Oral pulsed-dose calcitriol administration has been shown to be effective therapy for patients with secondary hyperparathyroidism. However, this effect is not consistently observed in the clinical setting. This study was undertaken to examine whether enlarged parathyroid glands can serve as a clinical marker that predicts the suppressive effect of calcitriol. Thirty-five patients undergoing chronic hemodialysis (HD) were examined (age, 51.9 +/- 14.9 years; duration of HD, 72.0 +/- 56.0 months). Based on the volume of parathyroid glands measured using an ultrasonographic scanner, patients were

Propinox is an antispasmodic drug frequently used in the treatment of disorders of the gastrointestinal tract, the uterus and the gallbladder, but little is known about its relaxing activity in gallbladder tissue. The main objective of this study was to determine the antispasmodic activity of propinox, compared to other antispasmodics, in the gallbladder and to assess its binding affinity to receptor sites which may be involved in its mechanism of action. Antispasmodic activity of propinox, (-) scopolamine-n-butyl bromide, atropine and verapamil was determined in human gallbladders to reduce t

In smooth muscle cells freshly isolated from the bovine ciliary body, effects of carbachol (CCh) on the membrane potential and current were examined by the whole-cell clamp method. The resting membrane potential of the muscle cells used was -60 +/- 1 mV (n = 111). Extracellular application of CCh (2 microM) depolarized the cells to -15 +/- 5 mV (n = 50) with an apparent increase in membrane conductance. Under voltage-clamp conditions, CCh (2 microM) evoked an inward current which exhibited inward-going rectification and reversed the polarity at about 0 mV. Removal of Na+ from the external solu

Cardiovascular complications are the main causes of morbidity and mortality of patients on regular dialysis treatment. Myocardial ischaemia in those patients may be promoted apart from accelerated atherosclerosis also by many extracoronary and extracardial factors. In a group of 60 haemodialyzed patients followed up on a long-term basis silent myocardial ischaemia is encountered during 24-hour Holter monitoring in 30-40% of the examined patients. Its occurrence at the end of haemodialysis and during the early posthemodialytic period and its long persistence are typical. The authors examined 20

Primary pulmonary hypertension is an uncommon but serious disease that often results in debilitating symptoms and early death. One approach to treatment has been to attempt reduction of pulmonary arterial pressure and right ventricular afterload by using vasodilator agents to decrease pulmonary arteriolar resistance. Use of a variety of vasodilators has yielded only limited and infrequent success. Although an occasional patient may respond to vasodilator therapy with improved pulmonary hemodynamics and symptomatic status, most patients do not. Reasons for lack of success include the presence o

GABA receptors are classically known for driving neuronal hyperpolarization and modulating synaptic transmission. In glial cells, however, GABA induces depolarization and triggers calcium-dependent signaling pathways. Müller glia, the principal retinal glial population, maintain retinal homeostasis and are the major source of neuroretinal VEGF-A, a key angiogenic factor in development and disease. Although GABA receptor (GABAR) activity has been proposed to influence retinal VEGF-A, it remains unclear whether this regulation occurs through Müller glial cells (MGC) and which mechanisms are invo

Metabolic syndrome (MetS) symbolises a cluster of interrelated risk factors including central obesity, dyslipidemia, insulin resistance and hypertension, those significantly increase the risk of cardiovascular diseases (CVD). Individuals with a MetS have a two- to fourfold greater risk of coronary artery disease, stroke, myocardial infarction, and heart failure. The pathophysiology of cardiovascular complications in MetS is multifactorial, driven by visceral obesity, insulin resistance, atherogenic dyslipidemia, endothelial dysfunction, oxidative stress, low-grade chronic inflammation, and pro

Hypertension and diabetes commonly coexist, sharing mechanisms such as insulin resistance, obesity, and endothelial dysfunction, which together amplify cardiovascular, renal, and cerebrovascular risk. This dual burden is particularly pronounced in the Asia-Pacific region, where rapid demographic and lifestyle transitions have led to a surge in non-communicable diseases. The Asian-Pacific Society of Hypertension (APSH) and the Diabetes Asia Study Group (DASG) have jointly developed this consensus to provide region-specific, evidence-based recommendations for managing hypertension in individuals

BACKGROUND: The aorta plays a crucial role in the blood circulation through its Windkessel function, buffering blood volume and regulating blood pressure. While traditionally viewed as primarily passive, this function may be actively regulated by vascular smooth muscle cells in the aortic wall through vasoconstriction or -dilation. Despite the widespread use of antihypertensive drugs that target vascular tone, our understanding of human aortic vasoreactivity remains limited. This review therefore aimed to evaluate the vasoreactivity of the human aorta in response to various pharmacological and

Resistant hypertension is highly prevalent among individuals with chronic kidney disease (CKD) and is closely associated with accelerated decline of renal function and increased cardiovascular morbidity and mortality. Recent clinical guidelines have established clearer blood pressure (BP) targets for this population, generally recommending office BP values below 140/90 mmHg, with a more stringent target of <130/80 mmHg for patients with albuminuria, diabetes, high cardiovascular risk, or transplant. Conversely, systolic pressure <120 mmHg is not advised due to concerns regarding renal hypoperf

Restless legs syndrome (RLS) is a sensory-motor neurologic disorder present to a clinically significant degree in 2% to 3% of the adult population, more commonly with advancing age and in women, that dramatically affects sleep and quality of life. Addressing factors that worsen RLS (eg, iron deficiency, antidepressant or antihistamine administration, OSA) is an important first step in treatment. RLS can generally be well treated with medications such as the alpha2-delta calcium channel ligands (A2Ds) gabapentin, pregabalin, and gabapentin enacarbil or, if these are poorly tolerated or lack eff

BACKGROUND: Systemic hypertension is increasingly recognised in premature infants. There is limited evidence regarding treatment, and most published treatment recommendations are based solely on expert opinions.
METHODS: We identified all infants born ⩽32 weeks of gestation and ⩽1500 g birth weight discharged from one of 348 neonatal ICUs managed by the Pediatrix Medical Group between 1997 and 2013. We defined antihypertensive drugs as vasodilators, angiotensin-converting enzyme inhibitors, β receptor blockers, calcium channel blockers, and central α2 receptor agonists. We compared characteri

Cyclic nucleotide elevation in intestinal epithelial cells is the key pathology causing intestinal fluid loss in secretory diarrheas such as cholera. Current secretory diarrhea treatment is primarily supportive, and oral rehydration solution is the mainstay of cholera treatment. There is an unmet need for safe, simple and effective diarrhea treatments. By promoting cAMP hydrolysis, extracellular calcium-sensing receptor (CaSR) is a regulator of intestinal fluid transport. We studied the antidiarrheal mechanisms of FDA-approved CaSR activator cinacalcet and tested its efficacy in clinically rel

S-nitrosothiols (SNOs) are metabolites of NO with potent vasodilatory activity. Our previous studies in sheep indicated that intra-arterially infused SNOs dilate the mesenteric vasculature more than the femoral vasculature. We hypothesized that the mesenteric artery is more responsive to SNO-mediated vasodilation, and investigated various steps along the NO/cGMP pathway to determine the mechanism for this difference. In anesthetized adult sheep, we monitored the conductance of mesenteric and femoral arteries during infusion of S-nitroso-l-cysteine (L-cysNO), and found mesenteric vascular condu

The cloning and characterization of a P2X receptor (schP2X) from the parasitic blood fluke Schistosoma mansoni provides the first example of a non-vertebrate ATP-gated ion channel. A number of functionally important amino acid residues conserved throughout vertebrate P2X receptors, including 10 extracellular cysteines, aromatic and positively charged residues involved in ATP recognition, and a consensus protein kinase C site in the amino-terminal tail, are also present in schP2X. Overall, the amino acid sequence identity of schP2X with human P2X(1-7) receptors ranges from 25.8 to 36.6%. ATP ev

BACKGROUND: Loperamide, a μ-opioid receptor agonist that slows gastrointestinal transit, is widely used as an antidiarrheal agent and an experimental model of gastric dysmotility and functional dyspepsia (FD). However, transcriptomic alterations associated with loperamide-induced delayed gastric emptying remain poorly characterized. This study aimed to investigate gastric gene-expression profiles associated with loperamide-induced gastric dysmotility using RNA sequencing.
METHODS: Male BALB/c mice were intraperitoneally injected with loperamide after 20 h of fasting. Delayed gastric emptying

Opioid receptors are expressed in the vestibular endorgans (afferent neurons and hair cells) and are activated by the efferent system, which modulates the discharge of action potentials in vestibular afferent neurons (VANs). In mammals, VANs mainly express the μ opioid-receptor, but the function of this receptors activation and the cellular mechanisms by which they exert their actions in these neurons are poorly studied. To determine the actions of μ opioid receptor (MOR) and cell signaling mechanisms in VANs, we made perforated patch-clamp recordings of VANs that were obtained from postnatal

INTRODUCTION: Extracellular ATP (eATP) is released by articular chondrocytes under physiological and pathological conditions. High eATP levels cause pathologic calcification, damage cartilage, and mediate pain. We recently showed that stable over-expression of the progressive ankylosis gene product, ANK, increased chondrocyte eATP levels, but the mechanisms of this effect remained unexplored. The purpose of this work was to further investigate mechanisms of eATP efflux in primary articular chondrocytes and to better define the role of ANK in this process.
METHODS: We measured eATP levels usin
