[Supraventricular tachycardia due to simultaneous conduction in the rapid and slow nodal pathways. Treatment by ablation of the slow pathway].

The authors report the case of a 70 year old woman with frequent attacks of supraventricular tachycardia resistant to antiarrhythmic therapy. The tachycardia was irregular with predominantly normal QRS complexes. Electrophysiological investigation showed dual conduction in the atrioventricular node and tachycardia was induced by atrial extrastimulus. However, reentrant tachycardia could not be induced, the refractory period of the slow pathway being much longer than that of the rapid pathway. The mechanism of the tachycardia was simultaneous conduction of the sinus rhythm through the two nodal…

[Trypanosomiasis presenting with trypanids and complicated by myopericarditis (author's transl)].

In a 26-year-old man who had spent time in a trypanosome endemic zone, lesions of erythema marginatum progressing by acute exacerbations for 2 years despite various forms of treatment led to the discovery of a multiple lymphadenopathy and a greatly accelerated sedimentation rate. These findings suggested a diagnosis which was confirmed by the discovery of Trypanosoma gambiense in the blood and lymph node aspirate. In addition, there was an albumino-lymphocytic reaction in the cerebrospinal fluid. Finally, an atrioventricular block was discovered. All these symptoms and signs responsed to treat

Concomitant ablation of atrial fibrillation: New pacemakers and early rhythm recovery.

OBJECTIVE: New permanent pacemaker (PPM) implantation after concomitant atrial fibrillation (AF) ablation has been associated with surgical ablation (SA). We sought to determine factors for PPM use as well as early rhythm recovery. METHODS: From 2004 through 2019, 6135 patients underwent valve surgery and were grouped: No AF (n = 4584), AF no SA (n = 346), and AF with SA (n = 1205) to evaluate predischarge PPM and 3-month rhythm recovery (intrinsic heart rate >40 beats per minute). RESULTS: Overall, 282 (4.6%) patients required a predischarge PPM: atrioventricular node dysfunction in 75.3%, …

[Atrial flutter with 1/1 nodo-ventricular conduction with amiodarone. From physiopathology to diagnosis].

Atrial flutter with 1/1 nodo-ventricular conduction is a classical complication of Vaughan-Williams's Class I antiarrhythmic drugs. The increase of the flutter cycle and weak action of the antiarrhythmic on the atrioventricular node leads to 1/1 conduction of atrial depolarisation to the ventricles. In view of their marked action on the atrioventricular node, this type of pro-arrhythmic effect is very unexpected with Class III antiarrhythmics. The authors report 7 cases of 1/1 atrial flutter with oral amiodarone observed between 1994 and 2001. The patients were 6 men and 1 woman with an averag…

A prospective evaluation of the impact of individual RF applications for slow pathway ablation for AVNRT: Markers of acute success.

BACKGROUND: Catheter ablation is highly effective for atrioventricular nodal re-entrant tachycardia (AVNRT). Generally junctional rhythm (JR) is an accepted requirement for successful ablation however there is a lack of detailed prospective studies to determine the characteristics of JR and the impact on slow pathway conduction. METHODS: Multicentre prospective observational study evaluating the impact of individual radiofrequency (RF) applications in typical AVNRT (slow/fast). Characteristics of JR during ablation were documented and detailed testing was performed after every RF application

Tachycardia after pacemaker implantation in a patient with complete atrioventricular block.

The atrioventricular (AV) node allows ante- and retrograde conduction between atria and ventricles. It is commonly assumed that these AV nodal conduction properties go hand in hand. However, ante- and retrograde AV conduction can be completely independent from each other in individual patients. We report about a patient with permanent AV block III degrees requiring implantation of a pacemaker. As soon as a dual-chamber device was connected to the implanted leads, a tachycardia started at the maximum tracking rate, which was subsequently reprogrammed from 120 to 170 bpm. Non-invasive electrophy

The relationship between refractory period and conduction time of the AV node under various cycle lengths in the canine heart. The significance of wavelength and excitable gap in reentrant tachycardia.

To evaluate the significance of the refractory period and conduction time of the AV node for reentrant tachycardias incorporating the AV node, the effective refractory period of the AV node (ERP) and AH interval (AH) during atrial pacing at various cycle lengths were measured in 12 anesthetized dogs after verapamil administration. ERP-AH tended to be constant in each dog at different cycle lengths and increased as the AV node was suppressed. In reentrant tachycardia incorporating the AV node, assuming that the conduction time outside of the AV node is constant, the excitable gap decreases as E

[First experiences with the effect of low-energy electric impulse on the atrioventricular junction in various types of paroxysmal supraventricular tachycardia].

Efficient treatment of paroxysmal supraventricular reciprocal tachycardias (nodal and orthodromal) was studied in 29 patients by applying low-energy discharges (mean 2.3 +/- 0.1 kV) to the area of the atrioventricular junction. Late results of the treatment were assessed 7-22 months later. Cessation of tachycardia paroxysms was observed in 10 patients, their lower frequency (by more than 50%) was seen in 14 cases. The value of discharge energy, their numbers, relations between potential amplitudes produced by atria and His' bundle, feasibility of paroxysm induction, presence or absence of bund

[Concept of the polyfascicular structure of the atrioventricular junction in patients with reciprocal paroxysmal nodal tachycardias].

Intracardiac electrophysiologic studies were carried out in 41 patients with paroxysmal supraventricular tachycardias. Reciprocal atrioventricular tachycardia due to dissociated conduction through the atrioventricular junction was diagnosed in 29 patients. Electrophysiologic data suggested trifascicular atrioventricular conduction in 4 patients, and quadrifascicular conduction in 2. A concept of polyfascicular impulse conduction through the atrioventricular junction in some cases of reciprocal nodal tachycardia is proposed. It is demonstrated that combinations of congenital cardiac conduction

Electrophysiologic properties of prenalterol.

We assessed the electrophysiological properties of prenalterol, a new beta-selective agonist, in 10 patients with normal and 10 patients with delayed atrioventricular (A-V) conduction times. We evaluated sinus node function, A-V conduction times, refractory periods, atrial or ventricular arrhythmias, spontaneous or induced by the single extrastimulus technique during basal conditions, 5 minutes after a first dose of 20 micrograms/kg of prenalterol, and 5, 15 and 30 minutes after a second injection of the same dose. Prenalterol increased heart rate about 20%, with statistically significant shor

High Dominant Frequencies and Fractionated Potentials Do Not Indicate Focal or Rotational Activation During AF.

BACKGROUND: Dominant frequencies (DFs) or complex fractionated atrial electrograms (CFAEs), indicative of focal sources or rotational activation, are used to identify target sites for atrial fibrillation (AF) ablation in clinical studies, although the relationship among DF, CFAE, and activation patterns remains unclear. OBJECTIVES: This study sought to investigate the relationship between patterns of activation underlying DF and CFAE sites during AF. METHODS: Epicardial high-resolution mapping of the right and left atrium including Bachmann's bundle was performed in 71 participants. We ident

The subtle tine: Asymptomatic Micra perforation incidentally discovered during cardiac surgery.

A 70-year-old woman with longstanding persistent atrial fibrillation underwent Micra leadless pacemaker implantation and atrioventricular nodal ablation. No postprocedural complications were noted. She subsequently underwent surgical mitral valve replacement 4 years later. During the surgery, Micra tine perforation of the right ventricular free wall was seen. No device revision was performed due to her asymptomatic status and stable pacemaker position/function. Pericardial effusion is a known complication of Micra implantation. The incidence of tine perforation is unknown as many patients may

Atrioventricular Junction Ablation for Atrial Fibrillation.

Atrioventricular junction (AVJ) ablation is an effective therapy in patients with symptomatic atrial fibrillation who are intolerant to or unsuccessfully managed with rhythm control or medical rate control strategies. A drawback is that the procedure mandates a pacing system. Overall, the safety and efficacy of AVJ ablation is high with a majority of the patients reporting significant improvement in symptoms and quality-of-life measures. Risk of sudden cardiac death after device implantation is low, especially with an appropriate postprocedure pacing rate. Mortality benefit with AVJ ablation h

Accelerated AV nodal conduction with use of procainamide in atrial fibrillation.

BACKGROUND: Atrial fibrillation is a common dysrhythmia seen in the emergency department (ED). Chemical or electrical cardioversion may be performed on patients who have had atrial fibrillation for < 48 h duration and who are at low risk for thromboembolic events. Multiple studies suggest that intravenous procainamide is an appropriate agent in the treatment of acute atrial fibrillation due to its relatively low risk profile and high conversion rate. OBJECTIVES: A case is presented that demonstrates an adverse reaction to the use of intravenous procainamide for chemical cardioversion of atria
