Minimally Invasive Aortic Arch Repair: Technical Considerations and Mid-Term Outcomes.

BACKGROUND: Reports of minimal invasive aortic arch surgery are scarce. We reviewed our experience with minimal access aortic arch surgery performed through an upper mini-sternotomy, with emphasis on details of operative technique and early and mid-term outcomes. METHODS: The medical records of 123 adult patients (mean age 66 ± 12 years), who underwent primary elective minimal access aortic arch surgery in two aortic referral centers, were reviewed. The most common indication was degenerative aortic arch aneurysm in 92 (75%) patients. Standard operative and organ protection techniques used in

Aortic events and reoperations after elective arch surgery: incidence, surgical strategies and outcomes.

OBJECTIVES: The true incidence of aortic events (AEs) and reoperations (REDO) following elective total aortic arch replacement remains unknown. The aim of this study was to review the incidence of AEs and surgical REDO, and its respective outcomes after 1232 elective arch repairs at 11 European aortic centres. METHODS: Retrospective chart review (in the absence of prospective data collection) was performed for statistical analysis. Follow-up was conducted during routine clinical examination or in a telephone interview with patients and/or their respective physicians. RESULTS: One hundred fif

Genomic characterization of a poxvirus isolated from a child.

A poxvirus was isolated from a six-year-old girl. The comparative analyses of the genome of this isolate (H-CP-LSax) which were carried out using the restriction endonucleases BamHI, HindIII, KpnI, MluI, NcoI, SacI, and SmaI revealed that this isolate is a member of the genus orthopoxvirus. Since the girl had never been vaccinated against smallpox, and had close contact to domestic animals, including cats, rabbits and guinea pigs, the genome of H-CP-LSax virus was genetically analysed in comparison with other known orthopoxviruses. The analysis demonstrates clearly that the HindIII cleavage pa

Oestrid myiasis at a cross-road.

The oestrids are a genuinely unique group of Diptera that represent many species whose larvae parasitize mammals, feeding on live tissues of a wide range of hosts, from mice to elephants, causing obligatory myiases. Those have had greatest impact on the activities of humans and their use of animals (e.g., as food, recreation, in the conduct of wars). To a large extent their impact has been controlled through the use of therapeutic application of synthetic chemical treatments. While in some limited cases eradication has been achieved, it is unlikely to become the norm as these parasites seem we…

First record of Toxoplasma gondii antibodiesin Royal Bengal tigers (Panthera tigris tigris) and Asiatic lions (Panthera leo persica) in India.

The purpose of this study was to detect the antibodies against Toxoplasma gondii in Royal Bengal tigers (Panthera tigris tigris), Asiatic lions (Panthera leo persica), leopards (Panthera pardus), and elephants (Elephas maximus indicus) residing in the Mahendra Chaudhury Zoological Park, in Chhatbir, Punjab (India) during winter and monsoon seasons. Using  indirect ELISA, 20 serum samples were analysed during the winter season. Results indicated that 1 lion (5%) tested seropositive, and 3 tigers and 1 lion (20%) were considered suspect. During the monsoon, 4 individuals (2 tigers and 2 lions, 2…

Results with an algorithmic approach to hybrid repair of the aortic arch.

OBJECTIVE: Hybrid repair of the transverse aortic arch may allow for aortic arch repair with reduced morbidity in patients who are suboptimal candidates for conventional open surgery. We present our results with an algorithmic approach to hybrid arch repair, based on the extent of aortic disease and patient comorbidities. METHODS: Between August 2005 and January 2012, 87 patients underwent hybrid arch repair by three principal procedures: zone 1 endograft coverage with extra-anatomic left carotid revascularization (zone 1; n = 19), zone 0 endograft coverage with aortic arch debranching (zone

Non-circulatory arrest aortic arch replacement.

The definitive management of combined aortic arch and descending aortic pathologies such as aneurysms and dissections is either a single or staged operation associated with high morbidity and mortality. Stroke, kidney dysfunction, coagulopathy and high blood transfusion requirements are all affiliated with hypothermic circulatory arrest and prolonged cardiopulmonary bypass times. Considering the perilous nature of these operations, the authors describe a step-by-step zone 2 arch replacement as a staged frozen elephant trunk procedure, which provides an adequate landing zone for a later-placed

A single stage hybrid repair of a complicated acute type B dissection with aortic arch involvement.

We present a case in which a single stage hybrid repair was successfully for a complicated acute type B dissection of the aortic arch involvement in a 63-year-old male patient. We performed a combination of different techniques; left subclavian artery debranching, elephant trunk insertion without aortic arch replacement, and thoracic endovascular aortic repair (TEVAR) from antegrade approach. The postoperative course was successful, and the patient was discharged on day 11 after surgery. A-half-year's follow up computed tomography (CT) scan showed shrinkage of thrombus lumen, vascular reverse

Can scavengers save zebras from anthrax? A modeling study.

Namibia's Etosha National Park (ENP) is home to many different animals such as lions, jackals, hyenas, zebras, elephants, etc. Each year, grazing animals are infected and die from anthrax caused by the bacteria Bacillus anthracis. This increases the number of carcasses in the park, which serve as food for scavengers such as jackals. This study investigates the interplay between anthrax transmission in zebras and the scavenging of zebra carcasses in ENP, using a deterministic mathematical model to describe the population dynamics. We strive to answer the following research questions: Under what…

Measuring the effects of wildlife contraception: the argument for comparing apples with oranges.

There are few wildlife populations existing today that can be supported without some form of management. Wildlife fertility control, as one option, has moved from the research stage to actual application with a number of species, including wild horses, urban deer, captive exotic species and even African elephants, but this approach remains controversial in many quarters. Strident debate has arisen over the possible effects of contraception on behaviour, genetics, stress and even management economics, among other parameters. Part of the debate arises from the fact that critics often fail to rec…

A cochlear frequency-position function for several species--29 years later.

Accurate cochlear frequency-position functions based on physiological data would facilitate the interpretation of physiological and psychoacoustic data within and across species. Such functions might aid in developing cochlear models, and cochlear coordinates could provide potentially useful spectral transforms of speech and other acoustic signals. In 1961, an almost-exponential function was developed (Greenwood, 1961b, 1974) by integrating an exponential function fitted to a subset of frequency resolution-integration estimates (critical bandwidths). The resulting frequency-position function w

Extended Aortic Repair for Acute Type A Aortic Dissection with Rupture and Malperfusion Complicated with Ehlers-Danlos Syndrome.

The patient was a 54-year-old gentleman with sudden chest pain. He suffered from cardiac tamponade and malperfusion of the left carotid artery and the right lower extremity due to acute type A aortic dissection. Rupture of the aortic root and a huge entry from the transverse arch to the proximal descending aorta were found. Extended repairs of valve-sparing root replacement and total arch replacement with frozen elephant trunk were successfully performed. He was discharged without any complications. He was finally diagnosed as having vascular Ehlers-Danlos syndrome by a genetic examination.

Secondary Aortoesophageal Fistula Treated by Covered Esophageal Stent and Muscle Wrapping.

Secondary aortoesophageal fistula is rare but is associated with serious complications and high mortality. We devised a treatment strategy involving a covered esophageal stent, instead of esophageal resection, and placing the latissimus dorsi muscle around the infected aorta without removing the aorta and the stent graft. A 72-year-old man with a history of total arch replacement with a frozen elephant trunk for a thoracic dissecting aneurysm developed aortoesophageal fistula and underwent surgical treatment using our strategy. He recovered well, with no evidence of reinfection 6 months after

Successful surgical exclusion of rapidly expanding kommerell diverticulum following a total arch replacement for an acute type a aortic dissection.

A 50-year-old man presented with an acute type A aortic dissection with an aberrant right subclavian artery. Emergent total arch replacement with an elephant trunk was performed. Intraoperatively, the origin of the aberrant right subclavian artery could not be resected because it was located too far from the distal arch. After two weeks, the patient became aware of dysphagia. Postoperative computed tomography showed the esophagus was compressed anteriorly by the aneurismal origin of this aberrant vessel (Kommerell diverticulum) with a patent false lumen. Additional replacement of the descendin
