This prospective study reports the results of the longevity of the correction achieved in 312 patients receiving injectable collagen implant and the histopathologic identification of the implant in the six who experienced reactions at the treatment sites. In reporting results in cosmetic patients, certain ones can be considered objective or factual, and other must be judged subjective. This study reports both kinds of results and proposes a new theory to explain the duration of the improvement.

The collagenase activity in skin fibroblast cultures from three patients with rare forms of epidermolysis bullosa was assayed before and after proteolytic activation of the medium. Otospongiosis-like lesions were induced in rats by immunizing them with native type II collagen. Immunized rats had antibody responses specific for native type II collagen and developed otospongiosis-like lesions. Antibodies to collagen types I, II, and IV were measured in patients with otosclerosis and patients with Meniere's disease. Levels of antibodies to type II collagen were significantly higher in these patients than in control subjects, while no differences were found among levels of antibodies to collagen type I or type IV. These observations suggest a possible role for type II collagen autoimmunity in the etiology of otosclerosis and Meniere's disease.

The effect of diabetes on myocardial collagen metabolism was investigated in rats. Diabetes was induced by an i.v. injection of streptozotocin (60 mg/kg). The animals were killed after 3, 6, 18, or 26 wk of diabetes. Urinary glucose levels were measured by Tes-Tape. The myocardium was quickly removed and homogenized. The enzyme collagen proline hydroxylase has been measured in liver biopsies from fourteen patients with primary biliary cirrhosis. The activity was elevated in ten of the patients, but not to the degree previously found in patients with active cirrhosis. There was no correlation between the enzyme activity and the liver copper concentration, which was elevated in all except one of those measured. This suggests that excessive collagen synthesis in PBC is not directly related to the high liver copper concentrations.

To ascertain if the features of osteogenesis imperfecta could be due to an abnormality in two genetically distinct collagens, Type III and Type I, we measured, in pepsin digests of skin, the ratio of the alpha 1, (III) to alpha 1 (I) chains derived from the two types, using a method of interrupted polyacrylamide-gel electrophoresis. Ultrastructural study of prolapsed atrioventricular valves from five patients, one of whom also had the Marfan syndrome, disclosed granular cells that were characterized by numerous membrane-bound, electron-dense inclusions resembling those in the Hurler syndrome. Collagen fibrils were present within some of these inclusions. The intracellular collagen deposits may form as a consequence of abnormal interactions between newly secreted collagen and acid mucopolysaccharides.
