Assessing the role of case mix in cesarean delivery rates.

OBJECTIVE: Implicit in comparisons of unadjusted cesarean rates for hospitals and providers is the assumption that differences result from m … and the presence of obstetric conditions influencing the risk of cesarean delivery (multiple birth, breech presentation or transverse lie, preterm, no trial of labor for a medical indication). We determined the percent of women in each parity-obstetric condition subgroup and calculated a standardized cesarean rate for the hospital-based practice using the case mix of …

[Posterior fossa subdural hematoma in the newborn caused by birth injury].

Among various cases of intracranial hemorrhage in the newborn caused by birth injury, posterior fossa subdural hematoma is of serious nature … urological deficits. The second case is a full-term (40 weeks gestation) twin female infant. Due to breech presentation, delivery was conducted per breech extraction. At three days of age, vomiting, fontanel bulging and hypotonia were observed. CT scan revealed hematoma similar to that seen in the first case, and steroid and glycerol were infused intravenously. Conse …

Prenatal hydrometrocolpos as an unusual finding in Fraser syndrome. Case report.

OBJECTIVES: Fraser syndrome is a rare congenital malformation characterized by cryptophthalmos, syndactyly and urogenital tract malformations. The association with hydrometrocolpos is infrequent, with only a few cases reported in the literature. CASE PRESENTATION: A 19-year-old primigravida presenting at 35 weeks of gestation, with prenatal finding of hydrometrocolpos associated with hypotelorism and microphthalmia. Pre-term cesarean delivery was performed due to breech labor and perinatal death. The autopsy confirmed hydrometrocolpos secondary to vaginal atresia and imperforate hymen, associ

Rare case of a newborn baby with left-sided Erb's palsy and a contralateral/right-sided paralysis of the diaphragm.

Brachial plexus birth injury (BPBI) and phrenic nerve injury can sometimes occur concurrently in neonates following difficult deliveries like breech presentation, shoulder dystocia, forceps or vacuum extraction. Phrenic nerve palsy should be suspected in a newborn with respiratory distress and an elevated hemidiaphragm on the imaging studies in presence of the associated risk factors. The right side is affected more often than the left side and most of it is associated with BPBI. We present here a rare case of a newborn baby with a left-sided Erb's palsy and a contralateral/right-sided diaphra…

Emergency cesarean delivery in primigravida with portal hypertension, esophageal varices, and preeclampsia.

The incidence of cirrhosis and advanced portal hypertension during pregnancy is very low, and the literature is scarce with regard to the anesthetic management of a parturient with this coexisting disease. We report the successful perioperative management of a parturi- ent with a history of cirrhosis and portal hypertension with esophageal varices and mild preeclampsia who presented at 38 weeks' gestation in active labor with a breech presentation requiring emergency cesarean delivery. She required endoscopic esophageal varices banding during the second trimester of pregnancy. After correction…

The objective of this review was to assess the evidence that supports cesarean delivery for fetal indications. The main fetal reasons for ab … e sensitive tools to assess fetal well-being in labor and practices to deliver or reduce fetuses in breech presentation at term have the potential for greatest impact on the overall primary cesarean delivery rate.

Prevalence and determinants of caesarean section in Jamaica.

The prevalence and determinants of primary caesarean section in Jamaica were estimated from a survey of women aged 14-49 years. Among 2328 women reporting 2395 live hospital births during the period January 1984 to May 1989, the prevalence of caesarean section was 4.1%. Repeat caesarean sections accounted for 1.3% of the hospital births during that period. Of the medical complications studied, prolonged labour and/or cephalopelvic disproportion carried the highest risks of primary caesarean section, followed by breech presentation, maternal diabetes, a high birth-weight baby, maternal hyperten…

[Early detection of hip dysplasia in the neonatal period].

Several risk factors are associated with an increased incidence of congenital hip dysplasia. Since the introduction of hip sonography the understanding of correlation between risk factors and US have been studied to decrease the prevalence of the deformity. Regression analysis and chi-square test were used to study the incidence of factors associated with congenital hip dysplasia. Positive family history, breech presentation, positive Ortolani manoeuvre, abduction deficit, asymmetric skin folds, feet or neuromuscular abnormities were compared to ultrasound examination. No significant correlati…

[Epidemiologic study of visible congenital malformations in Zaire].

This retrospective study over an 11-year period revealed 79 cases of visible congenital malformations in a total of 64,777 deliveries (prevalence 1.2%). No aetiological feature was demonstrated. In contrast to data from the literature, the maternal age most involved was between 20 and 35 years and the pregnancy affected most often appeared to be the first. A past history of malformations and miscarriage was rare and difficult to assess because of the retrospective nature of the study. The commonest special features of the pregnancy were breech presentation (present in almost all cases of crani…

Cesarean section in twin pregnancies in two Danish counties with different cesarean section rates.

OBJECTIVE: Based on a comparison of the clinical indications for cesarean section (CS) in two Danish counties and a review of the literature … haracteristics. Different attitudes were found towards CS in cases with previous CS, with twin A in breech presentation and in cases with vertex-breech deliveries. These differences could explain less than two thirds of the overall 29% (CI: 12-46%) difference in risk of CS between the two counties, indicating more subtle reasons for the discrepancy. No difference bet …

Optimal time intervals for vaginal breech births: a case-control study.

BACKGROUND: Breech births are associated with a high rate of hypoxic injury, in part due to cord occlusion during emergence. Maximum time intervals and guidelines oriented toward earlier intervention have been proposed in a Physiological Breech Birth Algorithm. We wished to further test and refine the Algorithm for use in a clinical trial. METHODS: We conducted a retrospective case-control study in a London teaching hospital, including 15 cases and 30 controls, during the period of April 2012 to April 2020. Our sample size was powered to test the hypothesis that exceeding recommended time lim

Determinants of pain perception after external cephalic version in pregnant women.

OBJECTIVE: A considerable proportion of pregnant women with a fetus in breech position refuses external cephalic version (ECV), with fear of pain as important barrier. As a consequence, they are at high risk for caesarean section at term. The current study investigated determinants of pain perception during ECV, with special attention to maternal mental state such as depression and fear of ECV. DESIGN: Prospective study of 249 third-trimester pregnant women with breech position with a request for an ECV attempt. SETTING: Department of Obstetrics and Gynaecology in a large teaching hospital i

Shoulder dislocation in a very-low-birth-weight infant: case report and review of the literature.

Shoulder dislocation in a preterm infant is very rare. We report a case of a preterm infant, with gestational age of 25 2/7 weeks and birth weight of 910 g, with anterior shoulder dislocation after breech delivery. The infant was successfully treated with closed reduction. In general, long-term outcome of shoulder dislocation in these infants is good. However, prompt recognition and treatment of the dislocated shoulder are essential to maintain normal shoulder girdle function and to prevent functional disability.

Mothers' views of their childbirth experience two years after term breech delivery.

INTRODUCTION: This study was performed to evaluate mothers' views of their childbirth experience two years after term breech delivery. METHODS: Two years after delivery mothers were asked to fill out a questionnaire concerning their breech birth experience and their view about the care provided to them while giving birth. Outcomes of the planned cesarean section (CS) group were compared with outcomes of the planned vaginal delivery (VD) group, whether or not a vaginal birth was realized or an emergency cesarean section was performed. Any differences were further analyzed by use of logistic re

Factors associated with adverse perinatal outcome in the Term Breech Trial.

BACKGROUND: In the Term Breech Trial, the risk of adverse perinatal outcome was lower with planned cesarean section versus planned vaginal birth. We undertook secondary analyses to determine factors associated with adverse perinatal outcome. STUDY DESIGN: By using multiple logistic regression analyses, we determined the effect of prelabor cesarean section, cesarean section during early labor, cesarean section during active labor versus vaginal birth, and other factors, on adverse perinatal outcome. For 1384 fetuses delivered after labor, we determined the effect of variables associated with l

[Ultrasonographic diagnosis of an epiphyseal detachment of the upper end of the humerus due to birth injury].

Injuries to the proximal humerus in infants are often missed or misinterpreted because of the non ossification of the epiphysis. Ultrasound allows direct visualisation of the proximal humeral epiphysis, metaphysis, joint space, and relationship with the glenoid cavity. Ultrasound diagnosis of other epiphyses birth trauma has been reported. Ultrasound should be considered as the first imaging modality if traumatic epiphyseal dislocation is suspected in a newborn.

[Pattern changes in the framework of cesarean sections in 1980/83 as compared to 1974/76 in the University Women's Clinic in Jena].

In a comparative study on Caesarean section problems-based on investigations from 1974/76 and 1980/83-conducted at the Gynaecological and Obstetrical Hospital of Jena University, changes are apparent in the indication for Caesarean section and in maternal morbidity. The evident decrease in statistically uncorrected perinatal mortality as well as in uncorrected intra-and postnatal mortality during this period (difference statistically significant at p less than 0.05) was not due to a further increase in Caesarean sections from 8.23% (period 1974/76) to 9.27% (period 1980/83), but to other facto

Factors affecting the lipid and apolipoprotein levels of cord sera.

In studying the human lipid transport system as it changes from the fetal to the adult form, data were collected on cord sera levels of lipids and apolipoproteins (A-I, A-II, ApoB, C-I, C-II, C-III, ApoD, and ApoE) from infants whose lipid metabolism might have been modified by a variety of factors present before, at, or after delivery. The results clearly indicated that many factors affect the levels of both lipids and apolipoproteins at birth. Method of delivery had little effect except in relation to either fetal immaturity (increased total cholesterol) or fetal stress (increased triglyceri
