First Reported Concomitant Pulsed-Field Ablation and Left Atrial Appendage Occlusion in a Patient With Prior Atrial Septal Defect Closure Device.

Transseptal access in patients with prior atrial septal defect (ASD) closure devices is technically challenging, particularly when large-bore sheaths are required. We report a 59-year-old woman with longstanding persistent atrial fibrillation (AF) and prior ASD closure who underwent concomitant pulsed-field ablation (PFA) and left atrial appendage (LAA) occlusion. Preprocedural cardiac computed tomography identified a narrow (~5 mm) inferior native septal window. A multimodality imaging approach incorporating intracardiac echocardiography, transesophageal echocardiography, fluoroscopy, and CT …

Myocardial Infarction With Non-obstructive Coronary Arteries (MINOCA) Revealing Coronary Embolism From Left Atrial Appendage Thrombus in Rheumatic Valvular Atrial Fibrillation.

Myocardial infarction with non-obstructive coronary arteries (MINOCA) represents a diagnostic challenge with multiple underlying mechanisms, … nch without significant atherosclerotic disease. Transesophageal echocardiography identified a left atrial appendage thrombus, supporting a cardioembolic mechanism. This case emphasizes the importance of considering coronary embolism in MINOCA, particularly in patients with valvular atrial fibrillation, and highlights the role of a structured diagnostic approach i …

Left atrial appendage closure versus anticoagulation in the management of atrial fibrillation: a systematic review, meta-analysis, and meta-regression analysis.

BACKGROUND: Left atrial appendage closure (LACC) is a nonpharmacological option for individuals with atrial fibrillation (AF). LAAC procedure is characterized by its minimally invasive nature and is considered an alternative to anticoagulants that were usually the first line of treatment for patients at risk for blood clotting. METHODS: A literature search of three databases was conducted and relevant randomized controlled trials (RCTs) were selected. A meta-analysis was performed using a random effect model to calculate risk ratios with 95% confidence intervals, and only the articles with th…

Guiding atrial fibrillation ablation combined with left atrial appendage occlusion procedure by fluoroscopy with or without transesophageal echocardiography achieved comparable outcomes.

BACKGROUND: Atrial fibrillation (AF) is the most common arrhythmia and can be treated with catheter ablation (CA) combined with left atrial appendage occlusion (LAAO). The study is designed to compare the safety and efficacy of guiding the combined procedure by digital subtraction angiography (DSA) with or without transesophageal echocardiography (TEE). METHODS: From February 2019 to December 2020, 138 patients with nonvalvular AF who underwent CA combined with LAAO procedure were consecutively included, and two cohorts were built according to intraprocedural guidance (DSA or DSA with TEE). P…

Thoracoscopic stand-alone appendectomy for atrial tachycardia originating from the left atrial appendage in a patient with severe left ventricular dysfunction.

Focal atrial tachycardia (AT) originating from the left atrial appendage (LAA) is one of the rare supraventricular tachycardias and is likely to cause arrhythmia-induced heart failure. Surgical treatment could be an alternative therapy because antiarrhythmic drugs and catheter ablation therapy to focal AT originating from the distal portion of the LAA is still challenging. We report a case of successful operation of minimally invasive thoracoscopic appendectomy in a patient with poor left ventricular (LV) function due to drug-resistant AT originating from the LAA for the first time. A 51-year-…

Evidence of relevant electrical connection between the left atrial appendage and the great cardiac vein during catheter ablation of atrial fibrillation.

BACKGROUND: Atrial fibrillation (AF) triggers within the coronary sinus (CS)/great cardiac vein (GCV) and the left atrial appendage (LAA) have been recognized as nonpulmonary vein triggers of AF. OBJECTIVE: The aim of this study was to describe an electrical connection between the LAA and CS/GCV and its importance in achieving LAA electrical isolation (LAAEI). METHODS: A total of 488 consecutive patients undergoing catheter ablation for persistent or long-standing persistent AF who showed firing from the LAA and/or from the CS/GCV were enrolled in this multicenter prospective study. In all p…

Incidence, Characteristics, and Clinical Course of Device-Related Thrombus After Watchman Left Atrial Appendage Occlusion Device Implantation in Atrial Fibrillation Patients.

OBJECTIVES: This study investigated characteristics and clinical impact of device-related thrombus formation after Watchman device implantation in atrial fibrillation (AF) patients. BACKGROUND: Left atrial appendage occlusion using the Watchman device is an effective alternative to anticoagulation for stroke prevention in AF patients. However, device-related thrombus formation remains an important concern after Watchman implantation. METHODS: From 2006 to 2014, 119 consecutive AF patients underwent Watchman implantation. Transesophageal echocardiographic (TEE) follow-up was scheduled at 45 d…

Left Atrial Appendage Occlusion in Patients With Atrial Fibrillation and Previous Major Gastrointestinal Bleeding (from the Amplatzer Cardiac Plug Multicenter Registry).

History of major gastrointestinal (GI) bleeding may represent a frequent clinical indication for left atrial appendage occlusion (LAAO) in patients with non-valvular atrial fibrillation (AF). This study aims to investigate the procedural safety and long-term outcome of patients with previous major GI bleeding (MGIB) who underwent LAAO. Data from the Amplatzer Cardiac Plug multicenter registry on 1,047 patients were analyzed. Patients with previous MGIB as indication for LAAO were compared with patients without previous MGIB. A total of 151 patients (14.4%) with previous MGIB were identified. P…

Catheter ablation of symptomatic postoperative atrial arrhythmias after epicardial surgical disconnection of the pulmonary veins and left atrial appendage ligation in patients with atrial fibrillation.

OBJECTIVES: Minimally invasive thoracoscopic epicardial pulmonary vein isolation (MIPI) has an important role in the surgical treatment of atrial fibrillation (AF). However, the management of recurrent atrial arrhythmias after MIPI and long-term success rate of catheter ablation have not been well studied. METHODS: Electrophysiological study was performed in 23 patients, 378 ± 282 days after MIPI surgery, because of recurrent symptomatic atrial arrhythmias. RESULTS: A total of 20 patients presented with paroxysmal and persistent AF, 2 patients had a combination of AF and atrial tachycardia (

Left Atrial Cavity Thrombus and Fatal Systemic Embolization in a Stroke Patient with Nonvalvular Atrial Fibrillation: A Caveat against Left Atrial Appendage Closure for Stroke Prevention.

An 88-year-old male with nonvalvular atrial fibrillation (NVAF) and severe congestive heart failure (HF), was admitted to the Neurological Intensive Care Unit because of the acute onset of aphasia and left hemiplegia. Transthoracic echocardiography revealed a left atrial (LA) cavity thrombus. Its "fatal" distal embolization to abdominal aorta occurred in a few days. These observations should lead to a cautious approach in proposing a percutaneous closure of LA appendage in older NVAF patients, with HF and/or left ventricular dysfunction and larger LA volumes, who are not adequately anticoagula

Pathophysiologic correlates of thromboembolism in nonvalvular atrial fibrillation: I. Reduced flow velocity in the left atrial appendage (The Stroke Prevention in Atrial Fibrillation [SPAF-III] study).

Stroke associated with atrial fibrillation (AF) is mainly due to embolism of thrombus formed during stasis of blood in the left atrial appendage (LAA). Pathophysiologic correlates of appendage flow velocity as assessed by transesophageal echocardiography (TEE) in patients with AF have not been defined. To evaluate the hypothesis that reduced velocity is associated with spontaneous echocardiographic contrast and thrombus in the LAA and with clinical embolic events, we measured LAA flow velocity by TEE in 721 patients with nonvalvular AF entering the Stroke Prevention in Atrial Fibrillation (SPA…

Robotic left atrial appendage ligation: a safe strategy for a frail octogenarian with recurrent bleeding.

Atrial fibrillation remains a challenging condition to treat, especially in older patients with multiple comorbidities. Anticoagulation may lead to life-threatening bleeding and therefore might not be possible in high-risk patients. We report the case of a successful robotic-assisted left atrial appendage closure in such a patient, which proved to be an effective treatment strategy and provided a rapid recovery.

Beyond Echocardiography: The Evolving Role of Imaging in Left Atrial Appendage Closure in the Modern Era.

Left atrial appendage closure (LAAC) has emerged as a crucial alternative for stroke prevention in patients with atrial fibrillation ineligible for long-term anticoagulation. While echocardiography, particularly transesophageal echocardiography (TEE), remains fundamental for procedural planning and guidance, the integration of multimodal imaging is increasingly shaping contemporary practice. This review explores the evolving landscape of imaging for LAAC, highlighting their complementary roles, technological advancements, and future directions in optimizing procedural outcomes. TEE is the stan…
