Early signs of alcoholic cardiopathy were studied spiroergometrically and electrocardiographically in 90 males, aged 25 to 40 years, with second-stage chronic alcoholism. A fairly high physical stress tolerance (600 kgm) was found in 75.6% of the patients. Some functional parameters (metabolic units, the "oxygen pulse" of exercise, oxygen debt liquidation time) were, however, disturbed in patients with a less than 5 years' history of the disease. In those cases where alcoholism had lasted for over 5 years, the aerobic reserve was more markedly decreased during exercise, while adaptation to exe

We studied the chronic effects of chlorpromazine (CPZ) on the myocardium of rats using light and electron microscopy. Wistar strain rats were divided into two groups and given either normal saline or CPZ intraperitoneally at a dose of 5 mg/kg body weight/day for 30 consecutive days. Myocardial degeneration, atrophic muscle fiber, and myocardial fibrosis were observed by light microscopy in all CPZ-treated rats. Ultrastructural alterations of the myocardium were also found in all CPZ-treated rats. They consisted of contracted myofibers, mitochondriosis, degenerated mitochondria, dilated sarcopl

The natural history of hypertrophic cardiomyopathy is characterized by a slow progression of symptoms and by sudden death. Ventricular tachycardia is detected during electrocardiographic monitoring in 20 to 30% of patients; this arrhythmia is a sensitive but non-specific marker of those patients who are at particular risk of sudden death. Other specific prognostic features (e.g. anatomic and haemodynamic) to which preventive therapy could be directed remain to be identified. Episodes of supraventricular tachycardia or paroxysmal atrial fibrillation occur in 40-50% of patients and may cause tro

A 9-year-old boy presented with increasing fatigue, anorexia, weight loss, fever, and absolute eosinophilia (48,000/microL). Pulmonary infiltrates occurred 3 months later. A murmur of mitral regurgitation was heard 5 months after onset of illness, and heart failure soon followed. Despite corticosteroid therapy the eosinophilia persisted intermittently until 1 month before death. The patient died within 9 months of the onset of illness. At necropsy there was cardiomegaly with subendocardial fibrosis in the right and left ventricles. Thrombi were present in the left ventricular apex and behind t

The clinical angiographic, and available autopsy data of 32 patients with cardiomyopathy with cardiomegaly were reviewed. Twenty-six patients had alcoholic cardiomyopathy. Of these, 20 patients had patient coronary arteries, and 6 had significant occlusive coronary artery disease. Six patients denied any history of excessive alcohol intake, had significant occlusive coronary artery disease, and were grouped as ischemic cardiomyopathic. A total of 12 patients (37.5%) had significant occlusive coronary artery disease. The clinical separation of patients with cardiomegaly due to alcoholic cardiom

In an English Cocker Spaniel kennel in which the adult population varied between 20 and 30, 11 dogs had evidence of cardiomyopathy over a 5-year period. Three of the affected dogs were found dead, with cardiac enlargement and pulmonary edema found at necropsy. In the other affected dogs, dyspnea and other signs of congestive heart failure developed; 5 of these died. Necropsies were performed on 4 dogs, all of which had cardiac enlargement and pulmonary edema. In 3 dogs, the dyspnea abated with treatment. Four of the 11 affected dogs were less than 5 years old. Chronic valvular disease was disc

A unique case of severe and diffuse endocardial fibroelastosis covering the entire left ventricle and associated with pseudoxanthoma elasticum is presented. The clinical picture was that of an apparently primary restrictive cardiomyopathy. Specific histologic lesions could be recognized in the endocardium.

Thirty-eight non-operated patients with hypertrophic obstructive cardiomyopathy (HOCM) were followed for 1--18 years (mean 8). Twelve patients died, nine of them instantaneously without any other apparent cause of death. The symptoms on the first admission did not discriminate between those who died and those who survived during the observation period, nor did the findings at heart catheterization at rest or left ventricular angiocardiography. However, cardiac enlargement on the first chest X-ray was significantly more common in the decreased group, 75% against 27%, as was the complete absence

Seven cases of hypertrophic cardiomyopathy, presenting in a cardiological practice over a period of three years, are documented to illustrate the clinical spectrum of this disease. Frequently this disorder presents with symptoms resembling those of ischaemic heart disease accompanied by left ventricular hypertrophy without obvious cause. It is often familial. The clinical course is variable, but in general slowly progressive; the prognosis depends largely on the severity of disease when the patient presents for treatment. Although no fully satisfactory treatment is available, beta-blockade oft

Alcoholic cardiomyopathy is a consequence of toxic effects of ethyl alcohol. Acute effects must be distinguished from chronic effects over many years. Chronic abuse of alcohol of 1.5-2 g ethyl alcohol per kg body weight (i.e. about 100-150 g/70 kg) per day for years can cause congestive cardiomyopathy in predisposed persons, usually between 30 and 50 years of age. The diagnosis is associated with some criteria for exclusion, i.e. coronary heart disease, hypertension, valvular heart disease, in addition all obstructive and restrictive cardiomyopathy must be excluded. On the other hand, a specif

Three cases of peripartum cardiomyopathy (CMP PP) are presented. In our opinion this term is more correct than post-partum cardiomyopathy. The study was performed both clinically and hemodynamically; patients were followed up for a long period (in one case up to 14 years). Although there were consistent differences in the rise and development of the disease, the clinical and laboratory results could be put together clearly. In the light of this clinical experience, a wide revision of the literature on the topic was carried out, using the hypotrophic-hypokinetic forms of cardiomyopathy (congest

It is reported on the course of an idiopathic congestive cardiomyopathy in a younger man which could be observed during three years. After a longer period free of symptoms acutely appeared the signs of a myocardial failure, in which case disturbances of cardiac rhythm at length caused death. In detail the clinical fingings, pathologo-anatomical changes as well as the differential-diagnostic considerations are discussed, which are important for the demarcation of the other forms of the idiopathic and secondary cardiomyopathies.
