BACKGROUND: Oligohydramnios represents a physiopathologic process, associated to a high rate of pregnancy complications and increased fetal morbidity and mortality. The purpose of this study is to determine whether amniotic fluid ultrasound evaluation is a useful parameter to identify pregnancies at risk for fetal distress.
METHODS: 91 pregnant women with oligohydramnios were followed, and fetal conditions monitored with cardiotocography and ultrasounds. Delivery way and new-born conditions at birth were considered too.

Using cardiotocogram and umbilical arterial gas analysis, the presence or absence and variations of umbilical cord coiling were examined with relation to the incidence of variable deceleration (V.D.) and neonatal asphyxia. Obstetric cholestasis has been associated with a high incidence of stillbirth and perinatal complications. Between 1975 and 1984, 83 pregnancies were complicated by cholestasis. Meconium staining occurred in 45%, spontaneous preterm labour in 44%, and intrapartum fetal distress in 22%. Of 86 infants two were stillborn and one died soon after birth. This case report documents the antepartum detection of congenital complete heart block in the fetus of an adolescent primigravida. Management utilizing direct fetal electrocardiogram and fetal scalp sampling is discussed. The report illustrates the appropriate management of a fetus in utero with congenital complete heart block and suggests procedures to be taken prior to delivery in the expectant management of a neonate with a congenital conduction defect.

The present report considers two questions: 1) Which patients should be monitored routinely and 2) the interpretation of antenatal cardiotocography. During 26 months 812 patients with risk pregnancies were monitored with antenatal CTG. Most of them were daily registered. One hundred and seventeen (14.5%) showed pathological CTG changes. A patient with Marfan's syndrome suffered aortic dissection with subsequent aortic arch replacement and coronary artery bypass grafts during pregnancy. Antepartum therapy consisted of bed rest, heparin anticoagulation, propranolol and fetal evaluation. After amniocentesis to determine fetal lung maturity, cesarean section with epidural anesthesia and invasive hemodynamic monitoring was performed at 34 weeks' gestation, resulting in delivery of a viable girl without demonstrable congenital abnormalities. The maternal postoperative course was uneventful. This report is the first of such a case.

Fetal breathing activity was monitored by ultrasonography in 14 patients with oligohydramnios (4 with fetal anuria and 10 with premature rupture of the membranes). 45 patients with intact membranes, with normal amniotic fluid volume, and matched for gestational age were the controls. Fetal growth restriction is a complex problem in modern obstetrics. It is a condition of suboptimal fetal growth based on a genetically predetermined potential and affects approximately 5% to 10% of pregnancies. It is traditionally defined as an estimated fetal weight less than the 10th percentile.
