OBJECTIVES: To elucidate the incidence of Meckel's diverticulum; to determine the correlation between the histologic type of the diverticulum's mucosa and its clinical presentation; and to review our experience with Meckel's diverticulum.
DESIGN: Case-control study.

Although it is not uncommon to encounter a Mecklel's diverticulum in a patient with Crohn's disease, the involvement of this diverticulum by Crohn's disease is less common. A well performed radiological small intestinal study strongly suggests the diagnosis, which is definite after histologic examination of the reported.

We report a 18-days-old newborn boy with right ventricular diverticulum associated with biventricular cardiomyopathy, diagnosed by two-dimensional and Doppler echocardiography and angiography. Heart transplant was carried out at the age of four months, dying the next day. The histologic study showed endocardial fibroelastosis.

A rare case of bleeding Meckel's diverticulum in an adult, in which Tc-99m pertechnetate scan, mesenteric angiography, and barium examination failed to establish the diagnosis, is presented. The diagnosis was made by laparoscopy, and a laparoscopically assisted resection was carried out with a smooth postoperative course, early discharge from the hospital, and rapid return to work.

In the submitted case-history the authors draw attention to the advantage of radionuclide diagnosis of ectopic gastric mucosae located in Meckel's diverticula. Diagnosis with 99mTc is according to the authors the treatment of choice in acute haemorrhage into the GIT with an obscure aetiology.

The initial technetium-99m pertechnetate abdominal scintigraphy revealed equivocal or normal results. However, a second scintigraphy without pentagastrin demonstrated a focal area of persistently increasing radioactivity in the right lower quadrant of the abdomen. At surgery, Meckel's diverticulum was confirmed, and histological examination of the excised specimen revealed that it was lined with ectopic gastric mucosa. Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract. We reviewed our experience with 31 cases of Meckel's diverticulum in children from 1966 to 1987. Twenty-four patients presented with clinical manifestations: ten from bleeding, ten from obstruction, and four from diverticulitis. Seven Meckel's were incidental findings at laparotomy. Fifty percent of the patients with bleeding had abdominal pain and 100% had ectopic gastric mucosa in the Meckel's.
