Conduction system pacing, a European survey: insights from clinical practice.

AIMS: The field of conduction system pacing (CSP) is evolving, and our aim was to obtain a contemporary picture of European CSP practice. METHODS AND RESULTS: A survey was devised by a European CSP Expert Group and sent electronically to cardiologists utilizing CSP. A total of 284 physicians were invited to contribute of which 171 physicians (60.2%; 85% electrophysiologists) responded. Most (77%) had experience with both His-bundle pacing (HBP) and left bundle branch area pacing (LBBAP). Pacing indications ranked highest for CSP were atrioventricular block (irrespective of left ventricular ej

Mechanisms of ventricular tachycardias with a 1:1 His-V relation in patients with heart disease.

BACKGROUND: Ventricular tachycardia (VT) with a 1:1 V-His relation can be seen in bundle branch reentry or with passive retrograde activation from scar-related VT or reentry using left ventricular Purkinje fascicles. OBJECTIVE: This study aimed to review the frequency with which 1:1 V-His relation occurs and to identify differentiating characteristics of these arrhythmias, including new measures obtained during right ventricular (RV) pacing based on orthodromic His-proximal right bundle potential (HisRB) capture and the stimulus to HisRB (S-HisRB) interval approximating RV electrogram to His

Variable morphology wide complex rhythm: what is the etiology?

We report a case of an infant who developed transient complete heart block following aortic and mitral valve surgery. In the course of his recovery, he developed a variable morphology wide complex rhythm with intermittent narrow complex beats. Review of his initial postoperative electrocardiogram (ECG), which demonstrated a right bundle branch block, helped to clarify the rhythm.

Bifascicular block produced by hyperkalemia.

Bifascicular block is an unusual manifestation of hyperkalemia. A patient who developed right bundle branch block and left anterior hemiblock as the result of elevated serum potassium concentration is reported. After correction of his hyperkalemia, atrial fibrillation with a rapid ventricular response occurred. There was normal ventricular conduction despite this rate-related stress to his conduction system.

Atypical Fast-Slow Atrioventricular Nodal Reentrant Tachycardia Incorporating a "Superior" Slow Pathway: A Distinct Supraventricular Tachyarrhythmia.

BACKGROUND: The existence of an atypical fast-slow (F/S) atrioventricular nodal reentrant tachycardia (AVNRT) including a superior (sup) pathway with slow conductive properties and an atrial exit near the His bundle has not been confirmed. METHODS AND RESULTS: We studied 6 women and 2 men (age, 74 ± 7 years) with sup-F/S-AVNRT who underwent successful radiofrequency ablation near the His bundle. Programmed ventricular stimulation induced retrograde conduction over a superior SP with an earliest atrial activation near the His bundle, a mean shortest spike-atrial interval of 378 ± 119 milliseco

Alternative pacing strategies for optimal cardiac resynchronization therapy.

Cardiac resynchronization therapy (CRT) via biventricular pacing (BVP) improves morbidity, mortality, and quality of life, especially in subsets of patients with impaired cardiac function and wide QRS. However, the rate of unsuccessful or complicated left ventricular (LV) lead placement through coronary sinus is 5-7%, and the rate of "CRT non-response" is approximately 30%. These reasons have pushed physicians and engineers to collaborate to overcome the challenges of LV lead implantation. Thus, various alternatives to BVP have been proposed to improve CRT effectiveness. His bundle pacing (HBP

[Induction of a double nodal pathway in dogs by transcatheter ablation with high frequency currents].

A dog weighing 15 kg and anesthetized with intravenous sodium thiopental was subjected to transcatheter ablation of the AV junction with high-frequency currents, in order to induce a partial alteration in AV conduction (first degree AV block). Two conventional bipolar electrode-catheters were introduced through the right femoral vein and used one for atrial pacing and the other for His bundle recording and current delivering. The catheter used for ablation was situated in the AV junction where the distal monopolar recording of the His bundle electrogram showed an A/V ratio close to one in the

Conduction System Pacing Versus Right Ventricular Pacing in Atrioventricular Block: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

INTRODUCTION: Right ventricular pacing (RVP) is the standard therapy for atrioventricular (AV) block but may cause ventricular dyssynchrony with long-term use. This meta-analysis evaluates the efficacy and safety of conduction system pacing (CSP), including His bundle pacing (HBP) and left bundle branch area pacing (LBBAP), as a physiologic alternative to RVP. METHODS: A systematic search was conducted across PubMed, Google Scholar, Cochrane CENTRAL, and ClinicalTrials.gov from inception to August 2025, for randomized controlled trials (RCTs). Outcomes were analyzed as risk ratios (RRs) or me

Challenges and Strategies in Cryoablation of Para-Hisian Ventricular Arrhythmia: Insights from Two Intractable Cases.

The use of the Freezor Xtra catheter (Medtronic, Minneapolis, USA) is preferred for atrioventricular nodal reentrant tachycardia owing to its low atrioventricular block risk. Therefore, use of this catheter is occasionally applied to para-Hisian ventricular arrhythmia (VA). However, the tips for catheter use in intractable cases remain challenging. This report consists of two cases of drug-resistant VAs originating from para-His. Case 1 involved a 76-year-old man who was treated VA using the reverse bending catheter technique. Case 2 involved 59-year-old man with VA who was treated at the earl

Single operator experience, learning curve, outcomes, and insights gained with conduction system pacing.

BACKGROUND: Conduction system pacing (CSP) is increasingly utilized to prevent and correct dyssynchrony. Barriers to CSP adoption include limited training, methodologic variability, laboratory slot allocation, and few data on learning curves. We report learning curves/clinical outcomes from a single experienced electrophysiologist who was new to CSP, and share gained insights. METHODS: Retrospective analysis of all patients who underwent attempted CSP implantation (2016-2023). Patient characteristics, ECGs, echocardiograms, fluoroscopy/procedure times, lead data were recorded at implant and f

Conduction System Pacing: Hope, Challenges, and the Journey Forward.

PURPOSE OF THE REVIEW: Cardiac pacing has evolved in recent years currently culminating in the specific stimulation of the cardiac conduction system (conduction system pacing, CSP). This review aims to provide a comprehensive overview of the available literature on CSP, focusing on a critical classification of studies comparing CSP with standard treatment in the two fields of pacing for bradycardia and cardiac resynchronization therapy in patients with heart failure. The article will also elaborate specific benefits and limitations associated with CSP modalities of His bundle pacing (HBP) and

Pacing of Specialized Conduction System.

Right ventricular pacing for bradycardia remains the mainstay of pacing therapy. Chronic right ventricular pacing may lead to pacing-induced cardiomyopathy. We focus on the anatomy of the conduction system and the clinical feasibility of pacing the His bundle and/or left bundle conduction system. We review the hemodynamics of conduction system pacing, the techniques to capture the conduction system and the electrocardiogram and pacing definitions of conduction system capture. Clinical studies of conduction system pacing in the setting of atrioventricular block and after AV junction ablation ar

Feasibility and safety of left bundle branch area pacing-cardiac resynchronization therapy in elderly patients.

BACKGROUND: Left bundle branch area pacing (LBBAP) is an emerging technique to achieve cardiac resynchronization therapy (CRT), but its feasibility and safety in elderly patients with heart failure with reduced ejection fraction and left bundle branch block is hardly investigated. METHODS: We enrolled consecutive patients with an indication for CRT comparing pacing parameters and complication rates of LBBAP-CRT in elderly patients (≥ 75 years) versus younger patients (< 75 years) over a 6-month follow-up. RESULTS: LBBAP was successful in 55/60 enrolled patients (92%), among which 25(45%) wer

Stimulation and propagation of activation in conduction tissue: Implications for left bundle branch area pacing.

BACKGROUND: Characterizing wavefront generation and impulse conduction in left bundle (LB) has implications for left bundle branch area pacing (LBBAP). OBJECTIVES: The purpose of this study was to describe the pacing characteristics of LB and to study the role of pacing pulse width (PW) in overcoming left bundle branch block. METHODS: Twenty fresh ovine heart slabs containing well-developed and easily identifiable tissues of the conduction system were used for the study. LB stimulation, activation, and propagation were studied under baseline conditions, simulated conduction slowing, conducti

A case of a "pseudo jump-up phenomenon" caused by retrograde ventricular-His prolongation.

A 35-year-old woman was referred for treatment of a supraventricular tachycardia. The tachycardia was diagnosed as a slow-fast form of atrioventricular nodal reentrant tachycardia. Radiofrequency ablation targeting the antegrade slow pathway was performed at the posterior septum of the right atrium. After the radiofrequency ablation, ventricular extrastimulus pacing was performed to assess the retrograde slow pathway, which was suggested before the ablation. The prolongation of the VA interval with a 10 ms decrease in the S2 pacing interval was 60 ms and retrograde dual pathways were suspected

Wide QRS-complex tachycardia with variable VA-conduction: what is the mechanism?

We describe the case of a 16-year-old woman with a surgically corrected tetralogy of Fallot presenting with recurrent wide-QRS-complex tachycardia. The tachycardia could be induced and terminated with ventricular stimulation only. QRS morphology during sinus rhythm and tachycardia was identical and variable VA-conduction was observed. Mapping of the tachycardia showed that variations of HH intervals preceded VV intervals. Therefore, a mechanism involving re-entry within the bundle branches was suggested. However, detailed mapping showed cranial to caudal depolarization of the His bundle, leadi

Radiofrequency catheter ablation of atrioventricular accessory pathways in 3 dogs with subsequent resolution of tachycardia-induced cardiomyopathy.

Incessant supraventricular tachyarrhythmias are known to result in myocardial dysfunction indistinguishable from idiopathic dilated cardiomyopathy by current testing methods. This tachycardia-induced cardiomyopathy (TICM), however, is uniquely reversible with adequate rhythm control. Two dogs were presented to The Ohio State University for incessant supraventricular tachycardia (SVT) and echocardiographic signs of dilated cardiomyopathy, later proven to be TICM. A 3rd dog presented for frequent paroxysms of SVT and syncope had echocardiographic signs of mild myocardial systolic dysfunction. Al

Radiofrequency catheter ablation of concealed atrio-His bypass tract involved in paroxysmal supraventricular tachycardia.

In a patient with paroxysmal supraventricular tachycardia and without any evidence for preexcitation syndrome or dual atrioventricular (AV) nodal pathways, the tachycardia reentry circuit consisted of the AV node as an antegrade limb of the circuit and a concealed atrio-His bypass tract located in the posterior septum as a retrograde limb. During the tachycardia, the atrial potentials in the septal region and coronary sinus were inscribed in the QRS complex, and the earliest atrial activation site was located in the posterior septum.
