OBJECTIVE: Duckerley type IIIB distal humerus fractures are rare and complex injuries that pose significant challenges in both diagnosis and treatment. Currently, no consensus exists on the fixation method, with existing approaches often struggling to handle small fragments and associated with issues like elbow instability. The purpose of this study is to evaluate the surgical outcomes of submerged Kirschner wires combined with plate or submerged screw fixation technique for the treatment of Duckerley type IIIB distal humerus fractures.
METHODS: A retrospective analysis was conducted on 10 pa

A 20-year-old right-handed fisherman experienced a severe injury in a high-energy traffic accident. These injuries included a shaft fracture of the right humerus, open comminuted fracture of the right forearm and multiple peripheral nerve palsies involving the musculocutaneous, median and radial nerves. Six months after the initial treatment, the patient was referred to our hand surgeons for further treatment. Several operations for functional reconstruction using only the ulnar nerve and ulnar nerve-innervated muscles were performed: the main procedures included a partial ulnar nerve transfer

BACKGROUND: Terrible triad complex elbow fracture-dislocations are represented by elbow dislocations associated with fractures of the coronoid and radial head. Published literature has focused on classifying coronoid fractures by their radiographic morphology on plain x-ray images and computed tomography imaging. No study has specifically related native coronoid osteology to in situ fracture morphology. We identified 3 distinct bony regions of the coronoid, (medial, intermediate, and lateral ridges) with correlation to common fracture patterns associated with terrible triad injuries.

PURPOSE: Brachial plexus birth injuries (BPBI) can be life-changing for both the patient and family. While most resolve spontaneously, surgical intervention is indicated for those who fail to improve. This can be performed via nerve grafting or nerve transfer. The spinal accessory nerve is a commonly used extraplexal donor in brachial plexus surgery. In cases of BPBI, it can be transferred to the musculocutaneous nerve to restore elbow flexion. While there are short-term reports in BPBI, few, if any, long-term outcome data are available.
METHODS: A retrospective review of patients who underwe

OBJECTIVE: Kendo is a Japanese martial art analogous to fencing, which is becoming increasingly popular worldwide. The large number of participants creates a need to assess injury patterns to better train them. The purpose of this study is to describe current injury rates in kendo and compare these rates to other martial arts.
METHODS: This retrospective study used an online questionnaire sent to 500 active members of the All United States Kendo Federation and World Kendo Federation. The questionnaire, based on the NCAA Injury Surveillance System, contains questions regarding location and typ

Handball is a high-intensity sport with significant risks of both acute and chronic injuries, particularly in the upper and lower extremities. This review article focuses on imaging findings of upper limb injuries sustained by handball players, with a special emphasis on shoulder, elbow, wrist, and hand pathologies. These injuries are common due to the repetitive throwing actions and frequent physical contact inherent in the sport. We discuss key epidemiological data, biomechanical demands, especially throwing, and review specific imaging characteristics and management strategies for these inj

BACKGROUND: Injuries to the ulnar collateral ligament (UCL) are increasing among baseball players at all levels. Elbow forces during pitching routinely exceed levels that cause failure in UCL cadaver studies, highlighting the importance of identifying protective measures against UCL injury. The flexor-pronator mass has been demonstrated as a dynamic stabilizer of the elbow against valgus loads. Increasing dynamic stability of the elbow may help mitigate medial elbow gapping, a known risk factor for UCL injury. The purpose of this study was to determine the effect of an 8-week forearm-strengthe

Monteggia lesions and Monteggia-like lesions involve ulna and radius injuries, which can not be successfully treated using the surgical principles of isolated fractures. Proximal ulnar fracture, radio-humeral dislocation, and additional dislocation in the proximal radioulnar joint result in the disintegration of the functional unit, and there is a complex injury across the elbow to the forearm, resulting in poor clinical outcomes. Thus, addressing all osteo-ligamentous injuries is essential for the long-term course.

Radial head arthroplasty has developed into a reliable procedure to address fractures of the radial head not amenable to reconstruction, particularly when associated with an unstable elbow or forearm. Good to excellent results can be anticipated when used for the correct indications and when care is taken to understand and address the concomitant injuries. This article will discuss the technique of radial head arthroplasty in the context of the injuries that often accompany radial head fractures.

BACKGROUND: Complex elbow fracture dislocations, dislocation with fracture of one or several surrounding bony stabilizers, are difficult to manage and associated with poor outcomes. While many studies have explored treatment strategies but a lack of standardization of patient-reported outcome measures (PROMs) makes cross-study comparison difficult. In this systematic review, we aim to describe what injury patterns, measured outcomes, and associated complications are reported in the complex elbow fracture dislocation literature to provide outcome reporting recommendations that will facilitate i

The radial head plays a crucial role in maintaining normal elbow kinematics, and radial head arthroplasty (RHA) can help restore normal elbow kinematics after radial head fracture. Most modern implants are made of polished metal alloy with stems that may be smooth, press-fit, or cemented. There is no consensus as to the superiority of smooth versus press-fit stems, and both can result in good clinical outcomes. Cemented stems are no longer indicated in routine RHA. Correct implant positioning is critical, and numerous methods of assessment have been described.

The coronoid process of the ulna forms the anterior boundary of the trochlear notch and is crucial for elbow stability. Coronoid fractures are uncommon and they occur in aproximately 10% of elbow dislocations. They are mostly associated with ligamentous and capsular disruptions as well as concomitant fractures. Posterior elbow dislocations, combined with fractures of the coronoid and the radial head are known as the terrible-triad of the elbow. These injuries result in instability of the elbowjoint and, if not treated properly, lead to recurrent dislocations that can cause further damage. Depe

Triceps tendon ruptures are rare injuries. Coexistence of ipsilateral ulnar collateral ligament injury is even rarer. Here, we describe an unusual combination injury to elbow of a 39-year-old male construction worker consisting of triceps tendon rupture, avulsion of elbow ulnar collateral ligament and flexor pronator muscle origin ipsilaterally. A simultaneous repair and reconstruction of all damaged structures was proposed with individualized postoperative rehabilitation. Return to pre-injury level of activities obtained with this treatment protocol. High degree of suspicion and careful exami

Patient age, the site and extent of the injury, and the delay of treatment significantly influence the outcome after repair of ulnar nerve injuries. Ulnar nerve repairs in older patients, high-level injury, and delayed cases may result in a poor prognosis. High-level lesions and lesions that are close to the elbow can also benefit from nerve transposition. Splinting in wrist flexion and/or elbow extension with carpal tunnel and Guyon's canal releases are necessary for lesions that are closer to the wrist. Arterial repairs combined with nerve repairs especially in low-level injury can provide s

Since Speed first treated the radial head fracture with a metal prosthesis in 1941, the purpose of the treatment of radial head fracture with a radial head changed from the prevention of heterotopic ossification to the prevention of proximal migration of the radius and instability of the elbow. The optimal indication is gradually determined as a non-reconstructable radial head fracture with associated injuries that would leave the elbow unstable if the radial head were resected. Types of prosthesis changed from mono-block, bipolar to modular prosthesis. Material of prosthesis changed from acry
