Lymphoblastic lymphoma.

Lymphoblastic lymphoma (LBL) is a neoplasm of immature B cells committed to the B-(B-LBL) or T-cell lineage (T-LBL) that accounts for approximately 2% of all lymphomas. Although histological features are usually sufficient to distinguish lymphoblastic from mature B- or T-cell neoplasms, of greater importance for diagnosis is the characterization of immunophenotype by flow cytometry. LBL occurs more commonly in children than in adults, mostly in males. A bone marrow involvement <25% (or 20% according to WHO) formally distinguishes LBL from ALL. The prognosis of LBL has dramatically improved wit

Diffuse leptomeningeal gliomatosis with osteoblastic metastases and no evidence of intraaxial lesions.

An autopsy-proved case of cerebral and medullary leptomeningeal gliomatosis and diffuse osteoblastic metastases without evidence of intraaxial tumor is described. MR findings included diffuse thickening of the cerebral and medullary leptomeninges on T1-weighted, proton density-weighted, and T2-weighted images and abnormal enhancement of the sulci and cisterns of the cerebrum, brain stem, cerebellum, and medulla on postcontrast T1-weighted images. MR also showed several areas of replacement of the normal bone marrow of the skull. No intraaxial lesion was seen.

MR imaging of cerebellopontine angle and internal auditory canal lesions at 1.5 T.

The high-field, thin-section (3-5 mm) MR imaging characteristics of 49 cerebellopontine angle and internal auditory canal lesions were reviewed. The diverse abnormalities include 20 acoustic neurinomas, eight neurinomas of other cranial nerves (six involving the fifth cranial nerve and two involving cranial nerves IX-XI), seven meningiomas, five subdural fluid collections, four brainstem tumors with exophytic components, three glomus jugulare tumors, one epidermoid tumor, and one basilar artery aneurysm. T1-, T2-, and proton-density-weighted images were obtained in all cases. T1-weighted image

Mixed dysembryoplastic neuroepithelial tumor and ganglioglioma.

We report a case of a 15-year-old girl with new onset seizures, who had a mixed dysembryoplastic neuroepithelial tumor (DNT) and ganglioglioma of the right parieto-occipital lobe. The tumor appeared well demarcated and exhibited a low T1 and a high T2 signal on magnetic resonance imaging. Architecturally it was in large part intracortical and multinodular, but also featured a leptomeningeal component. The former corresponded to DNT, a proliferation of oligodendroglia-like cells (OLCs) arranged in nodules, as well as comprising a diffuse internodular element featuring "floating neurons" in a mu

Association between hippocampal dose and memory in survivors of childhood or adolescent low-grade glioma: a 10-year neurocognitive longitudinal study.

BACKGROUND: Hippocampal avoidance has been suggested as a strategy to reduce short-term memory decline in adults receiving whole-brain radiation therapy (RT). The purpose of this study was to determine whether the hippocampal dose in children and adolescents undergoing RT for low-grade glioma was associated with memory, as measured by verbal recall. METHODS: Eighty patients aged at least 6 years but less than 21 years with low-grade glioma were treated with RT to 54 Gy on a phase II protocol. Patients underwent age-appropriate cognitive testing at baseline, 6 months posttreatment, yearly thro

The oncolytic virus Delta-24-RGD elicits an antitumor effect in pediatric glioma and DIPG mouse models.

Pediatric high-grade glioma (pHGG) and diffuse intrinsic pontine gliomas (DIPGs) are aggressive pediatric brain tumors in desperate need of a curative treatment. Oncolytic virotherapy is emerging as a solid therapeutic approach. Delta-24-RGD is a replication competent adenovirus engineered to replicate in tumor cells with an aberrant RB pathway. This virus has proven to be safe and effective in adult gliomas. Here we report that the administration of Delta-24-RGD is safe in mice and results in a significant increase in survival in immunodeficient and immunocompetent models of pHGG and DIPGs. O

Morphologic characterization of spontaneous nervous system tumors in mice and rats.

Spontaneous rodent nervous system tumors, in comparison to those of man, are less well differentiated. Among the central nervous system (CNS) tumors, the "embryonic" forms (medulloblastoma, pineoblastoma) occur both in rodents and humans, whereas the human "adult" forms (gliomas, ependymomas, meningiomas) have fewer counterparts in rodents. In general, the incidence of spontaneous CNS tumors is higher in rats (>1%) than in mice (>0.001%). A characteristic rat CNS tumor is the granular cell tumor. Usually it is associated with the meninges, and most meningeal tumors in rats seem to be totally o

A multi-task generative model for simultaneous post-contrast MR image synthesis and brainstem glioma segmentation.

OBJECTIVES: This study aims to generate post-contrast MR images reducing the exposure of gadolinium-based contrast agents (GBCAs) for brainstem glioma (BSG) detection, simultaneously delineating the BSG lesion, and providing high-resolution contrast information. METHODS: A retrospective cohort of 30 patients diagnosed with brainstem glioma was included. Multi-contrast images, including pre-contrast T1 weighted (pre-T1w), T2 weighted (T2w), arterial spin labeling (ASL) and post-contrast T1w images, were collected. A multi-task generative model was developed to synthesize post-contrast T1w imag

Pediatric brain stem lesions: introduction of a scoring system for clinical evaluation and their treatment analysis.

INTRODUCTION: Brainstem lesions in pediatric age group include mainly gliomas. As these are not usually amenable to complete surgical resection, multimodality treatments are used. To assess treatment responses, Karnofsky's Performance Score (KPS) has been the gold standard; however, in pediatric patients, comprehensive neurological examination is more valuable. OBJECTIVE: Our purpose was to formulate neurological examination based scoring system and compare its effectiveness with KPS. MATERIALS AND METHODS: A retrospective analysis of 37 pediatric patients with brainstem lesions was performe

Brainstem death.

A patient with the clinical picture of brain death resulting from brainstem hemorrhage and subsequent infarction is presented. The EEG showed activity similar to what has been described in the cerveau isolé animal preparations. Cortical evoked potentials were unobtainable from auditory or somatosensory stimulation, but of unusually high amplitude to flash stimuli. The point is made that a diagnosis of brain death cannot be made on clinical grounds alone when a patient is on life support systems, and the differences between cerebral death, brainstem death and brain death are discussed.

Osteolytic clear cell meningioma of the petrous bone occurring 36 years after posterior cranial fossa irradiation: Case report.

OBJECTIVE AND IMPORTANCE: While bone invasion and hyperostosis are frequent phenomena in meningiomas, primary intraosseous meningiomas are rare and their occurrence in the skull base is an extraordinary exception. Moreover, radiation-induced meningiomas represent a unique clinical dilemma given the fact that patients with these tumors had often received a prior full course of radiotherapy. CLINICAL PRESENTATION: A 42-year-old man presented with a 3-month history of progressively worsening facial asymmetry. His medical history was consistent for a posterior cranial fossa irradiation at the age

Schwannoma with tentorial attachment in the cerebellopontine angle mimicking a meningioma.

Intracranial schwannoma not associated with the cranial nerves is rare. It is also an intriguing neoplasm since the Schwann cell is not native to the central nervous system. To date only four cases of intracranial schwannoma arising from the tentorium have been reported. We present a 49-year-old woman who harboured a schwannoma with a tentorial attachment in the right cerebellopontine angle and describe the relevant clinical, radiological and pathological findings. In addition, we briefly review the main hypotheses for the origin of this neoplasm and highlight its resemblance to meningioma and

Diffusion tensor tractography of pyramidal tracts in patients with brainstem and intramedullary spinal cord tumors: Relationship with motor deficits and intraoperative MEP changes.

PURPOSE: To evaluate whether pyramidal tracts course alterations observed in diffusion tensor tractography (DTT) in cases of brainstem and intramedullary spinal cord tumors reflect patient clinical status and prognosis. MATERIALS AND METHODS: For this purpose, we assessed in 17 patients relationships between pyramidal tracts course alterations observed in DTT (classified into four categories: unaffected; displaced or interspaced; partially disintegrated and completely disintegrated) performed on a 1.5 Tesla scanner and the presence of preoperative motor deficits, changes observed in motor evo

A serologic marker of paraneoplastic limbic and brain-stem encephalitis in patients with testicular cancer.

BACKGROUND: In patients with cancer, symptoms of limbic and brain-stem dysfunction may result from a paraneoplastic disorder. Paraneoplastic limbic or brain-stem encephalitis occurs more frequently with testicular cancer than with most other cancers. We sought antineuronal antibodies that might be used in a diagnostic test for this syndrome. METHODS: Immunohistochemical and immunoblotting techniques were used to detect serum and cerebrospinal fluid antibodies. Serologic screening of a complementary DNA library and Northern blotting were used to clone the target antigen and determine which tis
