Patients' Preferences for Surgical Procedures in Hidradenitis Suppurativa: A Discrete Choice Experiment.

BACKGROUND AND OBJECTIVE: Hidradenitis suppurativa is a burdensome chronic inflammatory skin disorder. While surgery can be essential for many patients, preferences and values regarding surgery are insufficiently characterized. This study aimed to investigate preferences of adult patients with hidradenitis suppurativa in the USA when choosing an hidradenitis suppurativa surgery method. METHODS: A discrete choice experiment was conducted. Patients were asked to choose between two hypothetical procedures 15 times. Literature review and patients with hidradenitis suppurativa and expert interview

Assessment of a zero-shot large language model in measuring documented goals-of-care discussions.

CONTEXT: Goals-of-care (GOC) discussions and their documentation are important process measures in palliative care. However, existing natural language processing (NLP) models for identifying such documentation require costly task-specific training data. Large language models (LLMs) hold promise for measuring such constructs with fewer or no task-specific training data. OBJECTIVE: To evaluate the performance of a publicly available LLM with no task-specific training data (zero-shot prompting) for identifying documented GOC discussions. METHODS: We compared performance of two NLP models in ide

The Woodchuck Hepatitis Virus Model of Hepatocellular Carcinoma for Interventional Radiology Research.

Preclinical models that accurately recapitulate the pathogenesis, microenvironment, and therapeutic response of human hepatocellular carcinoma (HCC) are essential for advancing locoregional therapies. The woodchuck (Marmota monax) chronically infected with woodchuck hepatitis virus (WHV) offers a unique and highly translational large-animal model for HCC research. This manuscript reviews the utility of the WHV-woodchuck model, emphasizing its strengths in mimicking human HCC pathophysiology and its applicability to interventional radiology-based research. Unlike xenograft or chemically induced

Chronic condition clusters and associated disability over time.

OBJECTIVES: Recent evidence shows that more complex clusters of chronic conditions are associated with poorer health outcomes. Less clear is the extent to which these clusters are associated with different types of disability (activities of daily living (ADL) and functional mobility (FM)) over time; the aim of this study was to investigate this relationship. METHODS: This was a longitudinal analysis using the National Health and Aging Trends Study (NHATS) (n = 6179). Using latent class analysis (LCA), we determined the optimal clusters of chronic conditions, then assigned each person to a bes

Integrated Health and Social Care in the United States: A Decade of Policy Progress.

INTRODUCTION: Over the last decade in the United States (US), the burden of chronic disease, health care costs, and fragmented care delivery have increased at alarming rates. To address these challenges, policymakers have prioritized new payment and delivery models to incentivize better integrated health and social services. POLICY PRACTICE: This paper outlines three major national and state policy initiatives to improve integrated health and social care over the last ten years in the US, with a focus on the Medicaid public insurance program for Americans with low incomes. Activities supporte

[Frailty syndrome: an old new friend].

Frailty syndrome is defined as extreme stress vulnerability and decreased potential to adapt. The elderly and chronically ill patients are affected mostly. This condition increases the risk of adverse health outcomes as infections, falls, delirium, institutionalization, progression of comorbidities and mortality. The pathophysiological mechanism is a complex immune and neuroendocrine dysregulation. According to the phenotype model, frailty presents when three of the followings occur: weakness, exhaustion, slowness, weight loss and decreased activity, while cumulative model counts the number of

Increased medical costs of a population-based sample of depressed elderly patients.

BACKGROUND: We examined whether older adults with depressive symptoms below the diagnostic threshold and those with DSM-IV major depression and/or dysthymia have higher medical costs than those without depression. METHODS: We mailed the PRIME-MD 2-item depression screen to the patients of 2 large primary care clinics of a staff-model health maintenance organization in Seattle, Wash. All 11 679 patients 60 years and older with primary care providers at the participating clinics were included, and 8894 (76.2%) were successfully enrolled. An additional 107 patients were referred to the study by

The emotional and behavioural symptom trajectories of children in long-term out-of-home care in an English local authority.

The significant mental health needs of young people in out-of-home care has been well-documented. However, there is little empirical evidence on the timing or development of these difficulties, once these young people have been removed from the maltreatment-environment. Such information may provide useful clinical insight in to how problems develop and persist and whether intervention timings may allow for the prevention of later mental health problems. The current service-data study explored the emotional and behavioural symptom trajectories of 207 young people under the long-term care of a l

Is sleep disturbance in patients with chronic pain affected by physical exercise or ACT-based stress management? - A randomized controlled study.

BACKGROUND: Most people suffering chronic pain are plagued by sleeping difficulties. Cognitive behaviour therapy has produced promising results for insomnia comorbid with chronic pain, but the access to such treatment is often limited. Over the last ten years, interventions aiming to increase cognitive flexibility and physical activity have been assumed to be effective treatments for a variety of conditions, including insomnia and chronic pain. If proven effective, these treatments could constitute the first steps in a stepped care model for chronic pain and insomnia. METHODS: Two hundred nin

Evaluation of a Pharmacist-Led Clinic in a Canadian Remand Facility.

The health of Canadians in correctional facilities is poor when compared with the general population. Pharmacists effectively manage chronic illness and minor ailments; pharmacist-led prescriber clinics are being introduced in the community to improve access to care. However, there are no data on this model in correctional facilities. This article aims to evaluate the role of a pharmacist-led prescriber clinic in a provincial remand facility in Alberta, Canada, via a retrospective chart review of a weekly pharmacist-led clinic in a remand center from January to May of 2023. Data were collected

Using health psychology techniques to manage chronic physical symptoms.

Chest pain and palpitations, non-malignant pain, breathlessness and fatigue often endure despite the receipt of appropriate nursing and medical care. This is distressing for patients, impacts on their quality of life and ability to function and is associated with high healthcare usage and costs. The cognitive behavioural approach offers nurses a model to understand how people's perceptions and beliefs and their emotional, behavioural and physiological reactions are linked. Common 'thinking errors' which can exacerbate symptom severity and impact are highlighted. Understanding of this model may

Developing a persona-driven, mHealth-enabled care model for young men with hypogonadism: a methods-based approach to early risk stratification and prevention.

BACKGROUND: Early-onset hypogonadism is an emerging cardiometabolic risk marker in young men, yet current care pathways remain fragmented and insufficiently tailored to patients' multidimensional needs. Digital health technologies offer an opportunity to support early risk stratification, promote self-management, and facilitate long-term prevention. This study presents a methodological framework combining Blueprint Persona modeling and multidisciplinary co-design to support the development of tailored digital health interventions. OBJECTIVE: To apply a persona-based, co-design methodology to
