Pathological vertebral fracture after stereotactic body radiation therapy for lung metastases. Case report and literature review.

BACKGROUND: Stereotactic body radiation therapy (SBRT) is a radiation technique used in patients with oligometastatic lung disease. Lung and chest wall toxicities have been described in the patients but pathological vertebral fracture is an adverse effect no reported in patients treated with SBRT for lung metastases. CASE PRESENTATION: A 68-year-old woman with the diagnosis of a recurrence of a single lung metastatic nodule of urothelial carcinoma after third line of chemotherapy. The patient received a hypo-fractionated course of SBRT.A 3D-conformal multifield technique was used with six cop

Gender disparities in vehicle-related traumatic fractures among elderly individuals: A cross-sectional observational study.

This study examines gender-based variation in clinical characteristics of vehicle-related traumatic fractures (V-TFs) in elderly populations. A retrospective analysis evaluated medical records from 456 elderly patients (≥ 60 years) undergoing V-TFs treatment at a single trauma center between 2013 and 2019. Demographic characteristics, injury mechanisms, fracture patterns, and clinical outcomes were systematically compared between genders. The study population comprised 253 male and 203 female patients, with a mean age of 67.4 ± 6.4 years. Significantly higher rates of smoking (P < .001), alcoh

A case of IgG4-related spinal pachymeningitis with a large spinal cord cavity: case report and updated systematic review.

BACKGROUND: IgG4-related spinal pachymeningitis (IgG4-RSP) is rare, particularly in cases where spinal cord cavities develop after the removal of thickened dura mater, making diagnosis challenging. A 31-year-old male presented with IgG4-RSP, and the clinical presentation, diagnostic imaging, pathology, treatment, and follow-up outcomes were documented. We also conducted a thorough review of the relevant literature. CASE REPORT: The patient presented with two-year progressive lower limb weakness. He experienced lower back pain, numbness in both legs, and urinary and defecatory difficulties wit

Complications of Insufficient Dura and Blood Loss During Surgical Intervention in Shprintzen-Goldberg Syndrome: A Case Report.

BACKGROUND Shprintzen-Goldberg syndrome (SGS) is an extremely rare collagenopathy, most often caused by autosomal-dominant mutations in the SKI proto-oncogene, which is a component of the transforming growth factor beta (TGF-ß) signaling pathway. Approximately 50-60 cases of SGS have been recorded in the literature worldwide since its discovery in 1982. This collagen disorder affects bone and vascular development throughout the body, resulting in craniosynostosis, scoliosis, chest deformities, and aortic root dilation. Patients may have problems in the central nervous system, including Chiari

A modified cutaneous ureterostomy provides satisfactory short and midterm outcomes in select cases.

OBJECTIVE: We present the outcomes of modification of cutaneous ureterostomy by extreme lateralization of the stoma and use of skin flap for formation of ureterostomy. MATERIAL AND METHODS: Between June 2012 and June 2016, 36 patients had modified cutaneous ureterostomy for ureteral obstruction due to pelvic malignancy or genitourinary tuberculosis. Transureteroureterostomy was made with cutaneous stoma at anterior axillary line between iliac crest and lower rib cage, instead of spinoumbilical line. To prevent stenosis a 'V' shaped skin was fed into the stoma. Double J stents were used in all

Number of positive radiographic findings in pediatric trauma patients.

PURPOSE: Conventional radiography is frequently performed in pediatric patients in whom fractures and dislocations are suspected. However, until now, the rate of positive findings of the most commonly performed radiographic examinations in pediatric patients is unknown. The aim of this study was to evaluate the number of positive findings in the 20 most frequently requested standard radiographic examinations in pediatric patients in a level 1 trauma center systematically. METHODS: A transversal cohort study was conducted at a level 1 trauma center in Germany (2008-2014). In a statistical pre

Ischemia of the upper extremity due to noncardiac emboli.

Nine cases of microemboli of arterial origin to the upper extremity are reported. The source of emboli in five of these cases was in the subclavian artery compressed by osseous anomalies in the thoracic outlet. Three aneurysms, one in a subclavian vein graft and two traumatic false aneurysms in the hand, were also noted to be the sources of distal emboli. One unproved case of emboli from an atherosclerotic plaque of the subclavian artery is also reported. Chronicity of symptoms and delay in operation are often noted and lead to difficulties in surgical management. The compressing osseous struc

A Woman in Her 30s With a History of Cervical Cancer Presents With Shortness of Breath and Pleuritic Chest Pain.

A woman in her 30s presented to the ED with a 3-month history of shortness of breath on exertion, dry cough, and pleuritic chest pain. A month ago, the patient was seen at an internal medicine clinic and was found to have a right pleural effusion. A thoracentesis revealed straw-colored fluid, a total nucleated cell count of 1,260 × 106/L, and a differential with neutrophils of 0.15, lymphocytes of 0.55, macrophages/monocytes of 0.19, and eosinophils of 0.10. Fluid cytology and culture were negative. The patient was presumed to have a parapneumonic effusion and treated empirically with antibiot

A novel procedure for mediastinal vagotomy inhibits neurogenic inflammation in rat bronchial tree.

Tachykinin-containing sensory axons originating from the cervical vagal nerves and the first several pairs of thoracic spinal nerves are involved in neurogenic inflammation evoked by capsaicin in the bronchial tree. Unilateral degeneration of the cervical vagal trunk by surgical lesion inhibits neurogenic inflammation in the ipsilateral bronchial airways. The vagal trunk has two main branches, the thoracic vagus nerve and recurrent laryngeal nerve in the thorax. The main purpose of this study was to determine whether the thoracic vagus nerve or recurrent laryngeal nerve was significantly invol

Spinal ascending pathways in amphibians: cells of origin and main targets.

As part of a research program on the evolution of somatosensory systems in vertebrates, the various components of ascending spinal projections were studied with in vivo and in vitro tract-tracing techniques in representative species of amphibians (the large green frog, Rana perezi, the clawed toad, Xenopus laevis and the ribbed newt, Pleurodeles waltl). Three main ascending sensory channels, each with largely separate targets, were demonstrated: 1. Ascending projections via the dorsal funiculus include primary and nonprimary projections that ascend to terminate mainly in the dorsal column nucl

Critical periods of sensitivity to the developmental toxicity of inhaled methanol in the CD-1 mouse.

Exposure of pregnant CD-1 mice to methanol (MeOH) by inhalation on gestation days (gd) 6-15 results in dose-related increases in fetal cleft palate, exencephaly, and skeletal defects. Here, critical periods for the developmental toxicity of MeOH were assessed in pregnant CD-1 mice exposed to 10,000 ppm MeOH or filtered air for 7 hr/day on 2 consecutive days during gd 6-13, or to single day (7 hr) exposures to 10,000 ppm MeOH during gd 5-9. Mice received water but not food during exposure. Maternal blood MeOH was determined at times during, at the end of, and subsequent to a single 7 hr exposur

Concomitant body-wide trauma patterns in patients with head and neck injuries: a comparison based on the trauma register DGU® by the German trauma society and the dortmund maxillofacial trauma registry.

INTRODUCTION: Interdisciplinary trauma patients with multiple injuries require special attention from the first minute of care. This study aimed to determine the overall injury distribution in patients with head and neck injury (HNI) with respect to the head and neck injury location. METHODS: Data on patients with HNI were collected in the Dortmund Maxillofacial Trauma Register 2007-2017 based on a review of patient radiographs and files. All patients with concomitant injuries to central body parts were selected and analysed further by an expert traumatologist collecting data on GCS, ISS, inj

Anatomical relationship of Roos' type 3 band and the T1 nerve root.

BACKGROUND: The most common clinical presentation of thoracic outlet syndrome stems from compression of the lower trunk of the brachial plexus or ulnar nerve. A plethora of anatomical variations have been implicated in the cause of thoracic outlet syndrome. Roos' identification and classification of scalene muscle and fibrous bands have added an additional dimension to understanding its cause and subsequent treatment. Understanding the anatomy and relationship of this band with the lower trunk of the brachial plexus is of paramount importance. METHODS: As part of the authors' long-term study

Malformation complex. Spondylohypoplasia, arthrogryposis, and popliteal pterygium.

Two unrelated children were born with severe congenital malformations: a girl now seven years old and a boy who died in the newborn period. Both had severe cervical hyperextension, severe flexion deformities of the limbs, and bilateral popliteal pterygia. Postmortem examination of the second case showed small, well-formed thoracic vertebrae with mild epidural fibrosis and hemorrhage. These births defects are rare and usually occur separately. We postulate that they are related in these two cases; that the arthrogryposis and popliteal pterygia were probably the result of the spondylohypoplasia

Helmet Usage Reduces Serious Head Injury Without Decreasing Concussion After Bicycle Riders Crash.

BACKGROUND: The importance of bicycle helmets in reducing injuries is unclear. Our center receives a disproportionate number of bicycle crash victims. We sought to evaluate the types of injuries observed and the role of helmets in reducing head injuries. MATERIALS AND METHODS: We evaluated demographic data and compared injuries between bicycle riders that crashed with and without helmets over a 9-year period. Categorical variables were compared using linear regression methods and nominal variables using ANOVA. Differences were considered significant for P ≤ 0.05. RESULTS: There were 906 pati
