Extensive Isolated Intracranial Recurrence of Primary Mediastinal B-Cell Lymphoma on FDG PET Imaging.

Recurrent lymphoma tends to recur at the initial site of involvement or spread to the lymph nodes, but rarely involves the central nervous system. We present an interesting case of isolated recurrence of primary mediastinal B-cell lymphoma in the brain, confirmed on PET imaging, in a 15-year-old girl previously in remission.

Nuclear medicine techniques in gynaecological malignancy: a review of best practice.

Gynaecological malignancies are increasing in incidence in the UK, primarily driven by a rise in cases of endometrial cancer. The most common in order of prevalence thereafter are ovarian, cervical, vulval and vaginal cancers. Imaging is fundamental to their diagnosis, staging, response assessment and surveillance; and has significant implications in management by guiding local, regional or systemic treatment strategies. Advanced nuclear medicine imaging modalities, in particular [18F]-fluoro-2-deoxy-D-glucose (FDG) positron-emission tomography (PET)/computed tomography (CT), have an evolving

FDG PET/CT in a Case of α-Fetoprotein-Producing Urothelial Carcinoma With Hepatoid Differentiation of the Renal Pelvis.

An α-fetoprotein-producing tumor in the urinary tract is rare. We describe FDG PET/CT findings in a case of α-fetoprotein-producing urothelial carcinoma with hepatoid differentiation of the left renal pelvis with a significantly elevated serum α-fetoprotein level (1035 ng/mL). FDG PET/CT showed intense activity of the left renal pelvic tumor with invasion of the renal parenchyma. This case indicates that α-fetoprotein-producing urothelial carcinoma with hepatoid differentiation should be considered in the differential diagnosis of FDG-avid tumors of the urinary tract in patients with elevated

Evidence-based role of PET imaging in cardiac and systemic sarcoidosis: an umbrella review of the current literature.

BACKGROUND: Sarcoidosis is a multisystem granulomatous disorder characterized by diverse clinical presentations and imaging findings, usually affecting the lungs and lymph nodes and skin and less commonly the heart and other organs. Positron emission tomography (PET), especially with [18F]FDG, has increasingly been used to improve the detection, risk stratification, and therapeutic monitoring of both cardiac sarcoidosis (CS) and systemic sarcoidosis (SS). METHODS: This umbrella review systematically examined published systematic reviews and meta-analyses reporting the evidence-based performan

A Primitive Endobronchial Tumor Derived by Salivary Gland on FDG PET/CT.

A 55-year-old man presented with a 2-week history of cough, expectoration, and transient hemoptysis. Contrast-enhanced CT revealed a mildly heterogeneous soft-tissue nodule in the left upper bronchus, causing distal obstructive atelectasis. Bronchoscopy identified an endoluminal tumor at the left upper lobe orifice; initial biopsy suggested adenoid cystic carcinoma. Staging 18 F-FDG PET/CT demonstrated increased FDG activity in the nodule with no evidence of metastatic disease. Following surgical resection, histopathology confirmed a salivary gland-type tumor: intermediate-grade mucoepidermoid

Pulmonary Eosinophilia Mimicking Metastatic Lung Lesions on FDG PET-CT: A Diagnostic Pitfall.

Eosinophilic pneumonia (EP) is a rare disease, comprising a group of heterogeneous diseases. The 2 main subtypes are acute eosinophilic pneumonia (AEP) and chronic eosinophilic pneumonia (CEP), both of which are characterized by accumulation of eosinophils in lung tissues, BAL fluid, and metabolically active nodular lesions in FDG PET-CT scan. Here we present a case of 67-year-old woman, diagnosed with carcinoma cervix and staging whole body PET-CT scan suggestive of metabolically active nodular lesions in bilateral lungs with perilesional ground-glass haziness apart from primary lesion in cer

Sjögren Syndrome Associated Limbic Encephalitis and Its Longitudinal Brain FDG PET Manifestations.

A 62-year-old woman with Sjögren syndrome, rheumatoid arthritis, and autoimmune hepatitis presented with subacute confusion and generalized seizures. MRI revealed bilateral mesial temporal T2/FLAIR hyperintensity. Serology revealed anti-SSA and anti-SSB antibodies elevated at 5 times above the upper limit of normal. FDG-PET demonstrated marked relative mesial temporal hypermetabolism with widespread hypometabolism in the rest of the brain; z-score mapping confirmed focal mesial temporal hyperactivity. They received immunotherapy and antiseizure therapy. After 1 year, cognition improved, though

Head-to-head Comparison of [68Ga]Ga-FAPI-04 PET and [18F]FDG PET in the Evaluation of Primary Gastrointestinal Tumors and Metastases: A Systematic Review and Meta-analysis.

RATIONALE AND OBJECTIVES: Gastrointestinal cancers pose a major public health challenge due to very aggressive malignancy, the quick and widespread metastases, relapse frequently in lymph node and omentum, and poor prognosis. [68Ga]Ga-FAPI-04 PET is widely used to diagnose gastrointestinal tumors. This study aimed to conduct a head-to-head comparison of the [68Ga]Ga-FAPI-04 and [18F]-FDG imaging methods for diagnosing primary gastrointestinal tumors and metastases. METHODS: The PubMed, Embase, Web of Science, and Cochrane Library databases were systematically searched up to 16 April 2024. The

FDG PET/CT Finding in Aqueous Humor Relapse by Acute Lymphoblastic Leukemia.

The manifestation of aqueous humor relapse by acute lymphoblastic leukemia (ALL) on 18 F-FDG PET/CT is rarely reported. Herein, we report a rare representation of bilateral anterior chamber involvement by relapsed ALL with hypermetabolic activity observed on FDG PET/CT. This case emphasizes the higher sensitivity of 18 F-FDG PET/CT compared with CT/MRI in detecting aqueous humor relapse in patients with ALL, highlighting its potential as a noninvasive diagnostic tool.

Diagnostic Performance of Radiolabelled FAPI Versus [18F]FDG PET Imaging in Hepato-Pancreato-Biliary Oncology: A Systematic Review and Meta-Analysis.

Radiolabelled fibroblast activation protein inhibitor (FAPI) tracers have the potential to overcome the limitations of 2-deoxy-2-[18F]fluoro-D-glucose ([18F]FDG) and improve the diagnosis and staging of hepato-pancreato-biliary (HPB) cancers. This study aims to compare the diagnostic performance of radiolabelled FAPI versus [18F]FDG PET imaging in HPB cancers. A systematic search of PubMed, Embase, Web of Science and Cochrane Library was performed to identify eligible studies on the diagnostic performance of FAPI PET for primary HPB tumours (hepatocellular carcinoma (HCC), pancreatic cancer (P

Is early-phase amyloid PET ready for use as neurodegeneration marker?

Alzheimer's disease is marked by accumulation of amyloid-β (Aβ) and abnormal tau, associated with subsequent neurodegeneration. Validated markers of neurodegeneration include brain glucose metabolism, perfusion and atrophy. Dual-phase amyloid PET imaging allows to assess both Aβ buildup and perfusion changes through a single tracer injection. Increasing evidence supports the comparability of early-phase amyloid PET scans and glucose metabolism changes assessed by 18F-FDG PET at group and individual levels, regardless of the amyloid tracers and the disease, emphasizing the added value of dual-p

Rare Neoplastic Meningitis in Cerebellar Medulloblastoma Detected by PET/CT.

Neoplastic meningitis, also known as leptomeningeal metastases, is a rare clinical entity seen in less than 1%-2% of primary nervous system tumors. Diagnosis of leptomeningeal metastases is difficult and is achieved by cytologic evidence of malignant cells in cerebrospinal fluid, or demonstration of radiologic abnormality. 18 F-FDG PET/CT can detect leptomeningeal metastases before anatomical changes. In this case, a 42-year-old man with cerebellar medulloblastoma underwent surgery and radiotherapy and had FDG uptake in 3 separate locations after 3 months. The findings indicated leptomeningeal

Negative 18 F-FDG and Positive 68 Ga-FAPI-04 Findings in a Patient With Acute Cholangitis.

68 Ga-FAPI PET demonstrates superior detection of primary lesions in cholangiocarcinoma compared with 18 F-FDG PET, making it commonly used in the diagnosis of biliary diseases. Herein, we reported the 18 F-FDG and 68 Ga-FAPI-04 PET/MR findings in a 38-year-old man experiencing acute cholangitis. The lesions exhibited negative 18 F-FDG uptake but intense 68 Ga-FAPI-04 uptake on PET/MR.

Unveiling Ovarian Cancer With [ 18 F]FDG and [ 68 Ga]Ga-FAPI-46 PET/CT : A Rare Case of Malignant Pericardial Effusion.

A 71-year-old woman presented with fatigue, dyspnea, and weight gain, suggestive of a serious systemic disease. CT revealed a large pericardial effusion, and subsequent [ 18 F]FDG PET/CT showed focal areas of elevated FDG uptake in the pericardium and moderate, symmetric FDG uptake in normal-sized ovaries. Pericardial effusion cytology indicated a gynecological malignancy. Additional [ 68 Ga]Ga-FAPI-46 PET/CT demonstrated more distinct uptake in the pericardium and lymph nodes. Histopathology of ovarian surgical specimens confirmed poorly differentiated ovarian cancer. This case highlights the

Metastasis of Breast Cancer in Myometrium, Detected Exclusively Through 18 F-FDG PET/CT.

We share the case of a 50-year-old woman with isolated myometrial metastasis of recurrent invasive breast ductal carcinoma. Eight years after initial diagnosis at stage IIIC, a hypodense myometrial tumor was spotted in whole-body CT, which was also noted as a lesion with high SUV in 18 F-FDG PET/CT. Myometrial metastatic mammary carcinoma was then pathologically proven after hysterectomy and bilateral oophorectomy. This case warns us to be alert to how easily malignant uterine lesions could be misjudged as benign in traditional imaging and, consequently, how irreplaceable PET/CT is in the earl

Brain FDG PET Findings Mimicking Limbic-predominant Age-related TDP-43 Encephalopathy.

Limbic-predominant age-related TDP-43 encephalopathy (LATE) is a recently recognized neurodegenerative disorder that frequently affects elderly patients and is often misdiagnosed as Alzheimer's disease. It typically presents with mild memory impairment and disproportionate hippocampal atrophy. FDG PET commonly shows distinct medial temporal lobe hypometabolism, which differs from Alzheimer's disease. We report a case with FDG PET findings suggestive of LATE; however, CT and MRI revealed hippocampal sulcus remnant cysts instead of hippocampal atrophy. This highlights the critical importance of

Arterial Occlusion in Takayasu Arteritis Detected as a Blood Pool Defect on 18 F-FDG Semiconductor PET/CT.

A 75-year-old woman with Takayasu arteritis was referred to 18 F-FDG PET/CT due to elevated C-reactive protein in her laboratory data. FDG PET/CT with semiconductor detectors revealed no uptake in the arterial walls, suggesting active inflammation. However, the blood pool uptake in the proximal portion of the left subclavian artery was defective, suggesting arterial occlusion. Magnetic resonance angiography revealed complete occlusion of the left subclavian artery. Ultrasonography showed the reverse flow of the left vertebral artery. Semiconductor PET can visualize the blood pool defects of th

The Appearance of a Secondary Bone Lesion in Place of Nonspecific Vertebral Anomalies Related to Mediastinal Venous Thrombosis: Coincidence?

A 66-year-old woman with locally advanced lung adenocarcinoma underwent FDG-PET/CT, which revealed superior vena cava invasion and thrombosis of the brachiocephalic veins. Tracer injection on the side of thrombosis demonstrated hypermetabolism in the dorsal spine, likely due to collateral venous drainage. Following chemoradiotherapy and early discontinuation of immunotherapy for toxicity, she developed cervicobrachial neuralgia 18 months later. MRI showed marrow replacement lesions and FDG-PET/CT identified bone recurrence at D1, correlating with previously nonspecific fixation abnormalities.

COVID-19 vaccine-associated reactive axillary lymph nodes on FDG PET/CT: a systematic review and meta-analysis.

PURPOSE: COVID-19 vaccine-induced reactive axillary lymph nodes (RAL) on fluorodeoxyglucose (FDG) PET/computed tomography (CT) imaging can mimic malignant lymphadenopathy, leading to diagnostic errors. Reported RAL prevalence varies widely in the literature, and factors contributing to its development remain poorly understood. This meta-analysis aims to evaluate vaccine-induced RAL observed on FDG PET/CT imaging and consolidate evidence from multiple studies to assess its prevalence, duration, and associations with vaccine-related factors and demographic characteristics.
