The goals of reconstruction in orbital blow-out fractures are to restore floor continuity, provide support of orbital contents, and prevent fibrosis of soft tissues. Although ease of use has popularized alloplasts, autogenous material provides greater biocompatibility and results in low rates of infection, extrusion, and migration. Nasoseptal cartilage is an easily accessible, abundant, autogenous source that provides support to the orbital floor and minimal donor site morbidity. Cartilage-hair hypoplasia is an autosomal recessive metaphyseal chondrodysplasia with short-limbed short stature, hypoplastic hair, and defective immunity and erythrogenesis. We have analysed the clinical outcome of 108 Finnish patients. Birth length was below -2.0 SD in 70% of the patients; the adult heights ranged from -11.4 SD to -5.2 SD. The cartilage thickness in both normal hips and in coxarthrosis secondary to hip dysplasia was examined using a computer model. The hip cartilage thickness was calculated in three dimensions from computed tomographic images of the hip joint. In normal hip joints, the anterior cartilage was thicker than the posterior cartilage. In hip joints affected by secondary coxarthrosis, the cartilage thinned as the disease progressed. Approximately 80% of patients demonstrated thinning of the anterior apical cartilage, resulting in a forward and upward displacement of the femoral head.

We discuss several less well recognized disorders of articular cartilage which may affect pediatric patients, including relapsing polychondritis, idiopathic chondrolysis of the hip and chondrolysis after slipped capital femoral epiphysis. The activity of UDP-D-xylose:proteoglycan core protein beta-D-xylosyltransferase (EC 2.4.2.26), the enzyme that catalyzes the initiation of the polysaccharide chain linkage to the core protein of proteoglycans, was measured in costal cartilage from 20 fetal sheep of 65-138 days gestation. Mice homozygous for the recessive gene achondroplasia (cn) aged 16 and 17 days and some homozygotes aged 22-34 days have disruptions in the growth of the proximal tibial growth plate which are due solely to reduced hypertrophic cell height. A second class of homozygote, distinguishable at 22 days, has a greater disruption due to much reduced hypertrophic cell height, reduced labelling index and reduction of the number of cells in the effective proliferative zone.

The timing and molecular profile of cartilage destruction in Escherichia coli and Staphylococcus aureus infectious arthritis and killed Mycobacterium butyricum adjuvant arthritis are presented. Infectious arthritis was studied for 3 weeks; cartilage samples were analyzed at 2, 10, and 21 days. At 48 h postinfection, glycosaminoglycan content was reduced by 20% (p less than 0.05) in E. coli infected knees and by 42% (p less than 0.05) in tibial plateau cartilage of S. aureus infected knees. In rats immunized with articular cartilage antigens, delayed hypersensitivity was detected by the macrophage migration inhibition test and delayed footpad reaction. Additionally, a proliferative response was found in the regional lymph nodes of immunized rats. The mixed addition of sensitized lymphocytes and antibodies exerted a cytotoxic effect on cultured chondrocytes. Articular damage including accelerated chondrolysis may have some relation to delayed hypersensitivity to autologous articular cartilage antigens.
