The relationship between impulsivity and impulse control disorders in Parkinson's disease.

A range of behaviors presumed to be related to dopaminergic medications have been recently recognized in Parkinson's disease (PD). We evaluated 50 consecutive cognitively intact PD patients on stable dopamine agonist and levodopa therapy and 100 healthy controls for compulsive sexual behavior, compulsive buying, or intermittent explosive disorders assessed by the Minnesota Impulsive Disorders Interview (MIDI), pathological gambling (South Oaks Gambling Screen, SOGS), impulsivity (Barratt Impulsiveness Scale), compulsivity (Maudsley obsessional-compulsive inventory), and depression scores (Geri

American Indians and non-Indians playing a slot-machine simulation: effects of sensation seeking and payback percentage.

The research literature on gambling behavior indicates that American Indians (AIs) suffer from pathological gambling at a greater rate than the majority population. The literature also suggests that dispositional factors, such as sensation seeking, can influence gambling. However, situational factors, such as the payback percentage of a slot machine, may not. The present study recruited 12 AI and 12 non-AI participants to play a simulated slot machine in three different sessions. Half of the participants in each group were high sensation seekers. The other half were low sensation seekers. Acro

Olanzapine in the treatment of pathological gambling: a negative randomized placebo-controlled trial.

OBJECTIVE: Pathological gambling is associated with bipolar disorder and dopamine dysfunction. Olanzapine is a second-generation antipsychotic with mood-stabilizing properties and antagonistic activity at several dopamine receptors. The purpose of this study was to evaluate olanzapine in the treatment of pathological gambling. METHOD: In this 12-week, single-center, randomized, double-blind, placebo-controlled, flexible-dose (2.5-15 mg/day) trial, 42 outpatients with pathological gambling by DSM-IV-TR criteria received olanzapine (N = 21) or placebo (N = 21). The primary outcome measure was t

The "locals" market: an emerging gaming segment.

One of the newest customer segments to be identified by casino marketers is the "locals" market, or local resident gambler market. This study addresses the relative lack of empirical data on the "locals" market segment. The purposes of the study are to identify important reasons for visiting a particular casino; to identify which casinos respondents visit most often and why; to gain an understanding of such behavioral variables as gambling budget, time spent gambling, and favorite game; and to determine perceptions of different casinos in the Las Vegas area. The results may not generalize acro

Fracture of subthalamic nucleus deep brain stimulation hardware as a result of compulsive manipulation: case report.

OBJECTIVE AND IMPORTANCE: Trichotillomania (TTM) is an impulse control disorder characterized by the recurrent pulling of one's hair resulting in noticeable hair loss. There has been no definite association drawn between Parkinson's disease (PD) and TTM, although there is a suggestion that obsessive-compulsive symptomatology may be more prevalent in left-side predominant PD. We believe that it is important to be aware of psychiatric comorbidities in the surgical treatment of PD, as they may significantly impact the postoperative course. CLINICAL PRESENTATION: We describe the case of a 58-year

Reduction of food cravings through concurrent visuospatial processing.

OBJECTIVE: Two experiments adopted a working memory approach to evaluate the effectiveness of visuospatial tasks as a technique for reducing food cravings. METHOD: Dieting and non-dieting women were asked to form images of both food-related and nonfood items, induced by either pictures (Experiment 1) or verbal cues (Experiment 2). They were required to concurrently perform one of three tasks that load on the visuospatial sketch pad of working memory: saccadic eye movements, dynamic visual noise, or spatial tapping. RESULTS: In support of the working memory model of limited visuospatial capac

Impulsiveness as the intermediate link between the dopamine receptor D2 gene and alcohol dependence.

OBJECTIVES: Reinforcement and reward processes have been proposed as being an intermediate link between the risk for alcohol dependence and the gene coding for the dopamine receptor D2 (DRD2). This hypothesis remains open to speculation, and personality traits such as impulsiveness, a core dimension in addictive disorders, should also be taken into account. For instance, recent evidence in rats showed that DRD2 antagonists might increase impulsivity in decreasing the value of delayed rewards. METHODS: Considering the pro-impulsiveness role of ethanol observed in clinical practice and epidemio

Conditional covariation of binge drinking with predictors of college students' cheating.

Recent research has identified a variety of significant predictors of academic dishonesty, but virtually no studies have examined the conditional relation of binge drinking and students' cheating. Using a survey sample of 289 college students, this study tested the mediating relations of binge drinking with the correlation of independent variables--including opportunity, strain, and self-control--on self-reported academic dishonesty. Low self-control had a stronger correlation with students' cheating behaviors for those who were heavy binge drinkers (beta = -.52) than for students who were non

Topiramate for the treatment of kleptomania: a case series and review of the literature.

Kleptomania--the inability to resist the impulse to steal objects, not for personal use or monetary gain--is currently classified in psychiatric nomenclature as an impulse control disorder. There is no standard pharmacologic therapy for this disorder. If kleptomania was considered a form of obsessive-compulsive disorder, treatments used for this spectrum, including serotonin reuptake inhibitors (SSRI), other antidepressants, opioid receptor antagonist medications, and mood stabilizers, could be logically tested. Topiramate is currently used for the treatment of patients with affective and comp

Conceptual issues in trichotillomania, a prototypical impulse control disorder.

Impulse control disorders (ICDs) are characterized by irresistible urges to perform acts that result in a reduction of tension and possibly gratification, but also have negative consequences. Trichotillomania, an ICD, is characterized by a recurrent failure to resist impulses to pull out one's hair, resulting in noticeable hair loss. Limited information is available about structural and neurochemical differences in individuals with trichotillomania. Cognitive behavioral techniques are promising treatments for trichotillomania. Pharmacologic treatments have focussed on clomipramine and venlafax

Motivational disturbances and cognitive effort-based decision-making in Parkinson's disease.

BACKGROUND: Motivational disturbances, such as apathy and impulse control disorders (ICDs), frequently co-occur in patients with Parkinson's disease (PD). The assessment of these motivational disturbances has proven to be challenging due the absence of validated objective behavioral measures for evaluating motivation in older adults. This scenario may contribute to underdiagnosis. The present study aimed to investigate the clinical utility of a modified version of an existing effort-based decision-making task which required cognitive (e.g., working memory) instead of physical (e.g., finger tap

Impulse control and related behavioral disorders in Parkinson's disease. Risk factors, diagnosis, and management. Is there a possible role for non-invasive brain stimulation?

Parkinson's disease (PD) is a neurodegenerative disease characterized by motor and non-motor symptoms. Among the non-motor symptoms, Impulse Control Disorders (ICDs) and related behaviors (ICBs), severely affect patient's global health. Despite their psychosocial consequences, ICDs are underrecognized in PD because routine screening is still not common. The main risk factor for ICDs is the use of dopamine agonists (DAs). Therefore, the latest guidelines on ICDs management emphasize pharmacological adjustments. However, these approaches are not always feasible due to the recurrence or worsening

Prognosis of impulse control disorders in Parkinson's disease: a prospective controlled study.

BACKGROUND: The long-term prognosis of impulsive compulsive disorders (ICD) remains poorly studied in Parkinson's disease (PD). OBJECTIVE: Evaluating the natural history of ICD and its impact on PD symptoms including cognition and treatment adjustments. MATERIALS AND METHODS: We assessed PD patients at baseline (BL) with (BL-ICD+) or without (BL-ICD-) ICD despite dopamine agonist (DA) exposure of > 300 mg levodopa-equivalent daily dose for > 12 months at baseline and after more than two years of follow-up. ICD were assessed using the Ardouin's Scale of Behaviors in PD (ASBPD), cognition usin

Prolactinomas.

Hyperprolactinemia, defined by a level of serum prolactin above the standard upper limit of normal range, is a common finding in clinical practice and prolactinomas are the main pathological cause. Prolactinomas lead to signs and symptoms of hormone oversecretion, such as galactorrhea and hypogonadism, as well as symptoms of mass effect, including visual impairment, headaches and intracranial hypertension. Diagnosis involves prolactin measurement and sellar imaging, but several pitfalls are involved in this evaluation, which may difficult the proper management. Treatment is medical in the majo

Mapping sign-tracking and goal-tracking onto human behaviors.

As evidenced through classic Pavlovian learning mechanisms, environmental cues can become incentivized and influence behavior. These stimulus-outcome associations are relevant in everyday life but may be particularly important for the development of impulse control disorders including addiction. Rodent studies have elucidated specific learning profiles termed 'sign-tracking' and 'goal-tracking' which map onto individual differences in impulsivity and other behaviors associated with impulse control disorders' etiology, course, and relapse. Whereas goal-trackers are biased toward the outcome, si

Depression as a Risk Factor for Impulse Control Disorders in Parkinson Disease.

OBJECTIVE: To longitudinally evaluate the role of depression in the development of impulse control disorders (ICDs) in Parkinson disease (PD) patients. METHODS: Using data from the Parkinson's Progression Markers Initiative, we included PD patients without ICDs at baseline according to the Questionnaire for Impulsive-Compulsive Disorders in Parkinson's Disease (QUIP). Patients were prospectively evaluated first quarterly and then biannually. Development of an ICD was defined as an increase in QUIP scores during follow-up. Using survival proportional hazard models, we studied the effect of bas

Brain PET substrate of impulse control disorders in Parkinson's disease: A metabolic connectivity study.

Impulse control disorders (ICDs) have received increased attention in Parkinson's disease (PD) because of potentially dramatic consequences. Their physiopathology, however, remains incompletely understood. An overstimulation of the mesocorticolimbic system has been reported, while a larger network has recently been suggested. The aim of this study is to specifically describe the metabolic PET substrate and related connectivity changes in PD patients with ICDs. Eighteen PD patients with ICDs and 18 PD patients without ICDs were evaluated using cerebral 18F-fluorodeoxyglucose positron emission t

Capacity, control and responsibility in Parkinson's disease patients with impulse control disorders: Views of neurological and psychiatric experts.

Dopamine replacement therapy can induce impulse control disorders (ICDs) (e.g., hypersexuality) in susceptible Parkinson's disease patients. ICDs can sometimes result in criminal offending. In a number of past Commonwealth cases, it appears offending ICD patients have been considered to be suffering from 'irresistible impulses' such that their decision-making capacity, behavioural control and responsibility were totally compromised. This contrasts with courts' general scepticism of drug-induced 'compulsion' in cases of addiction-related offending. In one case of explicit ICD-related offending,
