Twenty-six patients with necrotizing fasciitis were treated during a nine-year period. Twenty-one (81%) had diabetes mellitus and 19 (73%) had gas in their soft tissues from nonclostridial infection. Although crepitation was found by physical examination in only five patients, roentgenographic examination of the involved site disclosed gas in every patient in whom it was subsequently found at surgery. Previous studies have not used roentgenography to search for soft-tissue gas. The 21 diabetic patients in our series had a mortality of 19%, the lowest yet reported. This suggested that roentgeno

Necrotising eyelid inflammation, also known as necrotising fasciitis (NF) of the orbit, is a rare but severe soft tissue infection. Prompt and appropriate treatment is crucial, as delayed intervention can result in the bacteria entering the bloodstream. Typical predisposing factors for NF include skin breaks after surgery or trauma, which may be small and overlooked. NF is mainly caused by group A beta-haemolytic streptococci, with significant cases also attributed to Pseudomonas aeruginosa and Staphylococcus aureus.Symptoms include swelling, fever and pallor in the orbital region. Untreated N

Necrotizing fasciitis is a rapidly progressive and life-threatening soft tissue infection usually caused by bacteria, with a high mortality of up to 80% when untreated. Any delay in time further increases the risk of mortality. Thus, early recognition and treatment are of upmost importance. The clinical assessment, consideration of laboratory results and surgical exploration are important diagnostic procedures. Cross-sectional imaging is highly sensitive with respect to assessment of the extent (computed tomography) and differential diagnostic considerations (magnetic resonance imaging). In in

BACKGROUND: Subgaleal abscess (SGA) is an uncommon but potentially life-threatening condition characterized by pus accumulation between the galea aponeurotica and skull periosteum. Necrotizing fasciitis (NF) of the head and neck is an aggressive infection with high morbidity and mortality. While SGAs have been reported following trauma, neurosurgical interventions, or regional infections, recurrent SGA secondary to cervicofacial NF (CNF) has not previously been documented.

INTRODUCTION: Retroperitoneal necrotizing fasciitis is a rare, rapidly progressive, and often fatal infection of the retroperitoneum. In many cases the source of infection is unclear, and cutaneous signs of necrotizing fasciitis may be absent.
CASE REPORT: We present the case of a 64-year-old female with a history of hypertension, hyperlipidemia, and breast cancer who developed acute kidney injury (AKI) and retroperitoneal necrotizing fasciitis following a 20-mile bike ride. The patient's initial symptoms included severe muscle aches, nausea, vomiting, and flank pain. Diagnostic imaging and l

A 75-year-old male with a history of poorly controlled diabetes, hypertension, coronary artery disease, chronic obstructive pulmonary disease, and obesity presented with severe bilateral periorbital edema with necrosis and purulent discharge. Although hemodynamically stable, laboratory markers indicated systemic toxicity. Imaging showed bilateral periorbital edema extending into the frontal scalp, temporal fossa, and right orbit. Suspected to have necrotizing fasciitis, the patient underwent urgent debridement of bilateral upper and lower eyelids and was found to have postseptal extension of n

PURPOSE: Necrotizing fasciitis, a severe soft tissue bacterial infection, is uncommon in the periocular region due to its rich blood supply. This report highlights a rare case in an immunocompetent infant.
OBSERVATIONS: A 10-month-old immunocompetent female exhibited fever, irritability, and right eyelid swelling post-fall. Despite initial treatment with cephalexin for presumed cellulitis, her condition rapidly deteriorated, suggesting necrotizing fasciitis. She stabilized after receiving broad-spectrum intravenous antibiotics and aggressive surgical debridement but later required orbital exe

Group A Streptococcus (GAS), particularly Streptococcus pyogenes (S. pyogenes), is a significant human pathogen responsible for infections often ranging from mild superficial conditions to severe, life-threatening diseases like necrotizing fasciitis (NF) and streptococcal toxic shock syndrome (STSS). This case report details the rapid deterioration of a previously healthy 49-year-old woman who presented with localized symptoms in her left thigh, later escalating to septic shock and multi-organ failure related to GAS infection. Initial evaluations indicated significant inflammation and acute ki

INTRODUCTION: Necrotizing fasciitis (NF) is a life-threatening infection and a surgical emergency. Not all clinicians have the experience or resources to detect NF in its early stages.
OBJECTIVE: To develop a diagnostic algorithm for primary care and emergency physicians to identify patients with possible NF, including an initial approach to triaging such individuals.
METHODS: Medline was searched to identify studies of validated algorithms for NF diagnosis and/or cohort or case series providing clinical and diagnostic features of NF. Candidate algorithms were validated via application to 3

OBJECTIVES: Aggressive surgical debridement is required in cervical necrotizing fasciitis, and in severe defects, subsequent free tissue transfer might be necessary. However, there is concern that the inflammatory environment of the infection site may threaten free flap viability, particularly with concerns for thrombosis of feeding vessels and compromised tissue integration. Cases in the head and neck area are rare, so there are limited data regarding outcomes of free tissue transfer in these patients.
METHODS: A retrospective chart review assessed patients with cervical necrotizing fasciiti

BACKGROUND: Empyema is a serious complication of lung surgery.
CASE PRESENTATION: We report a case of Streptococcus anginosus empyema complicated by descending necrotizing fasciitis after surgery. Ten days after the initial surgery, the patient presented with arrhythmia and hypotension with septic shock. Pleural fluid analysis led to the diagnosis of empyema, and surgical debridement (second surgery) was performed. One week after the emergency surgery, a palpable pink erythematous skin change was observed on the ipsilateral iliac crest. Hence, the second emergency surgery (third surgery) was

Necrotizing fasciitis (NF) is a devastating soft tissue infection affecting fascias and subcutaneous soft tissues. While it is associated with several risk factors, including malignancy, alcoholism, liver disease, drug use, malnutrition, diabetes, male gender and old age, few case reports in the literature describe its rare connection with genital malignancy. Vulvar squamous cell carcinoma (SCC) is the fourth most common malignancy, representing 5% of all gynaecological tumours among women. NF due to vulvar SCC is a rare complication. In this article, we present the 1991 case of a 58-year-old

(Peri)orbital infections comprise a multitude of diagnoses, ranging from common hordeolum to rare but life-threatening necrotizing fasciitis. However, these disease entities are rarely diagnosed by an ophthalmic pathologist because (peri)orbital infections are usually diagnosed clinically, with the help of imaging and microbiological techniques when indicated. In this review article, the role of ophthalmopathology in the diagnosis of (peri)orbital infections is illustrated on the basis of several exemple diagnoses. An infectious hordeolum must be distinguished from a noninfectious chalazion. A

Lucio phenomenon is a rare vasculopathy that can occur in patients with Hansen disease, particularly diffuse lepromatous leprosy. It is characterized by retiform purpura and necrotic ulcerations, most commonly affecting the extremities. Diagnosing Lucio phenomenon can be challenging, especially when secondary bacterial infections occur. We report a patient with Lucio phenomenon who presented with acute necrotizing fasciitis of his left upper extremity and a 10-year history of chronic ulcerations. Shortly following admission, he also developed acute kidney injury. The necrotizing fasciitis was

Acute uncomplicated appendicitis is a common surgical disease that has been well-studied, and its overall mortality has decreased over time. However, delay in treatment can be associated with rare complications such as necrotizing fasciitis, which carries a high mortality rate, and bladder perforation. We present such a case in an 81-year-old female with no significant surgical history who presented to the emergency department with four days of abdominal pain. A CT scan revealed extensive subcutaneous air in the abdominal wall, an inflamed appendix, and a periappendiceal abscess. During subseq

CONTEXT: The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score helps to diagnose necrotizing soft-tissue infection (NSTI). The LRINEC score has been reported to be associated with poor prognosis, although few studies have evaluated this association.
AIMS: We aimed to describe the characteristics of NSTI and assess whether the LRINEC score was associated with mortality and amputation.
SETTINGS AND DESIGN: We conducted a retrospective observational study from January 2007 to May 2018, in a Japanese tertiary care hospital.
SUBJECTS AND METHODS: Patients with NSTI were identifi

BACKGROUND: Necrotizing fasciitis (NF) is a rare and life-threatening necrotizing skin and soft-tissue infection. Infectious pathogens of NF must be detected early and treated rapidly to prevent loss of limb or a fatal outcome. This study aimed to detect more reliable predictors between gram-negative and gram-positive monomicrobial NF of limbs.
METHODS: A total of 100 patients with limb monomicrobial NF were diagnosed prospectively from April 2015 to July 2018. These monomicrobial NF pathogens can be divided into gram-negative and gram-positive groups according to the result of Gram staining
