This paper concerns the "H" reflex in eleven patients with spastic spinal cord injury (C5-T10), treated with Baclofen (50 mg/day) or Diazepam (20 mg/day). The "H" reflex was tested in the tibial nerve before and 15 days after the interrupted use of the drugs. Five patients used Baclofen and 6 patients used Diazepam. The latency time was measured. All the patients showed normal latency time before the use of the drugs. After the use all but one didn't show any significant alterations of the latency time. The effects of anesthetic premedication with diazepam and phentanyl on the arterial oxyhemoglobin saturation (SpO2) were monitored with a pulsioximeter (BIOX 3700, Ohmeda) in 83 patients scheduled for traumatologic and orthopedic surgery who had baseline SpO2 higher than 95%. Twenty-one children undergoing surgery received a caudal block using 1 ml kg-1 of a mixture of 50% lignocaine (1%) and 50% bupivacaine (0.25%). They were randomized into two groups, one of which received diazepam 10 mg rectally as premedication. Diazepam administration resulted in significantly higher Cmax and AUC values for bupivacaine but did not influence the plasma concentrations of lignocaine.

The effects on the respiratory function of two tranquillising drugs, lorazepam and diazepam, have been compared in 20 patients with chronic obstructive lung disease. Both drugs induce a respiratory depression (decrease in tidal volume and minute ventilation with acceleration of the respiratory frequency) with slight respiratory acidosis, but lorazepam causes no significant hypoxemia and has a shorter duration of action than diazepam. Nevertheless, if tranquillisers are indicated in such patients, they have to be used with care.
