"Partial duplication of lower lip and hemimandible" A rare case.

Duplication of mandible and lower lip is a very rare congenital entity. We report an extremely uncommon case of Congenital Duplication of Lower lip and Mandible in a 3 year old girl, who was treated surgically in a single stage for correction of both lip and mandible. This was a commissure preserving single staged procedure. The Surgical procedure, the problems related to this anomaly and the embryology are discussed.

Congenital bronchial artery-pulmonary artery fistula in an adult.

We report the case of a 21-year-old man who presented with a hilar mass and whose condition was diagnosed as a congenital anomalous communication between the bronchial and pulmonary artery. We review his lung vascular embryology and anatomy that led to this rare systemic to pulmonary arterial shunt, describe other etiologies that may lead to abnormal systemic artery to pulmonary artery communication, and briefly discuss treatment options.

Tracheal agenesis type B: further evidence to a lethal congenital tracheal malformation.

Tracheal agenesis is an extremely rare, typically fatal congenital tracheal malformation. Lack of prenatal symptoms and emergent presentation usually lead to a failure to arrive at the correct diagnosis and manage the airway properly before the onset of irreversible cerebral anoxia. We report the case history of an infant born with immediate respiratory failure who was diagnosed with tracheal agenesis. The clinical presentation, embryology, classification, and surgical management are discussed.

Traumatic unilateral deep cerebral venous infarction.

A case of unilateral deep cerebral venous infarction in a young man associated with mild head injury is presented. The diagnosis is made on the basis of the clinical, radiological, operative and histopathological findings. We propose that a thrombosis or obstruction of the anterior segment of the basal vein of Rosenthal (BVR) was the primary pathology. This is discussed in respect to current knowledge of deep cerebral venous anatomy and embryology.

Non-recurrent laryngeal nerve: identification during thyroid surgery.

The non-recurrent course of the inferior laryngeal nerve is a rare anomaly. The incidence varies from 0.3 to 4% on the right side. It is extremely rare on the left side. The awareness of such an anomaly is essential as the risk of damage to the nerve is higher during thyroid surgery. Two cases of non-recurrent laryngeal nerve are reported, and the embryology and method of identification are discussed.

[Uterus unicornis with rudimentary horn and ipsilateral kidney agenesis: clinical symptoms and therapy].

We report on a 21-year old patient with unicornuate uterus with a rudimentary non-communicating horn (Buttram II-A-1-b), haematosalpinx, associated with ipsilateral ureteranomaly and renal agenesis. In this case we illustrate symptoms, diagnosis and therapy. Despite extensive diagnosis surgical methods finally have to be adapted to the anatomical situation during operation. A short review of literature and embryology is given, more diagnostic preoperative measures are discussed.

Infected congenital mucocele of the nasolacrimal duct.

The authors report their experience with an infant presenting with an infected nasolacrimal duct mucocele, emphasizing correlation of clinical, CT, and surgical findings. CT is the imaging modality of choice to demonstrate the triad of 1) a cystic medial canthal mass, 2) dilatation of the nasolacrimal duct, and 3) a submucosal nasal cavity mass; findings which are diagnostic of this entity. A brief review of the relevant embryology is also presented.

[New aspects of the ontogenetic development of the human colon].

The parts of the fetal colon--later retroperitoneally located (ascending and descending colon)--were examined in 31 human fetuses in order to determine their ontogenetic formation, position, and dorsal attachment. The present data cast doubt upon so-called "descending of the caecum" described in current textbooks of embryology. Considering the literature available a new concept of the formation of the right colon flexure and the ascending colon is presented.

Developmental disorders of the pediatric spine.

Developmental disorders of the pediatric spine are a group of congenital malformations commonly referred to as spinal dysraphism. These malformations are characterized by incomplete or absent fusion of midline mesenchymal, bony, and neural structures. They can be classified according to the presence or absence of an associated back mass. A detailed analysis of the embryology, clinical findings, and radiologic characteristics of the common forms of spinal dysraphism is outlined.

Anorectal malformations.

This article reviews the current status of anorectal malformations in the neonate. It begins by discussing the history, epidemiology, and embryology, followed by a discussion of the genetic process causing these diseases. Diagnosis and workup is then discussed, followed by specific details of the surgical treatment of these malformations. The article concludes with a brief review of the complications and long-term outcomes in these children.

Development of the vestibular system and balance function: differential diagnosis in the pediatric population.

Dizziness is a rare complaint among children. In this article, the authors present the embryology and development of the vestibular system, and offer a rational approach to taking a careful history and ordering and interpreting appropriate vestibular and balance testing in children. A differential diagnosis is presented, so that the likely cause of the balance disorder can be elucidated even in the most complex pediatric patients.

Solid cervical ectopic thymus in an infant.

Cervical ectopic thymus presenting as a neck mass is rare in a neonate. Just more than 100 cases have been reported in the literature with less than 10% occurring in infants. We report a case of solid cervical ectopic thymus in an asymptomatic 2-month-old boy. We review the literature and discuss the embryology, pathophysiology, diagnosis, and management of an infantile ectopic thymus.

Unusual neck sinus--first or second cleft?

A sinus tract presenting with an opening around the angle of mandible is suggestive of first cleft remnant. We present the case of a 4-year-old boy with a recurrent discharging sinus around the angle of the right mandible whose internal opening was near the tonsil on imaging. Complete excision was performed with facial nerve monitoring. We discuss technical aspects of the surgery and possible embryology.

Fluoroscopic and CT fistulography of the first branchial cleft.

We present an unusual case of a complete first branchial cleft fistula communicating between the external auditory canal and the skin near the angle of the mandible. CT and fluoroscopic fistulography were used to establish the presence and course of the tract and to assist in surgical planning. The embryology and classification of first branchial cleft anomalies are discussed, with emphasis on the impact of imaging.

A review of harlequin ichthyosis.

Harlequin ichthyosis is an extremely rare and historically lethal congenital disorder of the skin caused by abnormal keratinization. This article reviews the embryology and currently understood pathophysiology of the disease, as well as current methods used to diagnose and treat these infants. There are serious implications for the family to consider: the high risk that their newborn will die soon, as well as future family planning issues.

Laparoscopic treatment of a bilobed gallbladder: a case report and review of the literature.

A double or bilobed gallbladder is a rare congenital anomaly. Because other congenital vascular and biliary duct anomalies may accompany this pathology, open cholecystectomy was thought to be the best treatment of symptomatic patients. In this paper, we report a patient with a bilobed gallbladder whose symptoms were successfully treated with laparoscopic cholecystectomy. We also discuss the characteristics and the embryology of this rare anomaly.

MR imaging of congenital anomalies of the pediatric spine.

Congenital anomalies of the pediatric spine are ideally evaluated with MR imaging. The wide spectrum of pathology in this category of disease is dramatically displaced for both diagnosis and treatment planning for the neurosurgeon. An understanding of embryology and knowledge of the clinical presentation combined with the MR imaging findings allows the radiologist to play a key role in the evaluation of these complex anomalies.

Congenital lateral cervical cysts of infancy.

The etiology of lateral cervical cysts in infancy is complex. The cysts are congenital in origin and are derived from the third or fourth branchial apparatus. The authors describe two cases of congenital lateral cervical cysts with an internal opening in the pyriform sinus and possible origin in the third branchial arch. The embryology, clinical presentation, and operative management of this condition is reviewed.
