[Treatment of a patient with type-2 diabetes mellitus and a depressive disorder].

HISTORY AND CLINICAL PRESENTATION: A 65-year-old man with type 2 diabetes mellitus and unfavorable metabolic values in combination with obesity and arterial hypertension and considerable psychosocial strains presented at a psychosomatic outpatient clinic. During the course of diagnostic sessions a problematic illness behavior as well as a depressive symptomatology that had developed after the death of his wife a few years earlier became apparent. DIAGNOSIS, THERAPY, COURSE: An avoidant illness behavior and a moderate depressive episode were diagnosed. Within a low threshold psychosomatic inter

[Association between suicidal behaviour and cyclothymic temperament in patients with recurrent depressive disorder].

BACKGROUND: Suicidal behaviour is a major health problem, particularly among patients with depressive disorders. AIMS: To determine the frequency of suicidal behavior among sample of patients with recurrent depressive disorder and to explore the relationship between suicidal behavior and cyclothymic temperament in these patients. METHODS: This was a cross-sectional study bearing on 98 patients (43 men and 55 women, mean age of 46.8 ± 9.9 years) followed for recurrent depressive disorder according to the criteria of DSM-IV recruited during partial or complete recovery interval. Information ab…

Predicting treatment response in major depressive disorder: the impact of early symptomatic improvement.

OBJECTIVE: Antidepressants (ADs) are the mainstay of treatment for major depressive disorder (MDD). Despite their widespread usage, a consensus does not exist as to the timing of clinically significant symptomatic improvement during an AD trial. The objective of this review is to provide practitioners with empirically based recommendations pertaining to the optimal duration of index (initial) AD therapy before a clinical intervention is warranted. METHODS: We conducted a nonsystematic review, using a combination of a MeSH key word search, Google Scholar, and the Scopus database. Our search st…

Impact of early-life stress and resilience on patients with major depressive disorder.

PURPOSE: Early-life stress (ELS) has a long-lasting effect on affective function and may entail an increased risk for major depressive disorder (MDD). However, resilience can play a protective role against developing psychopathology. In this study, we investigated the relationships of depressive symptoms with ELS and resilience in MDD. MATERIALS AND METHODS: Twenty-six patients with MDD as well as age- and gender-matched healthy controls were included in this study. Each subject was assessed concerning ELS, resilience, and depressive symptom severity with self-report questionnaires. Independe…

Prevalence of incompletely penetrant Huntington's disease alleles among individuals with major depressive disorder.

OBJECTIVE: Presymptomatic individuals with the Huntingtin (HTT) CAG expansion mutation that causes Huntington's disease may have higher levels of depressive symptoms than healthy comparison populations. However, the prevalence of HTT CAG repeat expansions among individuals diagnosed with major depressive disorder has not been established. METHOD: This was a case-control genetic association study of HTT CAG allele size in two discovery cohorts of individuals with major depressive disorder and comparison subjects without major depression as well as a replication cohort of individuals with major…

Phenotypic effects of a bipolar liability gene among individuals with major depressive disorder.

Variations in voltage-dependent calcium channel L-type, alpha 1C subunit (CACNA1C) gene have been associated with bipolar disorder in a recent meta-analysis of genome-wide association studies [Ferreira et al., 2008]. The impact of these variations on other psychiatric disorders has not been yet investigated. Caucasian non-Hispanic participants in the STAR*D study of treatment for depression for whom DNA was available (N = 1213) were genotyped at two single-nucleotide polymorphisms (SNPs) (rs10848635 and rs1006737) in the CACNA1C gene. We examined putative phenotypic indicators of bipolarity am

Evolving refractory major depressive disorder diagnostic and treatment paradigms: toward closed-loop therapeutics.

Current antidepressant therapies do not effectively control or cure depressive symptoms. Pharmaceutical therapies altogether fail to address an estimated 4 million Americans who suffer from a recurrent and severe treatment-resistant form of depression known as refractory major depressive disorder. Subjective diagnostic schemes, differing manifestations of the disorder, and antidepressant treatments with limited theoretical bases each contribute to the general lack of therapeutic efficacy and differing levels of treatment resistance in the refractory population. Stimulation-based therapies, suc…

Verbal working memory and functional outcome in patients with unipolar major depressive disorder.

In this naturalistic cross-sectional study, the author tested the hypothesis that verbal working memory (WM) in major depressive disorder (MDD) would predict functional outcome. The subjects consisted of 54 clinic adult out-patients. The author found that, in the patients with current episode of MDD, functional outcome was significantly correlated with depressive scores, but not with Digit Sequencing Task scores. Meanwhile, in a sample of full remitted or partial remitted (mildly depressed) patients, functional outcome was significantly correlated with both Digit Sequencing Task scores and dep…

Heterogeneity of DSM-IV major depressive disorder as a consequence of subthreshold bipolarity.

CONTEXT: There is growing evidence that major depressive disorder (MDD) might be overdiagnosed at the expense of bipolar disorder (BPD). OBJECTIVES: To identify a subgroup of subthreshold BPD among DSM-IV MDD, which is distinct from pure MDD regarding a range of validators of bipolarity, and to examine the pattern of these validators among different groups with affective disorders. DESIGN: Ten-year prospective longitudinal and family study including 3 follow-up waves. Data were assessed with the DSM-IV Munich Composite International Diagnostic Interview. SETTING: Community sample in Munich,…

Depressive symptoms and symptoms of post-traumatic stress disorder in women after childbirth.

This study examined the course of psychological problems in women from late pregnancy to six months postpartum, the rates of psychiatric, especially depressive and post-traumatic stress symptoms and possible related antecedent variables. During late pregnancy, one to three days postpartum, six weeks and six months postpartum, 47 of the 60 participating women completed a battery of questionnaires including the General Health Questionnaire, the State-Trait Anxiety Inventory, the Edinburgh Postnatal Depression Scale, and the PTSD Symptom Scale. In general, most women recovered from psychiatric an

Subsyndromal depressive symptoms in patients with bipolar and unipolar disorder during clinical remission.

BACKGROUND: Subsyndromal depressive symptoms seem to be quite prevalent in mood disorders although very few studies have assessed them in patients considered to be in remission by clinical and psychometric criteria. This study sought to evaluate the presence of subsyndromal depressive symptoms in bipolar and unipolar patients in clinical remission. METHODS: One-hundred seventy-six patients with DSM-IV bipolar (62 bipolar I, 58 bipolar II) or unipolar mayor depression (n=58) in clinical remission and 60 healthy subjects were assessed using several psychometric instruments including the 17 item

Symptoms following abrupt discontinuation of duloxetine treatment in patients with major depressive disorder.

BACKGROUND: Discontinuation symptoms are common following antidepressant treatment. This report characterizes symptoms following duloxetine discontinuation. METHODS: Data were obtained from 9 clinical trials assessing the efficacy and safety of duloxetine in the treatment of major depressive disorder (MDD). RESULTS: In a pooled analysis of 6 short-term treatment trials, in which treatment was stopped abruptly, discontinuation-emergent adverse events (DEAEs) were reported by 44.3% and 22.9% of duloxetine- and placebo-treated patients, respectively (p<0.05). Among duloxetine-treated patients r…

Whole genome linkage scan of recurrent depressive disorder from the depression network study.

Genome-wide linkage analysis was carried out in a sample of 497 sib pairs concordant for recurrent major depressive disorder (MDD). There was suggestive evidence for linkage on chromosome 1p36 where the LOD score for female-female pairs exceeded 3 (but reduced to 2.73 when corrected for multiple testing). The region includes a gene, MTHFR, that in previous studies has been associated with depressive symptoms. Two other regions, on chromosomes 12q23.3-q24.11 and 13q31.1-q31.3, showed evidence for linkage with a nominal P < 0.01. The 12q peak overlaps with a region previously implicated by linka…

The prevalence of major depressive disorder and low self-esteem in medical inpatients.

OBJECTIVE: To determine the prevalence of major depressive disorder in acutely ill medical inpatients, and the relationship of this to low self-esteem. METHOD: A total of 186 patients were interviewed 6 or 7 days following admission to detect the presence of DSM-IV major depressive disorder (MDD). Patients were assessed using a new brief psychiatric interview, the Silverstone Concise Assessment for Depression (SCAD), which has previously been validated for use in the physically ill. The cognitive function of the patients was measured, using the Mini-Mental State Examination (MMSE), with patie…

Antidepressant-associated mood elevations in bipolar II disorder compared with bipolar I disorder and major depressive disorder: a systematic review and meta-analysis.

OBJECTIVE: Antidepressant-associated manic and hypomanic episodes have been reported in bipolar I disorder but are rare in major depressive disorder (MDD). Several lines of evidence suggest that bipolar II disorder is a distinct illness from bipolar I disorder and MDD. The risk of antidepressant-associated mood elevations (AAME) in bipolar II disorder relative to bipolar I disorder and MDD is unknown. DATA SOURCES: We conducted a computer-aided MEDLINE search encompassing the dates 1949 to February 2008, using the keywords antidepressant and mania, antidepressant and hypomania, antidepressant…

Differential performance on tasks of affective processing and decision-making in patients with Panic Disorder and Panic Disorder with comorbid Major Depressive Disorder.

BACKGROUND: Neuropsychological studies have provided evidence for deficits in psychiatric disorders, such as schizophrenia and mood disorders. However, neuropsychological function in Panic Disorder (PD) or PD with a comorbid diagnosis of Major Depressive Disorder (MDD) has not been comprehensively studied. The present study investigated neuropsychological functioning in patients with PD and PD + MDD by focusing on tasks that assess attention, psychomotor speed, executive function, decision-making, and affective processing. METHODS: Twenty-two unmedicated patients with PD, eleven of whom had a…
