The authors report a case of recurrent priapism treated by saphenocavernous bypass and accompanied by a congenital internal pudendal arteriovenous fistula. In the light of this case and the literature, they discuss the physiopathology of priapism and the sound basis of the various therapeutic methods proposed.

Extraoral fistulas, whether primary or as the residue of an abscess, have different causes. According to case reports in the literature and from clinical experience the dentogenous cause is the more frequent. For this reason a possible tooth involvement should be ruled out before therapy, loss of dental pulp of various origin and periodontal diseases should be excluded, otherwise the treatment must begin with the cause, after which further intervention is usually superfluous.

Preoperative, operative and postoperative findings in 97 cases of labyrinthine fistula are presented. The majority of patients had had symptoms of chronic otitis media for 20 years or more and manifested some degree of sensorineural hearing impairment. Two-thirds had experienced dizziness. Parotid fistula is most commonly a posttraumatic situation. In posttraumatic cases, spontaneous closure of the fistula is the general rule. Conservative approaches to the occurrence of a parotid fistula are eliminating oral intake by the patient and applying a pressure dressing while maintaining nutrition by the intravenous route. Anticholinergic drugs decrease the production of saliva and thus would appear to be beneficial. Our experience in treating ten full-term and six premature infants with esophageal atresia and distal tracheoesophageal fistula was reported. Fourteen cases were treated by a one-stage procedure and two cases by a two-stage procedure. One patient died due to pulmonary infection following the one-stage procedure and one died of septicemia after the two-stage procedure. Anastomotic stricture developed in four cases and was treated successfully by dilatation.

Seventy-five percent of carotid-cavernous sinus fistulae result from high velocity head trauma. The most common symptom is a pulse synchronous bruit which usually becomes apparent when the patient regains his sensorium. The objective manifestations of such an arteriovenous fistula are characteristically localized to the ipsilateral orbit. A carotid-cavernous fistula in pregnancy is not rare. Increasing pain and worsening of the proptosis made prompt operative intervention necessary. The fact that other known cerebrovascular lesions lend themselves to surgical attack without undue risk during this time8--11 prompted us to proceed with surgery. There was no adverse effect on the pregnancy, other than premature delivery. Both the mother and child are doing well 4 years after surgery.

Two patients having the abdominal aneurysm with aortoduodenal fistula were treated surgically, but graft infection occurred. One patient died about 6 months after and the other about one year after the operation due to massive hemorrhage from the postoperative aortointestinal fistula. The difficulties in the treatment of the aortoduodenal fistula were discussed.

Complete excision of the trophic secretomotor parasympathetic nerve supply of the parotid gland produces abolition of its salivary secretion and ends in atrophy of the gland units. Arteriovenous fistula is now an accepted method of vascular access for hemodialysis in chronic end-stage renal disease. The arteriovenous fistula constructed is based on the status of the forearm vessels. Our results were satisfactory and comparable to other dialysis centers.
