[Hepatorenal syndrome. What's new in 2007?].

BACKGROUND: Hepatorenal syndrome (HRS) is a serious complication of end-stage liver disease that was associated with a fatal prognosis in the past. A clear definition of HRS and increased understanding of the pathogenesis have led to considerable progress in therapy outcome. PATHOGENESIS: The major pathogenetic factor is vasodilation of the mesenteric circulation with arterial underfilling and consecutive renal vasoconstriction. THERAPY AND CONCLUSION: Restoration of an effective arterial blood volume can be achieved by the combination of vasopressor therapy (terlipressin, norepinephrine), i

Transcatheter Adrenal Arterial Embolization for Life-Threatening Pheochromocytoma Crisis.

BACKGROUND: Pheochromocytoma crisis is a life-threatening endocrine emergency frequently complicated by Takotsubo syndrome (TTS) and cardiogenic shock. Hemodynamic stabilization is difficult to achieve with medical therapy alone, and emergency surgery carries high perioperative mortality. CASE SUMMARY: A 50-year-old woman presented with acute chest pain, de Winter sign, basal-type TTS, and refractory cardiogenic shock with extreme blood pressure lability. Imaging revealed a 6.5 × 4.7 cm adrenal mass suspicious for pheochromocytoma. Despite optimal medical treatment, hemodynamic stability coul

Gastrointestinal Endoscopic Interventions During Antithrombotic Therapy: A Comprehensive Review of Bleeding Risk and Periprocedural Management.

The increasing use of antiplatelet and anticoagulant therapy has made periprocedural management a central issue in patients undergoing diagnostic and therapeutic endoscopy. Upper endoscopy and colonoscopy are commonly performed in patients receiving aspirin, P2Y12 receptor inhibitors, warfarin, or direct oral anticoagulants, and the clinical decision to continue, interrupt, or bridge therapy requires balancing procedure-related bleeding against thromboembolic risk. Diagnostic esophagogastroduodenoscopy and colonoscopy with mucosal biopsy are generally low-risk procedures and can usually be per…

Exploring the associations between psychodynamic constructs and psychopathology: A network approach.

Psychodynamic therapy effectively reduces symptomatology by focusing on underlying (unconscious) processes instead of symptoms. Nevertheless, the exact interrelationship between psychodynamic constructs and psychopathology remains unclear. This study uses network analysis to explore these associations. We computed a cross-sectional partial correlation network between psychodynamic constructs (i.e., personality functioning, interpersonal relations, and active and passive modes of intrapsychic conflicts according to the Operationalized Psychodynamic Diagnostics [OPD] system) and psychopathology

Perioperative management of antithrombotic drugs in skin surgery - A survey of dermatologists in Germany.

BACKGROUND: We identified substantial heterogeneity in the perioperative management of antithrombotic drugs in skin surgery in Germany in 2012 and 2017 in two cross-sectional surveys. The first national guideline on this subject was published in 2014 and updated in 2021. We sought to identify whether the management of these drugs had changed. METHODS: We sent a paper-based survey to 1115 dermatologists throughout Germany asking them about their perioperative management of antithrombotic drugs in skin surgery, as well as their familiarity with the guideline. RESULTS: We received responses fro

Combined internal and external negative pressure wound therapy: breakthrough treatment for lymphocutaneous intractable fistula.

PURPOSE: Lymphocutaneous fistula after lymph node dissection is intractable, yet there is no established treatment strategy. This study demonstrates the wound closure time achieved by a new method of combined internal and external negative pressure wound therapy (CIEN) in patients with lymphocutaneous fistula. METHODS: The subjects of this study were six consecutive patients with lymphocutaneous fistula after lymphatic surgery, who were treated with CIEN between 2018 and 2020. The CIEN technique can be summarized as follows: first, internal foam is inserted into the fistula from the opening o

[Research advancement of the distal radius fracture].

Distal radius fracture is one of the most common injuries of human beings, particularly in young males and elderly females. There are various classifications among which classification by author' names and the AO fracture classification system are most frequently used. Although the latter one is overall and consummate, a precise classification system with complete description of the fracture remains to be raised. Recently, with the development of wrist biomechanics and microscope anatomy, more and more studies were made to treat distal radius fracture. Good reduction and fixation are the key p

Clinical and radiographic evaluation of vital pulp therapy in open apex teeth with MTA and ZOE.

The purpose of this study was to report the success rate of using mineral trioxide aggregate (MTA) and zinc oxide eugenol (ZOE) as vital pulp therapy agents in immature permanent teeth with carious or traumatic pulp exposure. Subjects were children with permanent teeth requiring pulpotomy (apexogenesis) and without systemic diseases. Both ZOE and MTA treatments as pulpotomy agents showed clinical and radiographic success in immature permanent teeth. Although MTA was more successful, it is more expensive, and dentin bridges may develop over canal orifices, complicating future root canal therapy

A Case of Hybrid Stent Placement Following Bronchial Artery Embolization for Bleeding Airway Stenosis due to Mediastinal and Hilar Lymph Node Metastasis by Renal Cell Carcinoma.

Here, we report a rare case of severe central airway obstruction caused by mediastinal and hilar lymph node metastases with persistent bleeding from renal cell carcinoma. Hemoptysis and critical left main bronchial stenosis were initially managed with bronchial arterial embolization, which reduced the risk of bleeding during subsequent interventions. This enabled safe placement of an AERO airway stent, leading to a rapid improvement in respiratory status. The patient was able to proceed with systemic therapy without delay. This case demonstrates the value of combining bronchial arterial emboli

ECMO-TO-GO: Application of a portable on the body veno-arterial ECMO device in a bridge-to-transplantation setting.

Extracorporeal membrane oxygenation (ECMO) is a well-established therapy to bridge heart- and lung-failure patients to recovery, transplantation, or permanent assist devices. However, it is often associated with thorough patient immobility resulting in an increased hospital length-of-stay and inferior outcomes. To address the challenge of immobility on temporary mechanical support system (tMCS), the "ECMO TO GO" concept was developed to offer full patient mobilization. We present the successful application of a wearable ECMO device. With this setup, the patient was successfully and recurrently

Vascular Function in Continuous Flow LVADs: Implications for Clinical Practice.

Left ventricular assist devices (LVADs) have been increasingly used in patients with advanced heart failure, either as a destination therapy or as a bridge to heart transplant. Continuous flow (CF) LVADs have revolutionized advanced heart failure treatment. However, significant vascular pathology and complications have been linked to their use. While the newer CF-LVAD generations have led to a reduction in some vascular complications such as stroke, no major improvement was noticed in the rate of other vascular complications such as gastrointestinal bleeding. This review attempts to provide a

Complications associated with mechanical circulatory support.

There has been a significant increase in the utilisation of mechanical circulatory support (MCS) devices for the management of cardiogenic shock over recent years, with new devices being developed and introduced with the aim of improving outcomes for this group of patients. MCS devices may be used as a bridge to recovery or transplantation or intended as a destination therapy. Although these devices are not without their complications, good outcomes are achieved, but not without risk of significant complications. In this article, the complications of MCS devices have been reviewed, including t

Management of portal hypertension and esophageal varices in alcoholic cirrhosis.

Portal hypertension is a frequent consequence of liver disease, especially alcoholic cirrhosis. Unabated elevations in portal pressure may presage an esophageal variceal hemorrhage. Propranolol or isosorbide therapy is effective in the prophylaxis of variceal bleeding. In patients with acute variceal hemorrhage, endoscopic sclerotherapy or ligation is the best initial intervention. Stapled esophageal transection is often effective when endoscopic management fails. Surgical shunts have a lower associated mortality rate when they are performed electively and involve partial shunting of hepatic b

Marginal corneal ulceration (limbal guttering) as a presenting sign of temporal arteritis.

A 68-year-old white woman was seen because of pain in each eye associated with the development of a marginal corneal ulceration (limbal guttering) bilaterally. Subsequently, the patient developed swelling over the bridge of the nose, painful necrotic lesions of the fingernail beds, scalp, on one knee, and conjunctival ulceration with scleral thinning. Medical examination revealed a markedly elevated erythrocyte sedimentation rate. A temporal artery biopsy specimen was positive for giant cell arteritis. The skin and ocular lesions resolved when the patient was given systemic corticosteroid ther

Cladribine-based salvage therapy in relapsed/refractory AML: A multicenter retrospective study.

Treatment options after venetoclax-hypomethylating agent (Ven-HMA) failure in acute myeloid leukemia (AML) remain limited, with a median overall survival of 2-3 months. Cladribine targets biologically distinct leukemia stem cells, including monocytic populations implicated in venetoclax resistance. We conducted a multicenter retrospective study across six Israeli academic centers to evaluate cladribine-based salvage therapy, predominantly cladribine, low-dose cytarabine, and venetoclax (CAV), in 31 patients with relapsed/refractory AML. Median age was 70 years (range 21-81); 87% had prior vene

Extracorporeal Membrane Oxygenation in Pediatric Patients.

The primary goal of extracorporeal membrane oxygenation (ECMO) is to provide time for the effective treatment of the underlying disease; it does not directly treat the disease itself but serves as a bridge to recovery or to definitive therapies such as heart or lung transplantation. This review article aims to examine the fundamental components and various cannulation strategies of ECMO in pediatric patients, as well as its indications, anticoagulation strategies, and potential complications.

Biventricular Heart Failure With Reduced Ejection Fraction in a Young Male: A Case Report.

A 33-year-old male with a past medical history of nonischemic cardiomyopathy with a reduced ejection fraction (HFrEF) and hypertension presented with worsening dyspnea and chest pain. This had started over the past three days but had not improved, prompting him to come to the emergency department. His vitals were unremarkable on presentation. Initial evaluation due to his history of HFrEF was obtaining an echocardiogram, which revealed an ejection fraction of 15%-20%, similar to the previous echocardiogram. He underwent cardiac MRI later, which showed an ejection fraction of 6% for the left ve
