Electrophysiological Examination of Ambient Speech Processing in Children With Cochlear Implants.

PURPOSE: This research examined the expression of cortical auditory evoked potentials in a cohort of children who received cochlear implants (CIs) for treatment of congenital deafness (n = 28) and typically hearing controls (n = 28). METHOD: We make use of a novel electroencephalography paradigm that permits the assessment of auditory responses to ambiently presented speech and evaluates the contributions of concurrent visual stimulation on this activity. RESULTS: Our findings show group differences in the expression of auditory sensory and perceptual event-related potential components occur

Phoneme Categorization in Prelingually Deaf Adult Cochlear Implant Users.

PURPOSE: Phoneme categorization (PC) for voice onset time and second formant transition was studied in adult cochlear implant (CI) users with early-onset deafness and hearing controls. METHOD: Identification and discrimination tasks were administered to 30 participants implanted before 4 years of age, 21 participants implanted after 7 years of age, and 21 hearing individuals. RESULTS: Distinctive identification and discrimination functions confirmed PC within all groups. Compared to hearing participants, the CI groups generally displayed longer/higher category boundaries, shallower identific

Preoperative Reading Efficiency as a Predictor of Adult Cochlear Implant Outcomes.

HYPOTHESES: 1) Scores of reading efficiency (the Test of Word Reading Efficiency, second edition) obtained in adults before cochlear implant surgery will be predictive of speech recognition outcomes 6 months after surgery; and 2) Cochlear implantation will lead to improvements in language processing as measured through reading efficiency from preimplantation to postimplantation. BACKGROUND: Adult cochlear implant (CI) users display remarkable variability in speech recognition outcomes. "Top-down" processing-the use of cognitive resources to make sense of degraded speech-contributes to speech …

Lexical Access in Preschool Mandarin-Speaking Children With Cochlear Implants.

PURPOSE: Children with cochlear implants (CIs) have less experience accessing spoken language. Mandarin Chinese uses pitch information to contrast word meaning, and the signal that the CI devices provide is degraded. Thus, Mandarin-speaking children with CIs may face more challenges in the development of language skills. This study examines preschool Mandarin-speaking children's performance in lexical access. We hypothesized that children with CIs and their peers with normal hearing (NH) have comparable naming ability, but they process phonological or semantic information differently. METHOD:

Nonauditory Functions in Low-performing Adult Cochlear Implant Users.

INTRODUCTION: Despite substantial benefits of cochlear implantation (CI) there is a high variability in speech recognition, the reasons for which are not fully understood. Especially the group of low-performing CI users is under-researched. Because of limited perceptual quality, top-down mechanisms play an important role in decoding the speech signal transmitted by the CI. Thereby, differences in cognitive functioning and linguistic skills may explain speech outcome in these CI subjects. MATERIAL AND METHODS: Fifteen post-lingually deaf CI recipients with a maximum speech perception of 30% in…

Maximizing Cochlear Implant Outcomes with Short-Term Aural Rehabilitation.

BACKGROUND: Increasing numbers of adults are receiving cochlear implants (CIs) and many achieve high levels of speech perception and improved quality of life. However, a proportion of implant recipients still struggle due to limited speech recognition and/or greater communication demands in their daily lives. For these individuals a program of aural rehabilitation (AR) has the potential to improve outcomes. PURPOSE: The study investigated the effects of a short-term AR intervention on speech recognition, functional communication, and psychosocial outcomes in post lingually deafened adult CI u

Speech Perception and Production in Cochlear Implant Recipients with Pendred Syndrome.

AIMS: Patients with the Pendred syndrome suffer very often from a hearing loss. They may be good candidates for a cochlear implantation, but unfortunately, due to the fluctuating character of the hearing loss, they may escape such indication. In the study, we compared speech production and speech acquisition in 2 groups of implanted patients: those with the Pendred syndrome, and standard non-syndromic patients. METHODS: Ten patients with Pendred syndrome were analyzed for speech perception and production. The control group consisted of 41 non-syndromic implanted patients. All implantees were …

Effect of auditory feedback on speech intelligibility of adults with cochlear implants.

PURPOSE: Auditory feedback (AF) contributes to speech intelligibility (SI) which is vital in social interactions to examine AF effect on SI of adults with cochlear implant (CI). The relationship between age of CI implantation and AF on SI was examined as well. METHOD: Twenty native Hebrew speaker pre-lingual adults with a hearing loss using CIs from ages 2 to 19 years. Participants were recorded reading excerpts from a book and word lists from MIDBAR test in two situations-with and without using their CIs. Sixteen judges listened to the recordings and rated the speech characteristics of the p

The Listening Network and Cochlear Implant Benefits in Hearing-Impaired Adults.

Older adults with mild or no hearing loss make more errors and expend more effort listening to speech. Cochlear implants (CI) restore hearing to deaf patients but with limited fidelity. We hypothesized that patient-reported hearing and health-related quality of life in CI patients may similarly vary according to age. Speech Spatial Qualities (SSQ) of hearing scale and Health Utilities Index Mark III (HUI) questionnaires were administered to 543 unilaterally implanted adults across Europe, South Africa, and South America. Data were acquired before surgery and at 1, 2, and 3 years post-surgery.

Comparison of vestibular rehabilitation on balance function in cochlear implant recipients.

Cochlear Implantation (CI) is an effective surgical approach to rehabilitate the deaf patients. However, the insertion of an electrode into the cochlea may have an adverse effect on vestibular receptors, resulting in vertigo or dizziness. The present study is aimed to investigate the impacts of vestibular rehabilitation therapy (VRT) exercises on dizziness symptoms of patients who underwent CIlimb.

The Importance of Access to Bilateral Hearing through Cochlear Implants in Children.

Children with hearing loss require early access to sound in both ears to support their development. In this article, we describe barriers to providing bilateral hearing and developmental consequences of delays during early sensitive periods. Barriers include late identification of hearing loss in one or both ears and delayed access to intervention with hearing devices such as cochlear implants. Effects of delayed bilateral input on the auditory pathways and brain are discussed as well as behavioral effects on speech perception and other developmental outcomes including language and academics.

The effect of increased channel interaction on speech perception with cochlear implants.

Cochlear implants (CIs) are neuroprostheses that partially restore hearing for people with severe-to-profound hearing loss. While CIs can provide good speech perception in quiet listening situations for many, they fail to do so in environments with interfering sounds for most listeners. Previous research suggests that this is due to detrimental interaction effects between CI electrode channels, limiting their function to convey frequency-specific information, but evidence is still scarce. In this study, an experimental manipulation called spectral blurring was used to increase channel interact

Effects of a particle placed on the ossicles for microphoneless cochlear implant design.

In a typical cochlear implant design, the ambient sound is detected via a microphone and the transmission unit of the implant is placed at the back of the auricle. However, this design has several drawbacks. Firstly, the subject cannot bath or swim comfortably with the microphone unit on, and secondly having an external attached unit which may be visible is cosmetically disturbing. Herein, the idea is to explore obtaining the acoustic signals that would directly drive the cochlear nerves, without using a microphone, in which only the vibrations of the ossicles are employed. Thus, the natural f

Speech perception with simultaneous bilateral cochlear implants: Is there a unilateral predominance?

OBJECTIVES: To assess the audiological outcomes and speech performance among children with simultaneous bilateral cochlear implants (CIs). METHODS: This was a retrospective case review of 41 patients with prelingual deafness who failed a hearing aid trial and received simultaneous bilateral CIs. Ear-specific responses in terms of pure tone average (PTA), speech reception thresholds (SRTs), and speech discrimination score (SDS) were recorded for both ears of all patients. RESULTS: The PTA and SRT for the right and left CIs were comparable (P-value = 0.861 and P-value = 0.524, respectively). T

Cochlear Implant Surgery: Endomeatal Approach versus Posterior Tympanotomy.

The aim of the present study was to compare the posterior tympanotomy (PT) technique to the endomeatal approach. The endomeatal approach (EMA) for Cochlear Implant (CI) surgery was performed on 98 patients with procident lateral sinus or a small mastoid cavity, on 103 ears (Group A). Conventional mastoidectomy and PT was performed on the other 104 patients, on 107 ears (Group B). Data on all patients were then collected for the following: intra- and post-operative complications, Tinnitus Handicap Inventory (THI), Vertigo Symptom Scale (VSS), duration of surgery, and postoperative discomfort. The Neural Bases of Tinnitus: Lessons from Deafness and Cochlear Implants.

Subjective tinnitus is the conscious perception of sound in the absence of any acoustic source. The literature suggests various tinnitus mechanisms, most of which invoke changes in spontaneous firing rates of central auditory neurons resulting from modification of neural gain. Here, we present an alternative model based on evidence that tinnitus is: (1) rare in people who are congenitally deaf, (2) common in people with acquired deafness, and (3) potentially suppressed by active cochlear implants used for hearing restoration. We propose that tinnitus can only develop after fast auditory fiber
