Cholera epidemics in 2010: respective roles of environment, strain changes, and human-driven dissemination.

The cholera burden has grown strikingly during the past 4 years, and has spread to countries previously spared by this disease. The current spread has proved especially violent, as illustrated by the recent deadly epidemics around the Lake Chad Basin, in East Africa, and in Haiti. This onset of severe cholera epidemics is part of the overall dynamic of the current seventh cholera pandemic, composed of successive epidemic waves. The current wave is attributable to new atypical El Tor strains, which spread from the Bay of Bengal to Papua in the east, Africa, and the Caribbean Sea in the west, an…

Costs and impact of meningitis epidemics for the public health system in Burkina Faso.

BACKGROUND: Epidemic meningococcal meningitis remains a serious health threat in the African meningitis belt. New meningococcal conjugate vaccines are relatively costly and their efficiency will depend on cost savings realized from no longer having to respond to epidemics. METHODS: We evaluated the cost and impacts to the public health system of the 2007 epidemic bacterial meningitis season in Burkina Faso through a survey at the different level of the health system. A micro-economic approach was used to evaluate direct medical and non medical costs for both the public health system and house…

Chikungunya virus-induced myopericarditis: toward an increase of dilated cardiomyopathy in countries with epidemics?

The acute stage of infection with chikungunya virus, which is characterized of fever, polyarthritis, and occasional rash, can be complicated by myocarditis, as reported in a 21-year-old woman. Persisting changes on cardiac magnetic resonance imaging one year after disease onset could lead to delayed myocardial damage. An unexpected delayed increase in dilated cardiomyopathy may be observed in countries affected by the outbreak of chikungunya virus disease during 2005-2007.

Tackling STI epidemics through the HIV clinic: is sex high enough on the agenda?

Sexually transmitted infection (STI) rates among men having sex with men continue to increase. HIV services may operate independently to genitourinary medicine clinics and the sexual health of HIV-positive patients may be of low priority in the context of medical problems related to HIV. A prospective study of HIV-positive gay men was conducted in a London outpatient clinic over a three-month period. Data were available for 90 men. Forty-five percent had STI screens in the preceding six months. These revealed a high rate of infections; 26 infections diagnosed in 14 men in the study period. Fif

Visceral leishmaniasis/human immunodeficiency virus co-infection in India: the focus of two epidemics.

India contributes heavily to the global burden of visceral leishmaniasis (VL, kala-azar) and human immunodeficiency virus (HIV)/AIDS. The prevalence of HIV seropositivity in VL patients at a tertiary care centre in northern India, as observed during a prospective study over a period of 2 years, is presented. Of the 104 cases of VL/post-kala-azar dermal leishmaniasis, six (5.7 %) were found to be HIV positive, compared to 11 (5.5 %) seropositive for HIV of 198 patients with fever due to other causes. Four of the six (67 %) VL/HIV co-infected patients had a chronic/relapsing course, not respondi

A myth too tough to die: the dead of disasters cause epidemics of disease.

Myths abound in the practice of health care, death, and disease. Akin to the old adage of swallowing camels and straining gnats, the myth that mass fatalities cause epidemics of disease following natural or other disasters is alive and well. Despite the findings of observers, microbiologists, epidemiologists, and other scientists, even medical doctors and public health professionals lend support to the ancient belief and rush into mass graves or mass cremations the bodies of those victims of trauma in a disaster. Putting this myth to rest depends on use of information concerning the transmissi…

Syndromic surveillance in the Netherlands for the early detection of West Nile virus epidemics.

West Nile virus (WNV) is an arthropod-borne flavivirus that is endemic in Africa, Europe, and Eastern Asia. The recent introduction and rapid dissemination of the virus in the United States as well as an increase in WNV outbreaks in Europe, has raised concerns for its spread in Europe. A surveillance system was developed to allow timely detection of an introduction of WNV infections in The Netherlands. This program focuses on cases presenting with neurological disease and includes the monitoring of hospital discharge diagnoses, trends in cerebrospinal fluid (CSF) diagnostic requests, laborator

[Changing trends in HIV epidemics: recent diagnoses in the Spanish VACH cohort (2000-2002)].

BACKGROUND: Studying the changing trends of HIV epidemics is a useful means of evaluating the results of current preventive plans as well as of defining future needs and objectives. METHODS: We performed a cross-sectional study of the newly-diagnosed cases of HIV infection included in the Spanish VACH cohort. New HIV cases were defined as those diagnosed between January 2001 and December 2002. Their epidemiologic characteristics were compared with those of patients included in the same cohort who had been diagnosed between January 1998 and December 2000. RESULTS: We studied 603 new cases (27…

Scaling properties of childhood infectious diseases epidemics before and after mass vaccination in Canada.

The goal of this paper is to analyse the scaling properties of childhood infectious disease time-series data. We present a scaling analysis of the distribution of epidemic sizes of measles, rubella, pertussis, and mumps outbreaks in Canada. This application provides a new approach in assessing infectious disease dynamics in a large vaccinated population. An inverse power-law (IPL) distribution function has been fit to the time series of epidemic sizes, and the results assessed against an exponential benchmark model. We have found that the rubella epidemic size distribution and that of measles

A study of excess mortality during influenza epidemics in the Czech Republic, 1982-2000.

Influenza related mortality rates have been established in many countries; nevertheless, studies focusing on the Central European population have been rare to date. We assess mortality attributable to influenza by comparing all cause mortality and mortality due to diseases of the circulatory system during influenza epidemic and non-epidemic periods, as defined by acute respiratory infection surveillance data. Data on total mortality, mortality due to diseases of the circulatory system and surveillance data for influenza and other respiratory infections were used in a general linear model with

Strengthening health services to control epidemics: empirical evidence from Guinea on its cost-effectiveness.

OBJECTIVE: To assess the cost-effectiveness of control measures implemented against epidemics in Guinea, West Africa. METHODS: We collected all routine data available on incidence, mortality, control measures implemented and their cost during epidemics of cholera, measles and meningococcal meningitis in 1993-95. Then we estimated for one prefecture the effectiveness and cost-effectiveness of epidemic control measures for three scenarios: (i) 'natural' situation, (ii) 'routine' health services and (iii) 'intervention'. Where uncertainty was considerable, we used sensitivity analysis and estima…

Two HIV-1 epidemics in Argentina: different genetic subtypes associated with different risk groups.

This study determined the risk behaviors and viral subtypes of HIV-1 found in 134 heterosexual HIV-seroprevalent maternity patients, 41 of t … HMA subtype F. Ninety percent of the MSM samples were subtype B. There are at least two independent epidemics of HIV-1 infection in Buenos Aires, Argentina. One, in heterosexual men and women, is dominated by env subtype F whereas the other, in homosexual men, is dominated by env subtype B, as determined by env HMA.

Preventive immunisation could reduce the risk of meningococcal epidemics in the African meningitis belt.

Control of meningitis epidemics is based on early case detection followed by mass campaigns of immunisation. However, this strategy showed severe inadequacies during recent outbreaks in Africa. In Niamey, Niger, meningococcal vaccinations began in 1978 and detailed bacteriological and epidemiological surveillance of meningitis started in 1981. When vaccine coverage rates were higher than 50%, the prevalences of Neisseria meningitidis A meningitis were low in Niamey, although there was a concurrent epidemic in rural Niger. A massive outbreak of meningitis in Niamey in 1994-1995 followed a 6-yea…

The dynamics of scarlet fever epidemics in England and Wales in the 19th century.

There was a marked rise in scarlet fever mortality in England and Wales in the mid-nineteenth century and spectral analysis of the registration details, 1847-80, shows that the interepidemic interval was 5-6 years, but after 1880 the endemic level fell and the fatal epidemics disappeared. The dynamics of the scarlet fever epidemics can be represented by a linearized mathematical model and because the system is lightly damped, it could be driven by an oscillation in susceptibility. Epidemics were significantly correlated with dry conditions in spring/summer (P < 0.001), suggesting that these pr…

An hypothesis for the periodicity of smallpox epidemics as revealed by time series analysis.

Parish registers have been studied by time series analysis to detect smallpox epidemics in England during 1600-1800. Confirmatory evidence was provided by the seasonality of child mortality. A 5-year cycle in smallpox epidemics was detected in medium-sized, rural towns. Consideration of the mathematics of the dynamics of viral diseases suggests that the true interepidemic period where smallpox is endemic should be 2-3 years and it is concluded that, in the towns studied, the disease was not endemic but that the oscillations were established by 5-year cycles of periods of famine associated with…

Epidemiological patterns of cutaneous leishmaniasis in Panama. I. Epidemics among small groups of settlers.

Epidemic of cutaneous leishmanniasis among a small group of people established in a Panamanian forested area was studied. The first settlers arrived in June 1971, and by June 1973 there were 37 persons, 33 of whom had no previous history of the disease. Epidemiological surveys were made in June 1973 and March 1974. The disease developed in 14 (42.4%) of the 33 susceptible persons by June 1973, with an exposure time ranging from one to 14 months. The last case occurred in April 1973, and no new cases were observed during the ensuing 11 months. Two (15.4%) of 13 dogs examined were found with cut
