity of data on the magnitude of T. gondii infection in these patients.
METHODS: Seroprevalence of T. gondii infection was assessed by enzyme-linked immunosorbent assay using a crude antigen of the parasite in 820 arthritic patients, and an equal number of healthy controls, from Qingdao and Weihai cities, eastern China. Sociodemographic, clinical and lifes

Differentiation of the many disorders that affect the sacroiliac joints can often be accomplished by attention to radiographic detail. By evaluating the distribution of disease, the presence of erosions and other changes as noted herein, and the course of disease over a period of time, a specific diagnosis can often be made, or at the very least, suggested (Table).

A large number of complications have been associated with rheumatoid arthritis (RA), those of infectious etiology being of special relevance. Their high incidence is closely linked to the use of immunosuppressive medication. The spectrum of agents causing opportunistic infections in patients with RA is very broad; however, there are relatively few cases of Leishmania infection, especially in patients not being treated with biological drugs.
