Assessment and treatment of co-occurring eating disorders in publicly funded addiction treatment programs.

OBJECTIVE: Publicly funded addiction treatment programs were surveyed to increase understanding of treatment options for persons with co-occurring eating and substance use disorders. METHODS: Data were collected between 2002 and 2004 from face-to-face interviews with program directors of a nationally representative sample of 351 addiction treatment programs. RESULTS: Half of the programs screen patients for eating disorders; 29% admit all persons with eating disorders, and 48% admit persons with eating disorders of low severity. Few programs attempt to treat eating disorders. Programs that a

Health status, trends, and issues in Sri Lanka.

It is widely recognized that better health is a prerequisite for the overall economic and social development of a nation. Sri Lanka, like many other countries experiencing the epidemiological transition, will have to make effective decisions on health-care service management and the development of education and training programs for health-care professionals. This paper provides a comprehensive review of current health service administration, health status, trends and issues, and health financing and resource allocation in Sri Lanka. The review revealed that Sri Lanka has achieved a relatively

Implications of Race Adjustment in Lung-Function Equations.

BACKGROUND: Adjustment for race is discouraged in lung-function testing, but the implications of adopting race-neutral equations have not been comprehensively quantified. METHODS: We obtained longitudinal data from 369,077 participants in the National Health and Nutrition Examination Survey, U.K. Biobank, the Multi-Ethnic Study of Atherosclerosis, and the Organ Procurement and Transplantation Network. Using these data, we compared the race-based 2012 Global Lung Function Initiative (GLI-2012) equations with race-neutral equations introduced in 2022 (GLI-Global). Evaluated outcomes included na

A feasibility study of breast cancer genetic risk assessment in a federally qualified health center.

BACKGROUND: The US Preventive Services Task Force (USPSTF) endorses routine screening for genetic risk of breast and/or ovarian cancer as a component of primary health care. Implementation of this recommendation may prove challenging, especially in clinics serving disadvantaged communities. METHODS: The authors tested the feasibility of implementing the USPSTF mandate at a federally qualified health center (FQHC) to identify women who were eligible for genetic counseling (GC). A 12-month usual-care phase was followed by a 12-month intervention phase, during which time cancer genetic risk asse

Financing immunization of adults in the United States.

Immunization is one of the most effective and cost-effective prevention measures available. As a result of universal vaccination of children, polio has been eliminated in the United States and much of the world, measles and rubella are no longer endemic diseases in the United States, and most of the other vaccine-preventable diseases of childhood are at or near record lows. A recent review of clinical preventive services by Partnership for Prevention gave childhood immunization a perfect score of 10, based on clinically preventable burden and cost-effectiveness.

Conducting stroke research with an exception from the requirement for informed consent.

BACKGROUND: Obtaining viable informed consent from stroke patients for participation in clinical trials of acute stroke therapies is often problematic because of patients' neurological deficits. Furthermore, obtaining permission from surrogates is often not possible or not legally permissible. SUMMARY OF REVIEW: In 1996 the Food and Drug Administration and Department of Health and Human Services published regulations that allow investigators to conduct emergency research without patient consent under a narrowly defined set of circumstances. We review requirements of these regulations, paying

Rheumatic Heart disease in the Young: Treatment needs, Hospitalization burden, and Measuring economic impact: The RHYTHM Study.

INTRODUCTION: Acute rheumatic fever (ARF) and its sequela, rheumatic heart disease (RHD), continue to be major contributors to morbidity and mortality among acquired pediatric cardiac diseases in underprivileged populations. The resulting health expenditures lead to catastrophic financial strain on affected families. MATERIALS AND METHODS: This study analyzed 48 children admitted with ARF or RHD. Data collection focused on total healthcare costs, out-of-pocket expenditures (OOPE) by families, and associated financial impact. RESULTS: A total of 70 hospital admissions were evaluated. The mean

Authorized Seclusion, Restraint, and Safeguards in Home and Community Based Medicaid Programs for Children With Serious Emotional Disturbance.

1915(c) Home and Community-Based Services (HCBS) Medicaid waivers allow states to finance community-based alternatives to institutional psychiatric care for youth with Serious Emotional Disturbance (SED), including diversion from Psychiatric Residential Treatment Facilities (PRTFs). Although federal HCBS policy emphasizes freedom from coercion and restraint, states define which seclusion, restraint, and restrictive interventions (SR/RI) are permitted and what safeguards apply. Waiver applications therefore provide a systematic source for examining how states structure SR/RI authorization and s

Integrated care programmes for severe mental illnesses and comorbid physical health disorders: the EU-MIND umbrella review to inform future actions in Europe.

BACKGROUND: People with severe mental illness (SMI) represent up to 6% of the population and experience a high burden of physical comorbidity and premature mortality. Despite longstanding international calls to develop integrated care, the optimal organisational approaches for this population remain poorly defined, and health systems lack guidance on how to redesign care pathways. This review aimed to characterise the core components, reported impacts, and implementation challenges of existing integrated care programmes for individuals with SMI and physical health conditions. METHODS: This um

Malaria Elimination in Suriname, Timor-Leste and Georgia: Strategic Lessons for Sub-Saharan Africa.

BACKGROUND: Malaria remains a major cause of morbidity and mortality worldwide, with over 263 million cases and 597,000 deaths estimated in 2023, of which 94% occurred in Sub-Saharan Africa. Despite this heavy burden, several low- and middle-income countries have recently succeeded in eliminating local malaria transmission. OBJECTIVE: This article describes and synthesises malaria elimination experiences in Suriname, Timor-Leste and Georgia, all three certified malaria-free by the World Health Organization in 2025, to identify strategic lessons and contextual factors that may inform eliminati

Approaches to implementing and financing primary health care in Kenya: a case of seven counties.

BACKGROUND: Kenya has prioritized Primary Health Care as an indispensable foundation to realize UHC by 2022. Integral to this approach's operationalization is the adoption of the primary health care networks (PCNs) model to strengthen service delivery efficiency and coordination. The PCNs are coordinated by a Multidisciplinary Team (MDT). The MDT is dynamic and should comprise a care and support team that matches patient health needs and the catchment population. This scholarly article delineates the outcomes of an investigative assessment reviewing the current state and trajectory of PHC impl

Brain health as a global priority.

Brain health is an evolving concept that has become increasingly popular within clinical and academic centers, journalism and the general public. It can be defined as the state of brain functioning across cognitive, sensory, social-emotional, behavioural and motor domains, allowing a person to realize their full potential over the life course. Multiple, interconnected determinants play a role in shaping brain health from pre-conception through the end of life. Brain health can be optimized by addressing the following determinants: physical health, healthy environments, safety and security, lea

Global support for new vaccine implementation in middle-income countries.

Middle-income countries (MICs) as a group are not only characterized by a wide range of gross national income (GNI) per capita (US $1026 to $12,475), but also by diversity in size, geography, governance, and infrastructure. They include the largest and smallest countries of the world-including 16 landlocked developing countries, 27 small island developing states, and 17 least developed countries-and have a significant diversity in burden of vaccine-preventable diseases. Given the growth in the number of MICs and their considerable domestic income disparities, they are now home to the greatest
