Tracheoesophageal voice restoration after laryngectomy is possible with a variety of neopharyngeal reconstructions. We have used the tubed radial forearm free flap for neopharyngeal reconstruction since 1991. Six patients have undergone voice restoration with the Blom-Singer prosthesis and were available for quantitative and qualitative speech analysis. These patients were compared to five laryngectomy patients with standard pharyngeal closures and similar voice restorations. Special attention is paid to the problems of the donor site (the forearm) in a series of 75 patients who underwent orofacial malignancy excision. An island skin flap from the ulnar aspect of the forearm, used in 15 patients, is reported to be an improvement of the surgical technique. No healing complications were observed, and the aesthetic and functional results were very good.

The authors discuss their experience with the ulnar artery forearm island flap in 54 cases of hand surgery. They discuss its advantages over the radial island forearm flap and examine further technical possibilities in hand reconstruction, such as the compound flap. Although these reconstructive techniques do not have any sequelae, they involve the sacrifice of a major artery and should thus be used only in complex cases with very clear indications and as a last resort.

A case with a severe gun shot wound of the palmar ulnar distal forearm and wrist is presented. The initial treatment included reconstruction of the ulnar nerve by means of nerve grafts and soft tissue coverage using a free microvascular groin flap. This represents a procedure which is certainly applied only very exceptionally at our department. Inspite of consequent complete flap necrosis due to arterial thrombosis, sensory recovery was sufficient for protective sensibility. The probable reasons of arterial thrombosis are discussed.

The uptake of acetate in the human forearm was studied in five fasting (14 h) subjects during 10-min periods of ergometer work at 7 and 10 kilopond-meters per minute (kpm/min). A constant arterial acetate concentration was established by administration of a small quantity of alcohol (25 g) to the subjects after a control work period. An all-out test has been created to measure critical force (CF) of the forearm flexor muscles, but its suitability for rock climbing is unclear.
