Tasuki clipping combined with high flow bypass for ruptured, previously coiled, partially thrombosed giant aneurysm at the carotid bifurcation.

A 33-year-old woman presented with a ruptured, partially thrombosed carotid bifurcation aneurysm after partial coiling, which was successfully treated by "tasuki" (a cloth sash crossing from one shoulder to the opposite hip, worn by relay marathon runners) clipping combined with radial artery and external carotid artery-to-middle cerebral artery bypass. "Tasuki" clipping can overcome the dilemma between achieving early complete thrombosis in the blind sac and maintaining anterograde flow of the parent artery to prevent inadvertent occlusion of the perforators and anterior choroidal artery.

[Postoperative epidural hematoma remote from the site of craniotomy for STA-MCA anastomosis].

We experienced a case in which the cause of acute brain swelling following resection of dura matter could not be recognized until the postoperative CT scan. A 30-year-old woman presented with a 4-year history of Moyamoya disease. Right hemiplegia developed a month before operation, so that the anti-platelet therapy was continued. At the end of dural resection the brain started to bulge and the brain swelling increased progressively. The patient was immediately placed on a head up position and received a rapid administration of mannitol for the treatment. The operator performed the echo examina

Multiple spontaneous dissecting aneurysms of the anterior cerebral and vertebral arteries.

A 50-year-old woman presented with rare multiple dissecting aneurysms that appeared first in the anterior cerebral artery (ACA) and shortly afterwards in the vertebral artery (VA). She initially suffered sudden motor weakness in the left lower limb due to acute brain infarction. Angiography revealed diffuse string sign in the right ACA. Conservative treatment resulted in resolution of the deficits. Follow-up angiography performed 1 year later revealed recovery of the ACA stenosis. Fourteen days later, she complained of sudden headache and became comatose. Computed tomography showed diffuse sub

[A case of severe stenosis of bilateral distal vertebral artery successfully treated with anastomosis operation].

A 65-year-old male complained of loss of consciousness for several minutes, transient diplopia and dizziness. He had no neurological deficits nor abnormalities in MR imaging. However, flow velocity of bilateral vertebral artery on ultrasonography indicated severe stenosis of bilateral distal vertebral artery. Brain angiography revealed severe stenosis of bilateral distal vertebral artery as well as occlusion of right middle cerebral artery (MCA). Single photon emission CT (SPECT: ECD-RVR method with acetazolamide loading) showed decreased cerebral blood flow and poor perfusion reserve in bilat

[A case of moyamoya disease in the aged associated with cerebral embolism].

We reported a rare case of Moyamoya disease in the aged associated with cerebral embolism. A 76-year-old female was brought into our hospital by ambulance 30 min after an attack of fainting. She had no past history of cerebrovascular disease. On admission her consciousness level was JCS 3-IA and she showed Rt. hemiparesis ECG exhibited Af tachycardia. No abnormal region was recognized on brain CT. Emergency cerebral angiograms (AG) disclosed that unexpectedly she had Moyamoya disease. Stenotic or occluded lesions were seen from bilateral IC terminal portions to MCA and ACA, and basal moyamoya

Surgical management of intraluminal carotid thrombi.

The most suitable treatment for intraluminal carotid thrombi remains still a much debated question. Some authors have reported a lower morbidity in patients treated with anticoagulant or antiplatelet therapy; on the other side, urgent or delayed surgery is burdened with a high risk of perioperative stroke. Over 11 years (october 1981-november 1992) 602 surgical revascularizations on epi-aortic vessels have been performed at Vascular Surgery Unit of Udine Regional Hospital. Only 2 cases of intraluminal carotid thrombi were observed: both fulfilled the angiographic requirements for endovasal fil

[Technic of the peroperative use of Doppler in neurosurgery].

Intraoperative Doppler exploration during neurosurgery was used during treatment of intracranial aneurysms, arteriovenous malformations (AVM) and cerebral revascularization by extra-intra cranial anastomosis (EICA). It was also employed to study behavior of cortical arteries under different conditions. During treatment of aneurysms, Doppler can show, prior to clipping, whether blood flow through. The flow is regular--spasm of supply artery provokes acceleration of blood rate. After clipping it is possible to evaluate whether artery is still permeable without stenosis or torsion. In AVM, Dopple…

The short retropharyngeal route for arterial bypass across the neck.

Occasionally, in order to revascularize the carotid bifurcation one may need to cross the neck with a bypass that originates in the opposite subclavian or common carotid artery. This report describes a short, natural route behind the pharynx that permits using a shorter bypass as well as a direct reimplantation of one common carotid into its opposite without an intervening graft.

Cerebral ischemic complications following intracranial bleeding in patients with moyamoya disease--three case reports.

Three patients with bleeding-type moyamoya disease suffered ischemic complications following their first episode of intracranial bleeding. The patients presented with intraventricular hemorrhage and suffered cerebral infarction on the 14th, 16th, and 11th days after the ictus, respectively. The clinical records revealed no obvious hypotension before progression to infarction and suggested that dehydration accelerated by the administration of hyperosmotic drugs was a contributing factor to the infarction. Two patients underwent superficial temporal artery-middle cerebral artery anastomoses in t

Rapid contralateral progression of focal cerebral arteriopathy distinguished from RNF213-related moyamoya disease and fibromuscular dysplasia.

BACKGROUND: Focal cerebral arteriopathy includes unifocal or multifocal lesions that are unilateral or bilateral. Large- and/or medium-sized vessels are involved and can be visualized on angiography. CASE REPORT: We report a case of cerebral infarction in a 9-year-old Japanese female who presented with a transient ischemic attack. Steno-occlusion involving the distal part of the internal carotid artery, proximal middle cerebral artery, and anterior cerebral artery was observed. Digital subtraction angiography demonstrated a beaded appearance in the cervical portion of the diseased internal ca

A single case report of Moyamoya disease presenting in a psychiatric setting.

Moyamoya disease (MMD) is a rare cerebral vasculopathy with limited information on the associated cognitive and emotional sequelae, particularly presenting in a psychiatric setting. We present the case of a 25-year-old female with a history of MMD and depressive disorder. She underwent revascularization surgery following an ischemic stroke at age 15 years. Magnetic resonance imaging at the time of the current assessment revealed markedly abnormal appearance in the anterior cerebral circulation with extensive periventricular collateral vasculature involvement. Neuropsychological assessment data

Idiopathic middle cerebral artery occlusion in children: report on three cases.

Three cases of middle cerebral artery occlusion in children are reported. The origin of M1 was the site of occlusion in two cases, while occlusion 1 cm after the origin of M1 was found in the third. The possible etiological diagnosis was infectious arteritis in the first two cases, cranial trauma in the third. In no case were serious contralateral motor deficits detected in follow-up examinations. Follow-up digital subtraction angiography showed complete regression of arterial lesions in the second case. The authors conclude that middle cerebral artery occlusion may have a benign clinical cour

A case of internal carotid artery occlusion successfully treated by encephalo-duro-arterio-synangiosis.

In many cases, intracranial occlusive strokes in children are treated symptomatically, with a diagnosis of acute infantile hemiplegia. This is because angiography is more difficult in children and, even if occlusive cerebral arterial disease is diagnosed, there is no definite therapeutic procedure. A case is reported of left internal carotid artery occlusion presenting with acute infantile hemiplegia and followed by repeated transient ischemic attacks. Encephalo-duro-arterio-synangiosis--developed as an operation for moyamoya disease--was performed on this patient, resulting in a marked revasc
