[Diabetic foot care. importance of education].

Diabetes is the most prevalent chronic disease at present. It is estimated that approximately 250 million people worldwide have diabetes, representing 5.9% of the adult population. According to a study published recently in Spain, the prevalence of type 2 diabetes varies between 10 and 15%. Cardiovascular complications associated with the disease produce a high social and health costs significantly reducing the quality of life of patients and their families and are the leading cause of death in developed countries. Complications of diabetes with higher economic costs are the "diabetic foot", w…

Pathology-designed custom molded foot orthoses.

Treating patients with custom foot orthoses for common pathologies is a rewarding experience when the proper steps are taken during foot casting and custom-orthosis prescription writing. This article describes successful methods for orthoses casting and prescription writing for custom-molded orthoses for Achilles tendonitis, pes planus, hallux limitus, plantar fasciitis/heel spurs, lateral ankle instability, metatarsalgia, and pes cavus. In addition, a summary of orthotic laboratory instructions for each pathology-designed custom orthosis is provided, which should be considered by orthotic lab…

Mycetoma (Madura foot): A case report.

INTRODUCTION: Mycetoma is a chronic disease, which is endemic in tropical and subtropical countries. We report a rare case of mycetoma located on the foot of a patient living in a temperate-climate country followed by a short discussion. OBSERVATION: A 50-year-old woman with painless swelling in her left foot. The swelling started after a banal penetrating injury on the sole of her left foot 23 years ago. X-rays images showed multiple osteolytic lesions of the tarsus. The histological analyses identified the fungus madurella mycetomatis. The treatment was radical surgery (amputation). The pat…

Unilateral cleft hand with cleft foot.

Congenital anomalies of the hand form an important class of congenital malformations. They have a huge functional importance because of the part played by the hand in the daily activities of a person. The deformities also have significant cosmetic significance and may also be associated with other anomalies. Amongst the congenital anomalies, central deficiency or cleft hand is relatively rare. The association of cleft foot with cleft hand is an even more rare occurance. We present a case report of a 6 year old child, born of a non-consanginous marriage, having congenital central deficiency of …

[Charcot foot. Current situation and outlook].

Neuro-osteoarthropathy is accompanied by the destruction of single or multiple joints involving little or no pain. Diabetic patients suffering from symmetric distal polyneuropathy are the main risk group. The main location is the foot (Charcot foot). Inadvertent or repetitive trauma appears to be the main trigger mechanism for Charcot foot. Undiagnosed and untreated fractures and dislocations, foot and ankle deformities and instabilities with consecutive ulcerations may be the consequence, which again may increase the risk for later extremity loss due to the need for amputation or at least fun…

Lipoblastoma--a rare paediatric foot tumour.

Lipoblastoma, a rare benign tumour arising from embryonic fat, is usually found in areas of abundant adipose tissue. Various reports describe a predilection of lipoblastoma for sites with primitive adipose tissue such as axilla, neck, retroperitoneum and prevertebral soft tissue. The plantar aspect of the foot is an extremely rare site due to scarcity of fatty tissue. Differential diagnosis includes lipomas, fibromyxolipomas and liposarcomas. Age of presentation, chromosomal markers and histopathological examination help in arriving at final diagnosis. Radical surgeries are not advocated; howe…

[Adult-onset primary focal foot dystonia].

INTRODUCTION: Adult-onset primary focal foot dystonia is a rare event. Up to now, only 7 cases have been reported in the literature. Since the idiopathic-type food dystonia is uncommon in adults, secondary types must be ruled out. CLINICAL REPORT: We present the case of a 51 year woman with a 3 year history of focal food dystonia. She had abnormal posture of the foot with plantar flexion and inversion, which worsened with action. Complementary studies aimed at ruling out secondary causes were all negative. The dystonia remained limited to her food and she did not develop any other neurologica…

Tetanus prophylaxis for diabetic foot ulcers.

The infected or ulcerated diabetic foot is a suitable environment for Clostridium tetani. Tetanus intoxication as a result of foot ulcer has been described in the literature. Immunopathy, vasculopathy, and ulceration place the diabetic patient at risk for developing tetanus. Of diabetic patients who contract generalized tetanus in the United States, foot ulcer or gangrene are responsible for 25% of cases. Patients who have diabetic wounds should receive tetanus prophylaxis. The prophylaxis should follow the "tetanus-prone" wound recommendations as set by the Centers for Disease Control and Pre…

Adult-onset primary focal foot dystonia.

Although primary focal hand dystonia has been well characterized, primary focal foot dystonia in adults has rarely been reported. Our objective was to describe the clinical phenotype and treatment outcomes in patients with primary, adult-onset focal foot dystonia. To this end we conducted a retrospective study of four consecutive patients (59.5+/-13.5 years, range 44-67) diagnosed over a period of 6 years and followed-up for at least 5 years. Focal foot dystonia resulted in variable physical impairment. Anti-dystonic agents were mildly effective whereas botulinum toxin injections provided subs…

[Complex microsurgical reconstruction of diabetic foot].

Microsurgical techniques increased the operability of patients with critical limb ischemia to more than 95%. However, the percentage of ischemic limb salvage remains limited, especially in diabetic patients with associated tissue gangrene and neuropathy. The study presents a 67 years old female patient with type II diabetes mellitus and critical limb ischemia, complicated with progressive gangrene of the pre-calcaneal region and thus representing a classical indication for thigh amputation. A sequential femur-popliteal-tibial anterior by-pass was performed, followed by soft tissue reconstructi

High-risk foot and ankle patients.

The surgical treatment of complicated foot and ankle problems requires recognition and appreciation of patient and disease characteristics that may compromise surgical results. Appropriate patient selection is an important factor in obtaining a successful outcome. Modulation and management of these patient characteristics are critical in minimizing complications that are associated with treatment.

[Ultrasound diagnosis of congenital foot deformities].

Assessment of tarsometatarsal skeletal malalignment in congenital foot deformities such as clubfoot and vertical talus is possible with ultrasound. Sonography can be useful in deciding on the therapeutic concept and evaluating therapeutic effects.

[Quality of life after foot injuries].

OBJECTIVE: Assessment of quality of life after foot injuries. METHODS: All patients with foot injuries treated operatively from July 1995 to December 1999 were asked to complete the SF-36 health survey. Values of four subgroups (male with foot injuries without any other injury, multiple injured male with foot injuries, male without any other injury and both calcaneus fractures or other injuries) were compared with the standardized German population (Mann-Whitney-U-Test, alpha = 0.05). RESULTS: 110 of 201 patients completed the questionnaire (follow-up 54.7 %). Values of most of the subscales…
