Transient cortical blindness due to hypertensive encephalopathy. Magnetic resonance imaging correlation.

Striking reversible signal intense magnetic resonance imaging (MRI) lesions were observed in the occipital cortex of a 16-year-old girl who presented with an attack of transient cortical blindness as the initial manifestation of hypertensive encephalopathy (HTE). The lesions were seen to best advantage on T2-weighted imaging and were not visible on computed tomography (CT). It is proposed that such occipital lobe MRI lesions likely reflect extravasation of fluid and proteins across the blood brain barrier, damaged as a consequence of cerebral autoregulation failure.

Onchocerca volvulus DNA probe classification correlates with epidemiologic patterns of blindness.

Onchocerciasis, or river blindness, results from infection with Onchocerca volvulus. The parasite is endemic to West Africa, in both rain forest and savanna bioclimes. Several lines of evidence suggest that different strains of the parasite exist in the rain forest and savanna. Furthermore, epidemiologic evidence indicates that ocular onchocerciasis is most severe in savanna regions. This has led to the hypothesis that there is a strain association with ocular pathology. To test this hypothesis, parasites from villages in which severe and mild onchocerciasis were endemic were classified with t…

[An epidemiological survey of blindness and low vision in Shandong Province].

An epidemiological survey of blindness and low vision was carried out in a sample population of 115 167 in Shandong Province to find that the prevalence of binocular blindness was 0.34%, that of unilateral blindness 0.65%, that of binocular low vision 0.46%, and that of unilateral low vision 0.64%. The prevalence rates increased with age and the leading causes of blindness, in order of importance, were cataract, corneal diseases and glaucoma in urban areas, and cataract, corneal diseases and retinal diseases in rural areas.

[Hospital study of the major causes of blindness in Western Cameroon].

The authors study the main causes for blindness in the Western Province thanks to the hospital data from the only Ophthalmology service of this region. When checking the files of the 204 patients the main blindness causes are (in decreasing order): cataracts, glaucomas, optical nerve atrophy, uveitis, onchocerciasis.... This study also shows the absence of trachoma as a blindness cause in this tropical area. These data could be used to plan an ophthalmological cure strategy waiting for the put into service of a blindness prevalence survey in the field.

[An epidemiological survey of blindness and low vision in Hebei Province].

Following the national unified criteria and methods, an epidemiological survey of blindness and low vision was carried out in Hebei Province from April to June, 1987. In 77,948 persons examined, there were 291 cases of bilateral blindness, representing a prevalence of 0.37% (male 0.29%, female 0.45%, urban areas 0.19%, plain areas 0.42%, and mountainous areas 0.36%). The prevalences of monocular blindness, bilateral and monocular low vision were 0.72%, 0.57% and 0.30% respectively. Subjects over 60 years of age had significantly higher prevalences of bilateral blindness and low vision. The chi…

Causes of blindness and visual handicap in the Central African Republic.

The causes of bilateral blindness (best visual acuity less than 3/60) in 1371 people in the Central African Republic seen between 1985 and 1989 who attended eye clinics in 10 out of the 16 prefectures across the country are given. The main causes of bilateral blindness were cataract (51%), glaucoma (12.7%), and onchocerciasis (8.1%). In 710 patients with unilateral blindness the main causes were cataract (38%), glaucoma (10%), iritis (7.5%), and trauma (6.3%). Bilateral blindness in children was rarely seen. The causes of visual impairment (vision between 6/18 and 3/60) in 424 patients were ca…

Encephalopathy, deafness and blindness in young women: a distinct retinocochleocerebral arteriolopathy?

Three young women (aged 18 years, 19 years and 19 years) who developed progressive neuropsychic and neurologic disturbances with hearing loss and multifocal retinal artery branch occlusions are reported. This retinocochleocerebral syndrome has been reported previously only in 12 young North American women. Its pathogenesis is unknown, but an atypical viral infection of the vessel walls has been suggested. Abnormalities of T lymphocytes subsets in blood in one of the patients suggested an immunological dysfunction, but all other tests, including immunological reactions on brain and skin biopsie

Transient blindness following mild head trauma. Criteria for a benign outcome.

A series of seven children in Hawaii experienced transient cortical blindness following mild head trauma. All children, ages 3 through 8, recovered fully. The most prominent clinical feature was initial restlessness and agitation following relatively mild head trauma without significant loss of consciousness (LOC). One child may have experienced this several times. The clinical features associated with a benign outcome in this syndrome include: pediatric age group, mild head trauma, brief or no LOC, onset of blindness occurring within hours of the head injury, absent optokinetic nystagmus, dur…

A case of sudden retinal artery occlusion and blindness in pregnancy.

Central retinal artery occlusion is generally a disease of old age associated with loss of vision in one eye. In older patients, it is usually secondary to emboli from atherosclerotic plaques; in younger patients valvular heart disease is a common precursor. Several other conditions have been associated with central retinal artery occlusion, but to our knowledge, there have been no previously reported cases with pregnancy as the sole associated factor. Such a case is presented.

Optic nerve trauma with prolonged blindness followed by visual-evoked potential.

An accidental deep intraorbital penetration of a radio antenna tip damaged the optic nerve and caused immediate amaurosis in a 19-year-old female with normal funduscopic, electroretinographic and computerized tomographic orbital findings. A small visual-evoked potential (VEP) recorded within 2 h of injury and in the following days revealed persistent electrical nerve conduction despite loss of vision and afferent pupillary reaction. The VEP constituted the only anticipating sign for the visual recovery which occurred after 3 days. Further increase of the VEP amplitudes preceded the visual impr

Blindness and convulsions associated with vitamin A deficiency in feedlot steers.

Blindness and convulsions were the predominant clinical signs in a group of feedlot steers inadvertently fed a vitamin-A deficient diet. Although vitamin A had been added to the feed at the time of preparation, it was believed that the vitamin A had been destroyed by heat, humidity, and minerals mixed in the feed. Feedlot heifers fed the same high-grain diet were clinically normal, and one heifer from which a blood sample was obtained had normal serum vitamin A content. The ingestion of sparse grasses and weeds found along the fence row of the heifers' pens was thought to have provided suffici…

Common causes of blindness: a pilot survey in Brooklyn, New York.

The causes of blindness in 105 consecutive legally blind patients were analyzed in a large urban multidisciplinary medical center. It was found that the leading causes of blindness, in order of frequency of incidence, were glaucoma, diabetic retinopathy, senile macular degeneration, and cataract. The periodic collection of statistics on the relative frequency of the causes of blindness is strongly suggested so that possible changes and improvements in health care may be determined.

Computed tomographic evaluation of the optic canal in sudden traumatic blindness.

Using computed tomographic scans and reformatted images of the optic canal region, we were able to define osseous abnormalities in nine patients with sudden traumatic blindness resulting from indirect trauma to the optic nerve. Six patients had optic canal fractures, two had fractures of adjacent structures, and one patient did not have a fracture. Patients received corticosteroid therapy, surgical decompression of the optic canal, both types of treatment, or no specific treatment for the nerve injury. Although there was no correlation between the abnormality identified by computed tomography …

[Prosopagnosia and disturbance of color recognition during recovery from cortical blindness].

Neuropsychologic findings during recovery from cortical blindness are described in four right-handed patients (two female, two male) aged 53 to 70 years. The lesions were due to occlusion of both posterior cerebral arteries (Case 1) and vascular spasm following subarachnoidal hemorrhage (Case 3) or angiography (Cases 2 and 4). Complete blindness lasted for 3 months until death in Case 1, 4 weeks in case 2, and 1 to 2 days in Cases 3 and 4. Confusional states and visual hallucinations were pronounced in three cases. Anosognosia (Anton's syndrome) was most pronounced in a patient with occlusion …

Autosomal recessive syndrome of pseudogliomantous blindness, osteoporosis and mild mental retardation.

We report a sibship of three sisters and two brothers who showed osteoporosis of variable severity; the propositus has incapacitating deformities following numerous fractures. Four of the sibs, including three with frequent fractures, were blind from infancy ("pseudogliomatous blindness"). In addition, two were mentally retarded. The osteoporosis-pseudoglioma syndrome is inherited as an autosomal recessive trait; similar reports from the literature support this assumption.
