Capillary stalling after traumatic brain injury: an emerging microcirculatory contributor to secondary brain injury.

Secondary brain injury after traumatic brain injury (TBI) has traditionally been understood and managed through the lens of intracranial pressure, cerebral perfusion pressure, and global cerebral blood flow. Yet a substantial proportion of patients continue to exhibit persistent tissue hypoxia and metabolic crisis even when these macrocirculatory parameters appear adequately controlled. Capillary stalling-the transient or sustained interruption of red blood cell transit within individual or focal capillary networks-may represent an under-recognized microcirculatory disorder that conventional m…

Multi-organ imaging genetics: towards an integrated view of brain, body and brain diseases.

Population-based cohorts that combine genetic data with whole-body multimodal imaging are transforming how we map the genetic architecture of the brain in its broader bodily context. Historically, imaging genetics has focused largely on single organs, especially the brain, with separate efforts targeting the heart, skeleton and other organs for systemic diseases, and limited integration across organ systems. Yet, strong brain-body interactions and the high multimorbidity of brain and systemic diseases highlight the need for a brain-centred multi-organ perspective. Here we introduce the emergin…

Quadra-Brain: Quadruplet Higher-Order Topological Representation Learning for fMRI-Based Brain Disorder Diagnosis.

Accurately characterizing higher-order coordination among distributed brain regions is important for understanding brain disorders from fMRI data. Existing methods mainly rely on pairwise connectivity or low-order motifs, which may overlook disease-related alterations in collective multi-region communication and higher-dimensional topological organization. To address this limitation, we propose Quadra-Brain, a computational framework for modeling quadruplet higher-order interactions in fMRI-based brain disease diagnosis. Quadra-Brain first estimates time-resolved quadruplet co-fluctuations usi…

The split homunculus: How in vivo brain parcellation provides new perspectives on brain health.

Body perception and movement control are altered in most neurological, neurodegenerative, and psychiatric diseases. The homuncular organization of the sensorimotor cortex, referring to the regular organization of neurons responsive to body movement and body touch, is therefore of fundamental relevance for brain health. Developmental changes of homuncular readouts, such as during aging and neurodegeneration, relate to mechanisms of resilience against cognitive, emotional, and behavioural decline. Recent advances in ultra-high field MRI allow the in-vivo parcellation of cortical areas. Based on …

Accelerated brain aging in Veterans with posttraumatic stress symptoms and mild traumatic brain injury.

Exposure to trauma and mild traumatic brain injury (mTBI), which often co-occur and can both trigger acute and chronic pathophysiological processes, have been linked to brain changes indicative of accelerated neurological aging. However, few longitudinal studies have examined the impact of these factors over time or characterized how the chronicity and severity of associated symptoms affect aging trajectories. This study included individuals (N = 79) who were exposed to psychological trauma, with most of the cohort additionally having sustained a mTBI. Participants completed structural MRI sca…

Digital Twin Brain simulation and manipulation of a functional brain network underlying mental illness.

Linking synaptic-level perturbations to distributed brain-network dynamics remains a central challenge for understanding and treating mental illness. Although recent whole-brain models can reproduce individual brain activity patterns, they largely function as descriptive simulators rather than mechanistic, intervention-capable systems. Here we present an intervention-capable digital twin of the human brain, integrating individual neuroanatomy and task-evoked dynamics within a neuronal-scale framework. Individualised digital twin brains recapitulate a participant-specific compact cortico-subcor…

Proactive Brain Screening Using Contrast-Enhanced Brain CT Scans in HER2+ Metastatic Breast Cancer.

PURPOSE: According to recent EANO-ESMO guidelines, proactive brain imaging can be considered in asymptomatic patients with HER2+ metastatic breast cancer (mBC) because of high risk of developing brain metastases. However, optimal imaging modality and timing remain unclear. We retrospectively assessed the impact of contrast-enhanced CT screening on symptomatic brain metastases in patients with HER2+ mBC. EXPERIMENTAL DESIGN: Consecutive patients newly diagnosed with HER2+ mBC treated with trastuzumab-pertuzumab plus taxane (2014-2024) were retrospectively identified. Brain screening was define…

Comparison of brain volumes in episodic and chronic migraine using automated whole-brain volumetry.

Migraine chronification is associated with increased clinical burden, yet structural neuroimaging findings remain inconsistent. We compared global and regional brain volumes among patients with episodic migraine (EM), chronic migraine (CM), and healthy controls (HC) using an automated whole-brain volumetric approach. This retrospective study included 58 CM patients, 55 EM patients, and 60 age- and sex-matched HC. Diagnoses were established according to the International Classification of Headache Disorders, 3rd edition (ICHD-3). Structural MRI data acquired on a 1.5-T scanner were processed us…

Parkinson's disease beyond the brain: erythrocyte α-synuclein transfer across the blood-brain barrier.

Parkinson's disease is characterized by the accumulation and propagation of α-synuclein pathology in the central nervous system, yet the contribution of peripheral α-synuclein sources remains unclear. Here, we identify erythrocytes as an important reservoir of α-synuclein and demonstrate that bone marrow-derived erythrocytic α-synuclein likely contributes to brain pathology and Parkinson's disease-related neurodegeneration. Using human tissues and mouse models, we show that erythrocytes harbour abundant α-synuclein species. Bone marrow transplantation revealed widespread distribution of bone m…

Yeast β-glucan alleviates alcohol-related brain injury by restoring gut-brain axis homeostasis.

This study investigated the protective effects of yeast β-glucan (YBG) against alcohol-related brain injury (ARBI), with a focus on potential mechanisms mediated by the gut-brain axis. Our findings revealed that YBG supplementation significantly alleviated alcohol-induced anxiety-like behaviors and cognitive deficits in mice. In the brain, YBG mitigated oxidative stress, reduced neuroinflammation, and ameliorated neurotransmitter dysregulation. In the gut, YBG protected the alcohol-damaged intestinal barrier, upregulating the expression of tight junction proteins (Claudin-1, Occludin, and ZO-1…

Intranasal Biodegradable Nanomedicine for Epilepsy Management: Targeting the Brain Beyond the Blood-Brain Barrier.

Epilepsy is a common chronic neurological disorder characterized by recurrent, unprovoked seizures arising from abnormal neuronal hyperexcitability and hypersynchronous electrical activity within the brain. Despite advances in antiseizure medications, effective epilepsy management remains challenging because of pharmacoresistance, limited blood-brain barrier (BBB) permeability, inadequate intracerebral drug accumulation, and systemic toxicity. Moreover, currently available therapies primarily provide symptomatic seizure control without addressing the fundamental pathological processes involved…

A brain-to-bone marrow circuit drives neutrophil mobilization and exacerbates ischaemic brain injury.

Ischaemic stroke triggers a detrimental inflammatory response, characterised by neutrophil mobilisation from the bone marrow to the injured brain. The neural circuits driving this response are poorly understood. Here we identify a brain-to-bone marrow axis that governs this process. We used transient middle cerebral artery occlusion, magnetic resonance imaging, neurological behavioural assessments and infarct volume analysis to establish and evaluate ischaemic brain injury. In addition, viral tracing, optogenetic and chemogenetic manipulation, chemical and surgical interruption of sympathetic …

Surface functionalized albumin nanoparticles of palbociclib with amplified brain delivery for treating brain glioblastoma.

Glioblastoma multiforme (GBM) has a global occurrence of 0.59 to 3.69 per one lakh people. Palbociclib (Palbo), an inhibitor of CDK4 (cyclin dependent kinase) and CDK6, is used for treating cancer. Palbo showed a limited therapeutic effect in brains of transgenic mice after oral administration. Further, studies on P-glycoprotein (P-gp) and breast cancer resistant protein (BCRP) knock-out mice showed that P-gp and BCRP (efflux transporters) restrict Palbo's permeability across the blood-brain barrier (BBB). Hence, the clinical usefulness of Palbo for treating GBM is limited. Palbociclib-loaded …

Compositional brain scores capture Alzheimer's disease-specific structural brain patterns along the disease continuum.

INTRODUCTION: Traditional multivariate methods for neuroimaging studies overlook the interdependent relationship between brain features. This study addresses this gap by analyzing relative brain volumetric patterns to capture how Alzheimer's disease (AD) and genetics influence brain structure along the disease continuum. METHODS: This study analyzed data from participants across the AD continuum from the Alzheimer's and Families (ALFA) and Alzheimer's Disease Neuroimaging Initiative (ADNI) studies. Compositional data analysis (CoDA) was exploited to examine relative brain volumetric variation…

Brain-computer-interface-based intervention increases brain functional segregation in cognitively normal older adults.

Brain-computer interface (BCI)-based cognitive training systems have shown promise in enhancing cognitive performance in cognitively normal older adults. However, the brain network changes underlying these behavioural improvements remain poorly understood. To address this gap, we investigated topological alterations in intrinsic brain functional networks following BCI-based training and their behavioural relevance in cognitively normal older adults using resting-state functional magnetic resonance imaging and graph theoretical analysis. Compared to a non-intervention waitlist (WL) group, the i…

Overall survival following whole-brain radiotherapy for brain and leptomeningeal metastases from solid tumors.

OBJECTIVES: Brain and leptomeningeal metastases are relatively common complications of advanced solid tumors, occurring in an estimated 20%-40% of cases and significantly reducing survival and quality of life. Despite advancements in systemic therapies and stereotactic radiosurgery, whole-brain radiotherapy (WBRT) remains the standard treatment. This study evaluates overall survival (OS) and prognostic factors in patients with brain and leptomeningeal metastases treated with WBRT at a single institution. MATERIALS AND METHODS: We retrospectively analyzed 228 patients with brain and leptomenin…
