Fetal Macrocephaly: Prenatal Findings and Follow-Up in Cases With High Risk for Abnormal Outcome.

OBJECTIVE: Fetal macrocephaly (HC Z-score ≥ +2) is a common reason for referral for neurosonography. While most cases are benign and asymptomatic, syndromic macrocephaly poses a significant risk of abnormal neurodevelopment. This study aimed to describe the sonographic and genetic features of fetuses at the highest risk of syndromic macrocephaly. METHODS: We retrospectively analyzed 19 fetuses with either HC Z-scores ≥ +2.5 or those with ≥ +2 and associated anomalies, evaluated between January 2015 and October 2024. Assessments included fetal neurosonography, anomaly scans, MRI, and genetic w

Genetic variant annotation scores in congenital long QT syndrome.

BACKGROUND: Congenital Long QT Syndrome (LQTS) is a hereditary arrhythmic disorder. We aimed to assess the performance of current genetic variant annotation scores among LQTS patients and their predictive impact. METHODS: We evaluated 2025 patients with unique mutations for LQT1-LQT3. A patient-specific score was calculated for each of four established genetic variant annotation algorithms: CADD, SIFT, REVEL, and PolyPhen-2. The scores were tested for the identification of LQTS and their predictive performance for cardiac events (CE) and life-threatening events (LTE) and then compared with th

Genetic risk for Alzheimer's dementia predicts motor deficits through multi-omic systems in older adults.

Alzheimer's disease manifests with both cognitive and motor deficits. However, the degree to which genetic risk of Alzheimer's dementia contributes to late-life motor impairment, and the specific molecular systems underlying these associations, are uncertain. Here, we adopted an integrative multi-omic approach to assess genetic influence on motor impairment in older adults and identified key molecular pathways that may mediate this risk. We built a polygenic risk score for clinical diagnosis of Alzheimer's dementia (AD-PRS) and examined its relationship to several motor phenotypes in 1885 olde

Genome-Wide Association Analyses Identify Distinct Genetic Architectures for Extreme Early-Onset and Late-Onset T2D.

AIMS: Type 2 diabetes (T2D) is a heterogeneous disorder with substantial variation in age at onset (AAO). This study aimed to characterize the distinct genetic architectures and biological mechanisms underlying extreme AAO-defined T2D subtypes. MATERIALS AND METHODS: Using 74 795 European-ancestry participants from the UK Biobank, we performed genome-wide association studies (GWAS) of relatively early-onset T2D (eoT2D; AAO < 55 years) and late-onset T2D (loT2D; AAO ≥ 70 years). We investigated subtype-specific genetic loci, SNP-based heritability, genetic correlations, Mendelian randomization

No prospective association of a polygenic risk score for coronary artery disease with venous thromboembolism incidence.

BACKGROUND: Previous studies are inconsistent about whether chronic coronary artery disease or generalized atherosclerosis is a causal risk factor for venous thromboembolism. No study seems to have taken a genomic approach to this question. OBJECTIVE: To test in a prospective study whether a polygenic risk score for coronary artery disease is associated with risk of venous thromboembolism. PARTICIPANTS/METHODS: Within the Atherosclerosis Risk in Communities Study cohort, we computed a previously validated polygenic risk score for coronary artery disease among 9144 White participants at basel

Associations between blue space exposure and rheumatoid arthritis: The modifying effect of genetic susceptibility and air pollutants.

Studies on the interaction among genetic susceptibility, blue space exposure, and rheumatoid arthritis (RA) risk have been lacking. Therefore, we examined the association between blue space exposure and RA incidence and assess the modifying effect of genetic susceptibility and air pollutants. Form the UK Biobank, 322,783 participants without RA were enrolled in this study. The association between blue space exposure and RA incidence was estimated using a cox proportional hazards model. The combined effect of blue space and genetic factors on the risk of RA was further evaluated. The polygenic

Genetic risk, metabolic syndrome, and gastrointestinal cancer risk: A prospective cohort study.

BACKGROUND: Gastrointestinal (GI) cancer risk has been associated with metabolic syndrome (MetS), a surrogate indicator for unhealthy lifestyles, and a number of genetic loci, but the combined effect of MetS and genetic variants on GI cancer risk is uncertain. METHODS: We included 430,036 participants with available MetS and genotype data from UK Biobank. During the follow-up time, 5494 incident GI cancer cases, including esophageal cancer, gastric cancer, and colorectal cancer, were identified. We created a GI polygenic risk score (GI-PRS) for overall GI cancer derived from three site-specif

Dissecting shared genetic architecture between obesity and multiple sclerosis.

BACKGROUND: Observational studies have associated obesity with an increased risk of multiple sclerosis (MS). However, the role of genetic factors in their comorbidity remains largely unknown. Our study aimed to investigate the shared genetic architecture underlying obesity and MS. METHODS: By leveraging data from genome-wide association studies, we investigated the genetic correlation of body mass index (BMI) and MS by linkage disequilibrium score regression and genetic covariance analyser. The casualty was identified by bidirectional Mendelian randomisation. Linkage disequilibrium score regr

A polygenetic risk score combined with environmental factors better predict susceptibility to hepatocellular carcinoma in Chinese population.

AIMS: This study aimed to investigate environmental factors and genetic variant loci associated with hepatocellular carcinoma (HCC) in Chinese population and construct a weighted genetic risk score (wGRS) and polygenic risk score (PRS). METHODS: A case-control study was applied to confirm the single nucleotide polymorphisms (SNPs) and environmental variables linked to HCC in the Chinese population, which had been screened by meta-analyses. wGRS and PRS were built in training sets and validation sets. Area under the curve (AUC), net reclassification improvement (NRI), integrated discrimination…

Clinical use of polygenic risk scores for detection of peripheral artery disease and cardiovascular events.

We have previously shown that polygenic risk scores (PRS) can improve risk stratification of peripheral artery disease (PAD) in a large, retrospective cohort. Here, we evaluate the potential of PRS in improving the detection of PAD and prediction of major adverse cardiovascular and cerebrovascular events (MACCE) and adverse events (AE) in an institutional patient cohort. We created a cohort of 278 patients (52 cases and 226 controls) and fit a PAD-specific PRS based on the weighted sum of risk alleles. We built traditional clinical risk models and machine learning (ML) models using clinical an

Polygenic risk score prediction of antipsychotic dosage in schizophrenia.

OBJECTIVE: Genetic variants have yet to be identified as reliable predictors of antipsychotic dosage. The purpose of this study is to quantify significant genetic risk variants prioritized from the Psychiatric GWAS Consortium (PGC2) study for schizophrenia as a polygenic score to test our hypothesis that it may represent symptom severity in patients and therefore predict antipsychotic dosage. METHODS: Antipsychotic medication and dosage were collected in our sample of 83 patients with schizophrenia spectrum disorders of a homogeneous European background. Antipsychotic dosage was standardized

Branched-chain amino acids and risk of major depressive disorder: a Mendelian randomization and colocalization study.

AIMS: Depression is associated with numerous metabolic pathway abnormalities, and several studies have suggested a link between depression and branched chain amino acids (BCAAs) metabolic disorders. However, the precise causality and direction remain inconclusive. Consequently, this study aims to ascertain the relationship between the risk of depression and BCAAs levels using two-sample Mendelian randomization (MR) analysis. MATERIALS AND METHODS: Single nucleotide polymorphisms associated with the BCAAs were extracted from the IEU OpenGWAS. Pooled level data for major depressive disorder (MD

Boosting Schizophrenia Genetics by Utilizing Genetic Overlap With Brain Morphology.

BACKGROUND: Schizophrenia is a complex polygenic disorder with subtle, distributed abnormalities in brain morphology. There are indications of shared genetic architecture between schizophrenia and brain measures despite low genetic correlations. Through the use of analytical methods that allow for mixed directions of effects, this overlap may be leveraged to improve our understanding of underlying mechanisms of schizophrenia and enrich polygenic risk prediction outcome. METHODS: We ran a multivariate genome-wide analysis of 175 brain morphology measures using data from 33,735 participants of

Causal relationship between uterine fibroids and cardiovascular disease: A two-sample Mendelian randomization study.

Previous studies have indicated that patients with uterine fibroids (UF) may have an elevated risk of cardiovascular disease (CVD), although the causal relationship between UF and CVD remains unclear. In this Mendelian randomization (MR) study, we aimed to investigate the causal association between genetic susceptibility to UF and the risk of developing CVD. We extracted summary statistics for single nucleotide polymorphisms associated with UF and 5 CVDs from multiple databases for further analysis. First, we used linkage disequilibrium score regression to assess the genetic correlation across

Machine Learning-Driven Prediction of Coronary Artery Disease Risk Based on UK Biobank Plasma Proteomics.

BACKGROUND: Coronary artery disease (CAD) is a leading global cause of mortality, yet the predictive accuracy of conventional risk models is limited. Here, we integrate conventional risk factors, polygenic risk scores, and large-scale proteomics to develop a unified model for enhanced CAD risk prediction. METHODS: Using data from UK Biobank, participants with plasma proteomics and genetic risk data were included after excluding prevalent CAD. Participants from England were split into training (n=32 330) and internal validation (n=13 857) sets, and Scotland/Wales participants formed an externa

Polygenic risk score analysis of noise-induced hearing loss: An integrated cross-sectional and longitudinal study.

BACKGROUND: Substantial variability in susceptibility to noise-induced hearing loss (NIHL) with comparable noise exposures suggests a genetic contribution, but population-based evidence remains limited. This study evaluated whether a polygenic risk score (PRS), summarizing the effect of genetic variants, was associated with NIHL susceptibility. METHODS: Participants were recruited from an ongoing Occupational Noise Exposure and Health Study (ONEHS), including 3836 NIHL cases and 2028 normal hearing (NH) controls. A weighted PRS, calculated from seven genetic variants, stratified participants

Network analysis of the genetic relationships between twenty psychiatric and substance use disorders.

We sought to clarify, using network analysis, the structure of genetic risk factors for a wide range of psychiatric and substance use disorders. Using split halves (N=1,510,974), we computed the family genetic risk scores (FGRSs) for 20 psychiatric and substance use disorders assessed in treated individuals from the entire Swedish population born between 1960 and 2000. We calculated a genetic correlation matrix for them to which we applied an EBICglasso network analysis. The network was dense, with five inter-connected clusters of genetic risks for internalizing, externalizing, neurodevelopmen…

Polygenic Risk for Schizophrenia Influences Cortical Gyrification in 2 Independent General Populations.

Schizophrenia is highly heritable, whereas the effect of each genetic variant is very weak. Since clinical heterogeneity and complexity of schizophrenia is high, considerable effort has been made to relate genetic variants to underlying neurobiological aspects of schizophrenia (endophenotypes). Given the polygenic nature of schizophrenia, our goal was to form a measure of additive genetic risk and explore its relationship to cortical morphology. Utilizing the data from a recent genome-wide association study that included nearly 37 000 cases of schizophrenia, we computed a polygenic risk score

Polygenic risk scores reveal shared polygenic risk across five common cardiovascular diseases.

BACKGROUND AND AIM: The five cardiovascular diseases (CVDs): Coronary artery disease (CAD), stroke, heart failure (HF), peripheral arterial disease (PAD), and atrial fibrillation(Afib) often co-occur. Most polygenic risk scores (PRSs) are developed for individual CVDs, limiting their clinical utility across multiple CVDs. We investigated associations between five PRSs and (1) their CVDs, (2) cross-disease associations for pleiotropy, and (3) across multimorbidity clusters. METHODS: We calculated restricted PRSs using genome-wide significant variants from published genome-wide association stud
