Design, development, and evaluation of a novel shoulder phantom testbed for analyzing interaction forces and reachability of wearable exoskeletons.

The human need for rehabilitation, assistance, and augmentation has led to the development and use of wearable exoskeletons. Upper limb exos … signing and developing a novel human shoulder phantom mimicking the shoulder complex motion and the humeral head translation that can help in the real-time testing of exoskeletons without the need for human volunteers. The device can be used to test the interaction forces and the maximum reachable position of the exoskeleton. It consists of three degrees of fr …

Evaluating the clinical effects of a dynamic shoulder orthosis.

BACKGROUND: Shoulder orthoses reduce the gravitational pull on the shoulder by providing an upward force to the arm, which can decrease shou … were the mean shoulder pain scores before and during the intervention, and the distance between the humeral head and the acromion without and with orthosis. RESULTS: Ultrasound evaluation showed that the shoulder orthosis resulted in a reduction of the distance between the acromion and humeral head at different levels of arm support. In addition, it was demon …

Shoulder Bankart lesion with posterior instability: A case report of ultrasound detection.

Minor instability of the shoulder has recently drawn attention as a cause of shoulder pain in athletes. However, it is difficult to correctly diagnose the direction in which the humeral head translates and subluxates, and to clarify the pathology of the instability. We present a case of a 20-year-old male with an unstable shoulder who could not raise his left arm due to pain. Since 6 years prior to the onset of pain, the patient could asymptomatically perform voluntary subluxation, but it was slight and the direction of the subluxation could not be confirmed. On physical examination, the conve…

Decreased range of motion following arthroscopic remplissage.

The Hill-Sachs lesion is an impression fracture of the posterolateral margin of the humeral head that commonly results from impaction with the anterior glenoid rim during subluxation or dislocation of the shoulder. This bony defect has been implicated as an etiology of recurrent instability of the shoulder. The "remplissage" technique described by Wolf is an arthroscopic method of filling the Hill-Sachs defect via infraspinatus tenodesis and posterior capsulodesis. The purpose of this technique is to prevent recurrent instability by making the Hill-Sachs lesion extra-articular thereby eliminat…

Role of us in the preoperative evaluation of patients with anterior shoulder instability.

PURPOSE: To assess the value of ultrasonography (US) in the preoperative evaluation of patients with anterior shoulder instability. MATERIALS AND METHODS: Twenty-two patients with one-sided anterior shoulder instability were examined with US by using three dynamic scanning approaches: two frontal and one axillary. The anterior labrum, the anterior ligamental-capsular complex, and the presence of humeral head and glenoid rim fractures were evaluated. Arthroscopy or arthrotomy was subsequently performed in all patients and was considered the standard. RESULTS: US correctly depicted the presenc…

[Shoulder instability and pain. Computerized arthro-tomography assessment].

The growing interest in shoulder joint imaging comes from the ever-increasing demand for such an examination in orthopedics. Since more comp … ctures were nearly always associated. Moreover, high incidence (65.8%) of Hill-Sachs lesions of the humeral head was observed. In atraumatic instability, abnormal anteversion of the scapular glenoid was always detected. In the patients with shoulder pain, the most common causes were the impingement syndrome (30.5%), superior labrum lesions (16.7%), adhesive ca …

[Reconstruction of a traumatic lateral trochlear defect with a radial head autograft].

OBJECTIVE: To report the result of a severe articular defect secondary to a complex open fracture of the distal humerus, using the ipsilateral radial head as a bone autograft for the trochclear region as an alternative to total arthroplasty. CASE REPORT: We describe a patient who suffered an open fracture of the elbow with bone loss in the distal humeral region and, after the initial stabilisation surgery, needed a reconstruction with an ipsilateral radial head autograft. DISCUSSION: Open fractures of elbow are rare. When there is a severe trochlear defect, surgical options are total elbow a

Distal pectoralis major tears: sonographic characterization and potential diagnostic pitfalls.

OBJECTIVES: Distinction between musculotendinous tears of the pectoralis major and distal tendon avulsions is important, as the latter typically requires surgical repair. The objective of this study was to characterize the sonographic appearances of surgically proven distal tendon avulsion tears of the pectoralis major. METHODS: A retrospective search of the radiology database (2001-2011) revealed 22 cases of pectoralis major tears on sonography, of which 9 had surgical correlation. Sonograms were retrospectively characterized by 3 fellowship-trained musculoskeletal radiologists in consensus

Total shoulder arthroplasty utilizing mini-stem humeral components: technique and short-term results.

INTRODUCTION: Throughout the field of orthopedic surgery, there has been a trend toward using smaller incisions and implants that preserve as much normal anatomy as possible. The use of bone sparing technology, such as partial and full surface replacements of the humeral head, while attractive in younger patients, does not allow the best exposure for proper glenoid replacement. Additionally, there are other situations when the use of surface replacements is contraindicated. There are also patients with an existing total elbow replacement or a humeral malunion or deformity in which a traditiona…

Corrective Open-wedge Valgus Proximal Humerus Lengthening Osteotomy. Technique and Case Series.

Symptomatic varus malunion after proximal humeral fractures is associated with weakness and painful limitation of shoulder range of motion. When there is conformity of the articular surface and no avascular necrosis, a head-preserving procedure is best indicated. Arthroscopic arthrolysis, subacromial decompression, and tuberoplasty have been described for the treatment of mild deformity. In cases with more severe deformity, corrective extracapsular lateral closing wedge valgus osteotomy has been reported as a reliable treatment option, in terms of both pain relief and improved function. While

Proximal humeral fractures with a severe varus deformity treated by fixation with a locking plate.

We treated 47 patients with a mean age of 57 years (22 to 88) who had a proximal humeral fracture in which there was a severe varus deformit … ion were attributable to violation of the operative protocol. In the 46 patients who retained their humeral head, all the fractures healed within the first year, with no sign of collapse or narrowing of the joint space. Longer follow-up will be required to confirm whether these initially satisfactory results are maintained.

Alternative humeral tubercle fixation in shoulder hemiarthroplasty for fractures of the proximal humerus.

HYPOTHESIS: We hypothesize that the sling technique provides better long term tuberosity fixation in hemiarthroplasty for severe fractures of the proximal humerus. BACKGROUND: After hemiarthroplasty for proximal humeral fractures, problems exist with deterioration of shoulder function by secondary displacement of the initially fixated humeral tuberosities. This study evaluated a new technique for fixation of the greater and lesser tuberosities in uncemented hemiarthroplasties in severely displaced 3- or 4-part proximal humeral fractures. MATERIALS AND METHODS: A consecutive series of 31 unce

Septic shoulder presenting as a shoulder dislocation in the pediatric patient: a case report.

UNLABELLED: Shoulder dislocation is relatively uncommon in the younger pediatric population. Because of the relative strength of the surrounding soft tissue structures of the shoulder compared with the proximal humeral physis, subluxation or dislocation resulting from a traumatic event or application of force is rare and instead a proximal humeral physeal injury occurs. Case presentation - We present a 5-year-old male who presented to the office with post-traumatic left shoulder pain for about 1 week. Radiographs of the left shoulder indicated inferior subluxation of the humeral head. He was u…

Treatment of complex proximal humeral fracture: plate and tension band fixation versus conservative therapy.

BACKGROUND: There is no consensus on the optimal treatment for patients with complex proximal humeral fractures. A meta-analysis was perform … ing plate and tension band did not find better shoulder function, higher rate avascular necrosis of humeral head, lower rate nonunion, and higher rate osteoarthritis. In further, high quality and large randomized trials should be recommended to make a choice between these treatment modalities.

Arthroscopically treated proximal humeral fracture malunion.

Articles describing the treatment of proximal humerus malunion are limited. Although in most of the cases, shoulder arthroplasty is the treatment of choice, when the articular surface of the humeral head is intact, other techniques can be considered and successfully used as well. Using arthroscopic techniques for proximal humerus malunion treatment is rarely reported in the literature. We could find only a few cases in which arthroscopic subacromial decompression was used to treat greater tuberosity malunion. Arthroscopic debridement and capsulotomy are also considered in the treatment of prox…
