Placebo Responses in Trials of Parkinson's Disease.

Parkinson's disease is well documented to be associated with a placebo response. The striatonigral degeneration and resulting dopamine deficiency make it a particularly compelling condition for studying placebo effects, which are most commonly explained by a hyperdopaminergic hypothesis. The placebo response can vary across different motor and nonmotor symptoms. Several factors may influence this response, including symptom severity, the presence of motor fluctuations, expectation of benefit, and the type of intervention. While minimizing placebo effects is essential in clinical trials-where s

Clinical trials for patients with salivary gland cancers: A systematic review of worldwide registers and an evaluation of current challenges.

BACKGROUND: Clinical trials (CT) are crucial for generating scientific evidence and improving clinical outcomes, but they can be challenging in the context of rare cancers. Salivary gland cancers (SGC) are rare and heterogeneous tumors, without standard-of-care approved systemic therapies. We analyzed completed and ongoing CTs to assess the current state of clinical research activity in the field. METHODS: ClinicalTrials.gov, WHO-ICTRP, HealthCanadaCT were searched for antineoplastic pharmacological and interventional CT involving patients with SGC from the trials database creation until Augu

From Fresh Frozen Plasma to First-in-Human: Bringing Coagulation Factor V Deficiency into Therapeutic Trials.

Congenital factor V (FV) deficiency, affecting approximately 1 in 1 million individuals worldwide, remains among the rare bleeding disorders (RBDs) without a licensed factor-specific replacement therapy. While other RBDs have successfully transitioned from plasma-based treatment to approved factor concentrates-exemplified by factor X deficiency's progression to US Food & Drug Administration (FDA)-approved Coagadex and two FDA-approved concentrates for factor XIII deficiency-FV deficiency treatment has remained unchanged for decades, relying solely on plasma and platelet transfusions. Two promi

Barriers to publishing early phase clinical trials: the oncologists' perspective.

INTRODUCTION: Findings from early phase studies are not always placed in the public domain. This study aims to explore why many early phase clinical oncology studies are not published, as well as identify the potential barriers investigators encountered in the publication process. METHODS: Semi-structured interviews were conducted among investigators with experience in early phase clinical oncology studies. Interviews were analyzed using reflexive thematic analysis. RESULTS: Twenty-one investigators were interviewed. The majority worked in Europe (n = 13), while other investigators were base

Current advances and unmet needs in Alzheimer's disease trials for individuals with Down syndrome: Navigating new therapeutic frontiers.

Individuals with Down syndrome (DS) have a genetically determined form of Alzheimer's disease (AD) due to the amyloid precursor protein (APP) gene dose effect. Nearly all individuals with DS develop AD pathology by age 40. Although dementia is rare before this age, its incidence rises sharply thereafter. Longitudinal studies estimate a lifetime dementia risk exceeding 90%, with prevalence reaching 88%-100% after age 65-a marked contrast to 10%-15% in the general population. Recent breakthroughs in sporadic AD, including anti-amyloid therapies such as lecanemab and donanemab, have shown efficac

Oncological and Survival Endpoints in Cancer Cachexia Clinical Trials: Systematic Review 6 of the Cachexia Endpoints Series.

BACKGROUND: In patients receiving anti-cancer treatment, cachexia results in poorer oncological outcomes. However, there is limited understanding and no systematic review of oncological endpoints in cancer cachexia (CC) trials. This review examines oncological endpoints in CC clinical trials. METHODS: An electronic literature search of MEDLINE, Embase and Cochrane databases (1990-2023) was performed. Eligibility criteria comprised participants ≥ 18 years old; controlled design; ≥ 40 participants; and a cachexia intervention for > 14 days. Trials reporting at least one oncological endpoint wer

Con: Cardiac Arrest Patients Should Be Included in Cardiogenic Shock Trials.

While often being both triggered by acute myocardial infarction, cardiogenic shock (CS) and cardiac arrest (CA) constitute two distinct clinical entities with different underlying pathophysiologic backgrounds. CS is a syndrome characterized by systemic hypoperfusion and end-organ dysfunction due to a primary impairment of the cardiac pump function. CA arises instead from an abrupt loss of cardiac mechanical function-commonly triggered by arrhythmias, structural heart disease, or ischemic events-which leads to the immediate loss of effective circulation. Their diverse sequelae and factors that

Artificial Oxygen Carriers: Lessons From Hemoglobin-Based Oxygen Carrier Clinical Trials and Current Development Efforts.

The clinical benefit of blood transfusion is undeniable, and yet reliance on donated blood products has several shortcomings. These include risks of supply shortages due to reliance on volunteer donors, short storage life, and potential infectious disease transmission. Immune response complications to donated blood cannot be completely avoided, despite significant advances in testing. Many of these limitations are exacerbated in low-income countries. For decades, cell-free hemoglobin-based blood substitutes have been developed and tested as potential substitutes for red blood cell transfusions

Revisiting the therapeutic landscape of tauopathies: assessing the current pipeline and clinical trials.

Microtubule associated protein tau (MAPT) is a naturally occurring protein that plays a significant role in stabilizing microtubules, which are essential for the transport of nutrients and other materials within neurons. In tauopathies, tau protein assembles into mis-folded multimers ranging from soluble oligomers to insoluble aggregates, known as neurofibrillary tangles, neuropil threads and are components of neuritic plaques. These abnormal tau assemblies collectively are thought to disrupt the normal function of neurons and lead to their death. Tauopathies are a leading cause of neurodegene

Efficient Testing Using Surrogate Information.

In modern clinical trials, there is immense pressure to use surrogate markers in place of an expensive or long-term primary outcome to make more timely decisions about treatment effectiveness. However, using a surrogate marker to test for a treatment effect can be difficult and controversial. Existing methods tend to either rely on fully parametric methods where strict assumptions are made about the relationship between the surrogate and the outcome, or assume the surrogate marker is valid for the entire study population. In this paper, we develop a fully nonparametric method for efficient tes

Clinical/surgical considerations.

While there are a diverse number of indications for the potential use of cell and gene therapies in people, many of the medical conditions being treated need to consider some general clinical and surgical issues, not only in designing clinical trials, but also in the eventual application of the therapy if shown to be successful. Such issues include the precision of the diagnosis and stage of disease, the presence of significant comorbidity, as well as the challenges that may be encountered in trial designs involving the neurosurgical delivery of these advanced interventions. This chapter discu

A roadmap for clinical trials in MASH-related compensated cirrhosis.

Although metabolic dysfunction-associated steatohepatitis (MASH) is rapidly becoming a leading cause of cirrhosis worldwide, therapeutic options are limited and the number of clinical trials in MASH-related compensated cirrhosis is low as compared to those conducted in earlier disease stages. Moreover, designing clinical trials in MASH cirrhosis presents a series of challenges regarding the understanding and conceptualization of the natural history, regulatory considerations, inclusion criteria, recruitment, end points and trial duration, among others. The first international workshop on the s

Exosomes for Treating Hair Loss: A Review of Clinical Studies.

BACKGROUND: The regenerative properties of exosomes make them especially appealing to treat skin and hair diseases. Preclinical studies suggest that exosomes may fuel hair growth by stimulating dermal papilla cells, activating hair follicle stem cells, and promoting angiogenesis. However, very limited data are available on the safety and efficacy of exosome use in human subjects. OBJECTIVE: To review the published literature on exosome use in human subjects with a focus on safety and the challenges facing clinical implementation in the treatment of androgenetic and nonscarring alopecias. Enhancing site selection strategies in clinical trial recruitment using real-world data modeling.

Slow patient enrollment or failing to enroll the required number of patients is a disruptor of clinical trial timelines. To meet the planned trial recruitment, site selection strategies are used during clinical trial planning to identify research sites that are most likely to recruit a sufficiently high number of subjects within trial timelines. We developed a machine learning approach that outperforms baseline methods to rank research sites based on their expected recruitment in future studies. Indication level historical recruitment and real-world data are used in the machine learning approa

Recommendations to promote equity, diversity and inclusion in decentralized clinical trials.

Decentralized clinical trials involving the use of digital tools to facilitate remote research are gaining momentum. Rapid advancements in digital technologies have supported the adoption of these trials. These innovations facilitate virtual interactions between clinical trial teams and participants by making it easier to collect, transfer and store electronic data. While some studies have demonstrated the potential for these approaches to reduce barriers to clinical trial participation, they are associated with several challenges that may create or worsen existing health inequalities and limi
