Dispatcher-Assisted CPR in Italy: A Nationwide Survey of Current Practices and Future Challenges in Emergency Medical Communication Centers.

Background/Objectives: Dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) is widely recognized as a critical intervention that significantly reduces no-flow time, improving survival rates in out-of-hospital cardiac arrests (OHCAs). This study evaluates current practices and the organization of DA-CPR in Italian emergency medical communication centers (EMCCs) and identifies areas for improvement. Methods: A cross-sectional survey was conducted between April and May 2024 among all Italian EMCCs, achieving a 92.6% response rate (62 out of 67) and covering 95.5% of the population. Data wer

Enhancing Palliative Care for Individuals With Intellectual and Developmental Disabilities: A Scoping Review of Communication Strategies for Assessing Health Needs.

BACKGROUND: Individuals with intellectual and developmental disabilities (IDD) face significant health disparities, particularly in accessing appropriate palliative care. Communication challenges remain a major barrier to understanding and addressing their complex needs. Despite palliative care's emphasis on person-centered approaches, research on how to communicate with people with IDD to support effective and equitable care delivery and decision-making is limited. AIM: This scoping review aimed to identify and synthesize communication strategies used across diverse disciplines that can supp

Staff perspectives on the impacts of the COVID-19 pandemic on the provision of emergency department care for patients who use opioids.

PURPOSE: The COVID-19 pandemic and Canada's drug poisoning crisis placed exceptional demands on emergency departments (ED). We aimed to explore the impact of these intersecting crises from the perspectives of ED staff to understand how EDs can improve care and protect the health and well-being of patients who use opioids, ED staff, and healthcare providers. METHODS: We conducted a focused ethnographic study involving 29 semi-structured interviews with ED staff who cared for patients who use opioids during the pandemic. Interviews explored ED staff perspectives on how the pandemic impacted car

Masking for COVID-19 Is Associated with Decreased Emergency Department Utilization for Non-COVID Viral Illnesses and Respiratory Conditions in Maryland.

BACKGROUND: Masking, which is known to decrease the transmission of respiratory viruses, was not widely practiced in the United States until the coronavirus disease 2019 (COVID-19) pandemic. This provides a natural experiment to determine whether the percentage of community masking was associated with decreases in emergency department (ED) visits due to non-COVID viral illnesses (NCVIs) and related respiratory conditions. METHODS: In this observational study of ED encounters in a 11-hospital system in Maryland during 2019-2020, year-on-year ratios for all complaints were calculated to account

Diagnostic accuracy of verbal autopsies in ascertaining the causes of stillbirths and neonatal deaths in rural Ghana.

This study evaluated the diagnostic accuracy of a verbal autopsy (VA) tool in ascertaining the causes of stillbirths and neonatal deaths in rural Ghana and was nested within a community-based maternal vitamin A supplementation trial (ObaapaVitA trial). All stillbirths and neonatal deaths between 1 January 2003 and 30 June 2004 were prospectively included. Community VAs were carried out within 6 months of death and were classified with a primary cause of death by three experienced paediatricans. The reference standard diagnosis was obtained by the study paediatrician in 4 district hospitals in

Measurement of Long-Term, Quality of Life Outcomes in Injury Databases.

INTRODUCTION: Traumatic injuries affect patients' long-term quality of life (QOL). In 2016, the National Academies highlighted the need to record long-term QOL outcomes within trauma databases. We studied national injury-specific databases to assess whether they capture QOL outcomes, and their key determinants, across the entire continuum of trauma care. METHODS: We coded variables in the Trauma Quality Program (TQP), Burn Care Quality Platform, and the Burn, Spinal Cord Injury (SCI), Traumatic Brain Injury (TBI) Model Systems databases to identify QOL and social determinants of health (SDOH)

Understanding a Minority Group's (Roma) Experiences of Access and Quality in Maternity Services.

BACKGROUND: Maternal health is a fundamental human right, ensuring that all women have access to non-discriminatory, equitable and high-quality maternal and reproductive healthcare. Minority groups and migrant populations face significant challenges in accessing maternity services. The Roma are Europe's largest and most marginalised ethnic group. Ireland has a sizable population of Roma migrants, and there is limited information on their experiences of maternal care in Ireland. Their participation in research and user panels is challenging as many lack proficiency in English and tend to have l

Closing the Gap: Addressing Racial Bias in Bystander CPR Administration.

Bystander cardiopulmonary resuscitation (BCPR) significantly improves survival rates for out-of-hospital cardiac arrest (OHCA), yet racial disparities persist. Black and Hispanic individuals are markedly less likely to receive BCPR than White individuals, contributing to preventable deaths and inequities in survival outcomes. Structural factors such as educational barriers, implicit bias, and systemic racism contribute to these disparities. This article examines key contributors to racial disparities in BCPR administration and presents targeted, evidence-based solutions. Educational barriers,

Incarceration and Psychiatric Emergency Department Visits Among Black Americans.

UNLABELLED: Policy Points The spillover effects of jail and prison incarceration extend to acute psychiatric emergencies in Black communities. State-level drug reform policies and policies that reduce labor market and housing discrimination for former inmates may have salutary mental health benefits for the broader community. CONTEXT: The justice system incarcerates nearly 2.3 million individuals in the United States. Black Americans comprise 40% of those incarcerated despite representing less than 15% of the population. Theoretical work posits that mass incarceration can erode social capital

Effect of drinking water source on associations between gastrointestinal illness and heavy rainfall in New Jersey.

Gastrointestinal illness (GI) has been associated with heavy rainfall. Storm events and periods of heavy rainfall and runoff can result in increased microbiological contaminants in raw water. Surface water supplies are open to the environment and runoff can directly influence the presence of contaminants. A time-stratified bi-directional case-crossover study design was used to estimate associations of heavy rainfall and hospitalizations for GI. Cases of GI were identified as in-patient hospitalization with a primary diagnosis of infectious disease associated diarrhea [ICD-9 codes: specified ga

Biofilm removal in hospital sink drains drives unintended surges in antibiotic resistance.

The prevalence and proliferation of antimicrobial-resistant bacteria is considered one of the critical issues of our time. Wastewater is a habitat for complex microbial communities where bacteria share antimicrobial-resistance genes through horizontal gene transfer. Hospital wastewater plumbing systems are an ideal reservoir for environmental and pathogenic bacteria to interface and exchange antimicrobial-resistance genes. Replacement of contaminated plumbing may be the most intuitive and widely deployed response to the detection and colonization of highly-resistant potentially pathogenic bact
