INTRODUCTION: Cryptococcosis is an opportunistic infection, that rarely occurs in immunocompetent patients.
CASE REPORT: We report a case of a 82 year-old woman who suffered from a bacterial pneumonia associated with lymphopenia. During the hospitalisation, she developed a cutaneous and ganglionnary cryptococcosis. No other cause of immunosuppression was found. The lymphopenia disappeared with the healing of the pneumonia treated with systemic antibiotics. Treatment with fluconazole was effective in 2 months on the cryptococcosis.

The efficacy of two different daily doses of itraconazole (ITRA) and the combination of flucytosine (5-FC) with ITRA in the treatment of an experimental model of cryptococcosis in hamsters was studied. Five groups of 20 animals each were inoculated by the intracardiac route with 10(5) cells of Cryptococcus neoformans. A case of primary pulmonary cryptococcosis with increased serum carcinoembryonic antigen (CEA) (7.0 ng/ml) is presented. The diagnosis was made by transbronchial lung biopsy (TBLB) and no abnormality was found in either gastrointestinal tract or immunity. Immunohistochemical study of the lung specimen showed that CEA was present on the surfaces of alveoli. The level of CEA fell to 0.8 ng/ml after administration of an antimycotic agent (Fluconazole) with simultaneous improvement of the chest radiographic findings. Cryptococcosis is a systemic disease caused by the yeast-like fungus Cryptococcus neoformans. Cutaneous manifestations, although rare, may precede other organ involvements. A case of cutaneous cryptococcosis is described herein. A latex reactive was designed for detecting Cryptococcus neoformans antigen. It was evaluated by a comparative study with a commercial system (Meridian Diagnostic, Inc.). Total coincidence was observed with both latex systems after studying 3 sample groups: patients with cryptococcosis diagnosis, blood bank donors and patients with clinical signs of the disease. We have presented a case of unsuspected cryptococcal laryngitis, clinically masquerading as Kaposi's sarcoma in a patient with AIDS. The spectrum of laryngeal disease in AIDS patients includes a variety of infections and neoplasms, which can be treated satisfactorily when accurate and timely diagnosis is made.

Two cases of pulmonary infection with Cryptococcus neoformans are reported. In both patients the cryptococcal antigen titres remained positive following initiation of effective therapy, and despite clinical evidence of resolution of disease. The possible causes for these findings are discussed and it is suggested why this atypical serological response does not necessarily indicate dissemination or active infection.
