The predictive value of elevation in specific serum enzymes for subsequent development of hypoxic-ischemic encephalopathy or intraventricular hemorrhage in full-term and premature asphyxiated newborns.

Serum lactate dehydrogenase (LDH), aspartate aminotransferase (ASAT), and hydroxybutyrate dehydrogenase (HBDH) activities are significantly elevated in asphyxiated newborns within the first days of life. The approach of the present study was to evaluate firstly if serum levels of these enzymes correlate with the development of hypoxic-ischemic encephalopathy (HIE) and periventricular-intraventricular hemorrhage (PIVH) in full-term and premature asphyxiated newborns, and secondly if postnatally elevated enzyme activities could be predictive for these disorders. ASAT, LDH and HBDH activities wer

[Acute complications in full term neonates with severe neonatal asphyxia].

The acute systemic complications of perinatal asphyxia, defined as an umbilical artery pH at birth of 7.10 or less, were evaluated in fifty full-term newborn infants. We also investigated the prenatal complications that lead to asphyxia, and the relationship between Apgar score and cord pH. Asphyxia or fetal distress was not identified in 56% of the cases. The most common condition associated with asphyxia was prolonged labor followed by abruptio placentae. About half of the babies studied, suffered some degree of renal and brain dysfunction; 24% had severe respiratory disease. Myocardial fail

Complement fragment C3a in plasma of asphyxiated neonates.

Recent clinical studies with adult polytrauma patients indicate that elevated plasma levels of anaphylatoxin C3a correlate with the subsequent development of the adult respiratory distress syndrome (ARDS). However, there are no parameters which allow a reliable diagnosis of ARDS in neonates. As the most predisposing condition for ARDS seems to be shock, plasma C3a was determined in 30 ventilated premature infants and neonates with respiratory distress syndrome (birth weights 660-3350 g) within the first 24 h post partum or 6-24 h after acute asphyxia or shock during the neonatal period. The ra

Some animal models for the study of perinatal asphyxia.

Much of our current understanding concerning the pathophysiology of perinatal brain disorders has evolved from animal studies over the past three decades. Fetal and neonatal nonhuman primate, pregnant sheep, lamb, puppy, piglet and immature rodents, all have been important animal models for perinatal brain research. Although no model can be considered 'perfect' in reflecting the variety and complexity of human brain pathology, the investigator must assess the merits and limitations of each model within the framework of the research questions being asked. A variety of experimental designs have

Severe neonatal asphyxia due to X-linked centronuclear myopathy.

Severe neonatal centronuclear myopathy is inherited as an X-linked condition characterized by primary asphyxia, extreme muscular hypotonia and absent spontaneous movements. We report seven cases from three families to point out the importance of diagnosis with regard to prognosis, outcome and genetic counselling. In hypotonic diseases, analysis of cerebrospinal fluid, electromyography, nerve conduction velocity creatine kinase and a skin biopsy for fibroblast cultures for metabolic investigations are usually carried out. Needle muscle biopsy is an additional valuable investigation to establish

Current and potentially new management strategies for perinatal hypoxic-ischemic encephalopathy.

Thus, the therapeutic dilemma that confronts physicians in the management of hypoxic-ischemic encephalopathy in the newborn infant is not entirely bleak. As new modalities of therapy for hypoxic-ischemic encephalopathy may soon be available for clinical use, it is hoped that the use of prospective therapeutic agents initially will be scrutinized through experimental trials adequate to determine their efficacy in the clinical setting. Only through the conduct of such trials will physicians avoid the pitfalls of routinely treating newborn infants with inappropriate drugs, as has occasionally occ

The role of oxygen free radicals in postasphyxia cerebral hypoperfusion in newborn lambs.

Previous work in a neonatal lamb model has demonstrated abnormalities in cerebral blood flow (CBF) and oxygen consumption (CMRO2) after asphyxia. Immediately after resuscitation, there was a marked increase in CBF and a significant decrease in CMRO2 compared to control. During the late period after asphyxia (30 min to 4 h), both CBF and CMRO2 were significantly depressed. The same postasphyxia model (n = 16) was used to examine the hypothesis that generation of oxygen free radicals during cerebral reperfusion may be involved in the genesis of late postasphyxia hypoperfusion and depressed CMRO2

Diagnosis of birth asphyxia on the basis of fetal pH, Apgar score, and newborn cerebral dysfunction.

Imprecise diagnosis of birth asphyxia coupled with uncertainties about causal factors for neurologic abnormalities in the newborn have greatly fueled the current litigation crisis in obstetrics. Our goal was to more precisely define birth asphyxia based on fetal condition as measured by umbilical artery blood pH, Apgar scores, and neurologic condition of newborns. We selected for study 2738 patients with singleton pregnancies with cephalic presentations who were delivered of infants at term to avoid complications such as prematurity, which may affect infant outcome independent of birth conditi

Birth asphyxia and neurodevelopmental outcome.

Thirty six neonates with severe birth asphyxia (Apgar score less than or equal to 3 at 1 min), 32 with moderate birth asphyxia (Apgar score 4 to 6 at 1 min) and 35 controls (Apgar score greater than or equal to 7 at 1 min) matched for weight and gestation were followed up prospectively for neurodevelopmental outcome. Fetal distress occurred more frequently in babies with severe birth asphyxia when compared to controls (p less than 0.05). Six neonates with severe birth asphyxia had abnormal neurological signs such as delayed sucking, hypo or hypertonia, apneic spell or seizures. Of these, only

Renal function and fluid therapy in high risk infants.

In recent years, the survival rate of high risk infants has markedly increased. The role of such medical management as fluid, electrolyte and nutritional therapy have assumed a greater importance in assuring optimal quality of the survivors. The very low birth weight infants, particularly those with respiratory distress syndrome and perinatal asphyxia, are at highest risk. The inefficient renal function, unique characteristic of body fluid composition and/or presence of severe clinical illness often make the management of fluid and electrolytes in this group of infants difficult. The numerous

Learning of disabled children and their environment.

It has been pointed out that cases of reading disability are rare in Japan due to the specificity of the language structure. Fifty children with learning disabilities were examined both neurologically and psychologically, and they were classified into three groups; the first group had alexia and agraphia (2 cases), 30 cases showing signs of minimal brain dysfunction with choreiform movements of the eyeballs comprised the second group and 18 cases with a history of asphyxia at birth and/or severe jaundice and clumsiness without choreiform movements of the eyeballs formed the third group. The ch

[A variant of pathogenetic therapy in the acute period of perinatal hypoxic-traumatic brain lesions in premature infants].

The restorative potential of reparative cellular processes was studied in 39 immature infants born in hypoxia and receiving a pathogenetic treatment. In addition to the conventional treatment the test group of 24 infants received intravenously 20% piracetam solution, 1% riboflavin-mononucleotide solution, and 0.5% lipoic acid solution beginning from the first days of life. To correct the cellular metabolic processes in a control group of 15 infants, they were given 10% glucose solution, 5% ascorbic acid solution and cocarboxylase. The activity of dehydrogenases in the peripheral blood remained

Association of changes in bombesin immunoreactive neuroendocrine cells in lungs of newborn infants with persistent fetal circulation and brainstem damage due to birth asphyxia.

The pulmonary neuroendocrine (NE) cells, from 16 term infants dying at 1-4 days of age from birth asphyxia, were immuno stained for bombesin-like immunoreactivity by the immunoperoxidase method. The distribution and frequency of bombesin-immunoreactive NE cells were quantified morphometrically and correlated with the presence or absence of brainstem function and persistent fetal circulation (PFC). In infants with loss of brainstem function, the frequency of bombesin immunoreactive NE cells was significantly increased compared to infants with intact brainstem function, i.e. meconium aspiration

Postasphyxial lung disease in newborn infants with severe perinatal acidosis.

The pulmonary course and respiratory management of 65 asphyxiated infants with at least one arterial pH less than or equal to 7.00 within the first 2 hours of life was determined. Asphyxia in the preterm and term infants in the absence of respiratory distress syndrome or meconium aspiration syndrome was associated with a transient respiratory insufficiency requiring assisted ventilation which markedly improved in the first 24 hours of life. In contrast, infants with asphyxia complicated by respiratory distress syndrome or meconium aspiration syndrome developed profound lung disease including p

Changes in the sensitivity to hypoxia and glucose deprivation in the isolated perfused rabbit heart during perinatal development.

The isometric contraction of the isolated rabbit myocardium was measured from 24 days post coitum (dpc) to young adulthood. Tension per gram of heart as developed by the isolated perfused hearts remained constant during late foetal life but increased during the first postnatal week. Sensitivity to hypoxia rapidly increased during foetal life from 26 to 28 days post coitum. In young foetal hearts (up to 28 days post coitum), contraction continued for several hours in the absence of glucose. In contrast, from 28 days post coitum onwards foetal hearts became increasingly dependent on external glu
