Factors Associated With Lumbar and Femoral Bone Mineral Density in Kidney Transplants Candidates.

INTRODUCTION: Data on risk factors associated with low bone mineral density are limited in patients with end-stage renal disease. This study … and osteoporosis, respectively. Age, serum levels of creatinine and parathyroid hormone, and use of calcitriol and calcium carbonate were associated with femoral densitometry scores. Serum total protein level, Rh-negative status, and B blood type were associated with the lumbar scores. CONCLUSIONS: Parathyroid hormone contributed to bone loss in our kidney …

Outcome of protracted hypoparathyroidism after total thyroidectomy.

BACKGROUND: Although the variables that influence the development of post-thyroidectomy hypocalcaemia are now better understood, the risk factors and long-term outcome of persistent hypoparathyroidism (HPP) are poorly defined. A retrospective review of a prospective protocol for the management of post-thyroidectomy hypocalcaemia was performed. METHODS: Patients with a serum calcium level below 8 mg/dl (2 mmol/l) 24 h after total thyroidectomy were prescribed oral calcium with or without calcitriol and followed for at least 1 year. Protracted HPP was defined as an intact parathyroid hormone (i…

Effect of kidney transplantation on bone.

A broad range of different factors aggravates renal osteodystrophy, which is present in virtually all patients with chronic kidney disease and after successful kidney transplantation. Altered hormonal status, including sex hormones and parathyroid hormone (PTH), a deficit of 1,25(OH)(2) vitamin D(3) (calcitriol), immunosuppressive therapy and post-operative immobilization contribute to a progressive loss of bone density and structure. The decrease of bone mass is particularly prominent during the first 6 months after kidney transplantation and is associated with an increased number of fracture…

Genetic influences on bone density: physiological correlates of vitamin D receptor gene alleles in premenopausal women.

Common vitamin D receptor (VDR) gene alleles have recently been shown to contribute to the genetic variability in bone mass and bone turnover, however, the physiological mechanisms involved are unknown. To examine this, the response to 7 days of 2 micrograms oral 1,25-dihydroxyvitamin D[1,25-(OH)2D] (calcitriol) stimulation was assessed in 21 premenopausal women, homozygous for one or other of the common VDR alleles (bb, n = 11; BB, n = 10). Indices of bone turnover and calcium homeostasis were measured during 2 weeks. Baseline osteocalcin, 1,25-(OH)2D, type I collagen carboxyterminal telopept…

Tacalcitol ointment for the treatment of nail psoriasis.

Tacalcitol is a vitamin D analog with proven efficacy for the topical treatment of psoriasis, but to date it has not been tested in psoriasis of the nail. However, successful treatment has been reported with other vitamin D derivatives such as topical calcipotriol, and topical calcitriol in one study. The present study aimed to assess the efficacy and tolerability of topical tacalcitol in the treatment of nail psoriasis in a sample of 15 patients over a 6-month period. Treatment with tacalcitol ointment may be useful to treat nail psoriasis as it achieves a significant improvement in all nail …

[Clinical aspect of recent progress in phosphate metabolism. Treatment of hypophophatemia].

Most hypophosphatemic patients will not require immediate replacement therapy with phosphate. In only the patients with acute severe hypophosphatemia (<1 mg/dL) or symptoms, intravenous phosphate replacement therapy is indicated. On the other hand, the patients with chronic hypophosphatemia require the medical treatment with oral administration of calcitriol and neutral phosphate since this condition causes hypophosphatemic rickets/osteomalacia. However, the treatment regimen has not been standardized, and nephrocalcinosis and tertially hyperparathyroidism are the complication of the long-term…

Hypercalcemia associated with granulomatous disease in a cat.

A 6-year-old cat was examined because of recurrence of a draining mass involving skin and subcutaneous tissues of the caudoventral aspect of the abdomen. Previous treatment included administration of antimicrobial drugs and corticosteroids and surgical excision. Atypical mycobacteria were seen during cytologic examination of biopsy specimens of the mass; Nocardia sp was cultured. While hospitalized, the cat developed hypercalcemia and was found to have high serum calcitriol concentrations. Treatment consisted of administration of ciprofloxacin and trimethoprim-sulfadiazine because of the infec…

Use of Teriparatide in Hyperphosphatemic Familial Tumor Calcinosis: Evaluating the Interaction Between FGF23 and PTH on the Phosphaturic Effect.

Hyperphosphatemic familial tumor calcinosis (HFTC) is a rare disease characterized by hyperphosphatemia and calcium and phosphorus crystal d … th consistent increase in phosphaturia and reduction in phosphatemia, in addition to an increase in calcitriol. The resulting hypercalciuria precludes the therapeutic use of TPTD in HFTC and suggests an important role of FGF23, not only in phosphate homeostasis but also in avoiding any excess of calcitriol.

Health outcomes of vitamin D. Part I. characteristics and classic role.

Vitamin D is a compound responsible for maintaining mineral homeostasis. It protects against calcium and phosphate deficiency through the effects on the intestine, kidney, parathyroid gland and bone. All mechanisms that help maintain mineral homeostasis of the body are regulated by the vitamin D hormonal form - calcitriol. Synthesis of vitamin D starts in the skin as a non-enzymatic process, which begins during exposure to sunlight, when the absorption of ultraviolet B (UVB) radiation results in convertion of 7-dehydrocholesterol, a metabolite of cholesterol that is stored in the skin, to prec…

Recent findings in phosphate homeostasis.

PURPOSE OF REVIEW: We summarize the most recent findings on the proteins that interact with sodium/inorganic phosphate (Na/Pi) cotransporters, the factors that regulate Pi homeostasis and their role in pathology. RECENT FINDINGS: Studies in animal models and cell lines identified proteins mandatory to correct trafficking of the kidney-specific Na/Pi cotransporter NPT2a and its control by the parathyroid hormone. Expression of the intestinal cotransporter NPT2b is controlled by calcitriol, the ubiquitin ligase Nedd-4 and the serum glucocorticoid inducible kinase. Recent data confirm that fibro…
