Risk Factors for Surgical Revision Following Ventricular Shunt Placement for Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND AND OBJECTIVES: Ventriculoperitoneal shunt (VPS) placement is a common treatment for idiopathic normal pressure hydrocephalus (iNPH), but patients may require VPS revision. This study aims to evaluate the effectiveness of a standardized VPS placement protocol by comparing indications and rates of shunt revision compared with a historical institutional cohort. METHODS: All patients treated with VPS for "suspected" iNPH at a tertiary care center between January 2000 and January 2023 were retrospectively reviewed and divided into temporally distinct historical (before 2017) and standa

Minimizing complications in lumboperitoneal shunt for normal pressure hydrocephalus: Technical note and case series.

BACKGROUND: Normal pressure hydrocephalus (NPH) is a complex syndrome that requires diversion of cerebrospinal fluid in an absorbing compartment. Nowadays, lumboperitoneal shunt is widely validated as treatment option. We present a novel surgical technique to minimize post-surgical complications. METHODS: All patients treated for a NPH between 2019 and 2024 at our department were analyzed. Pre- and postoperative clinical and surgical data were assessed. A novel technique with curvilinear lumbar skin incision and tight valve fixing to the lumbar muscular fascia was described. RESULTS: One hun

Brain ventricle morphology markers in predicting shunt surgery outcome in idiopathic normal-pressure hydrocephalus.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is characterized by a clinical triad of symptoms: abnormal gait, memory problems, and urinary incontinence. Neuroimaging plays a crucial role in diagnosing iNPH. However, current radiological markers, though indicative, are not definitive, suggesting the limited capacity of these indices to capture mechanisms associated with iNPH and the reversibility of the symptoms. AIMS: This study aims to (1) determine the geometric features of the lateral ventricles, (2) develop a quantitative method for three-dimensional analysis, and (3) test t

Case report: Hyponatremia due to syndrome of inappropriate antidiuresis associated with normal pressure hydrocephalus.

Hyponatremia is common in hospitalized patients. Syndrome of inappropriate antidiuresis (SIAD) is a known cause of hyponatremia, with many different causes. Normal pressure hydrocephalus-related SIAD with resultant hyponatremia is a less known association. This case report is of a 77-year-old male with known normal pressure hydrocephalus (NPH) presenting with acute mental status changes and severe hyponatremia and found to have SIAD due to his NPH. A lumbar puncture quickly improved both his confusion and hyponatremia. The association between SAID and NPH is important as the management may req

MR Fingerprinting Predicts Gait Improvement After Lumbar Cerebrospinal Fluid Drainage for Normal Pressure Hydrocephalus.

BACKGROUND: Normal pressure hydrocephalus (NPH) results in gait disturbance, but clinical response to extended lumbar drainage (ELD) of cerebrospinal fluid varies. Noninvasive predictors of response are needed. PURPOSE: To determine whether MR fingerprinting (MRF) findings have potential as a quantifiable biomarker for predicting clinical response to ELD in patients with NPH. STUDY TYPE: Prospective cohort. POPULATION: Twenty individuals with NPH (mean age 77.7 years; 11 male sex). FIELD STRENGTH/SEQUENCE: 3T; 2D T1,T1/T2 MRF, 3D T1. ASSESSMENT: Patients with NPH referred for ELD between

Long-term outcomes after shunt surgery in older patients with idiopathic normal pressure hydrocephalus.

OBJECTIVE: This study aimed to evaluate the survival and functional outcomes in older patients with idiopathic normal pressure hydrocephalus (iNPH) following shunt surgery. METHODS: We performed shunt surgery in patients with iNPH and followed up at an outpatient clinic. The survival time was analyzed using Kaplan-Meier survival analysis. The maintenance period of activities of daily living (ADL) was measured using the modified Rankin Scale in part of patients. RESULTS: Out of 200 patients with iNPH (mean age, 78.1 years old, standard deviation, 5.0), 110 had a lumboperitoneal shunt, 68 had

Symptom Burden and Intervention Outcomes in Idiopathic Normal Pressure Hydrocephalus: A Multicenter Retrospective Study.

INTRODUCTION: Normal pressure hydrocephalus (NPH) is a potentially reversible cause of dementia, characterized by a clinical triad of gait disturbance, urinary incontinence, and cognitive decline. Despite suggestive radiologic findings, many patients remain untreated. This study aimed to determine the clinical correlation between radiographic findings of NPH and clinical symptoms. It also investigated whether the number of NPH symptoms predicts the likelihood of intervention and clinical improvement (in any of the triad of symptoms), and assessed gender-based disparities in treatment. METHODS

Ventriculoperitoneal shunt outcomes in neurodegenerative normal pressure hydrocephalus: A case series of 29 patients.

BACKGROUND: Management of normal pressure hydrocephalus (NPH) can be complicated by the progression of underlying neurological conditions such as Alzheimer's disease or Parkinson's disease. The purpose of this study is to explore the long-term shunting response on gait dysfunction in patients with neurodegenerative NPH, a subtype of NPH occurring in patients with another neurodegenerative condition, as the duration of symptomatic improvement with shunting is unclear in this population. METHODS: A retrospective chart review of patients undergoing evaluation for NPH at the Adult Hydrocephalus C

A novel prognostic role of proton MR spectroscopy in shunted idiopathic normal pressure hydrocephalus.

OBJECTIVE: To assess the prognostic potential of preoperative proton MRS in predicting postoperative outcomes for patients with INPH undergoing VPS. METHODS: Preoperative MRS data were obtained and analyzed from 15 patients with INPH who underwent VPS. The concentrations of key metabolites-Cho, Cr, NAA, and Lac-were quantified in the Tha, BG, PV, and MF regions. Metabolite ratios were calculated, and clinical response to shunting was assessed using the Hellström INPH scale. Prognostic accuracy was determined via ROC analysis. RESULTS: • Cho: Elevated levels in the left MF region were associa

Molecular signatures of normal pressure hydrocephalus: a large-scale proteomic analysis of cerebrospinal fluid.

Given the persistent challenge of differentiating idiopathic Normal Pressure Hydrocephalus (iNPH) from similar clinical entities, we conducted an in-depth proteomic study of cerebrospinal fluid (CSF) in 28 shunt-responsive iNPH patients, 38 Mild Cognitive Impairment (MCI) due to Alzheimer's disease, and 49 healthy controls. Utilizing the Olink Explore 3072 panel, we identified distinct proteomic profiles in iNPH that highlight significant downregulation of synaptic markers and cell-cell adhesion proteins. Alongside vimentin and inflammatory markers upregulation, these results suggest ependymal

Association between idiopathic normal pressure hydrocephalus and Alzheimer's disease: a bidirectional Mendelian randomization study.

Observational studies have suggested a bidirectional relationship between idiopathic normal pressure hydrocephalus (iNPH) and Alzheimer's disease (AD). However, the causal association between these two neurodegenerative disorders remains unclear. This study aimed to explore the causal relationship between iNPH and AD using a two-sample bidirectional Mendelian randomization (MR) method. Large-scale genome-wide association studies of iNPH (Ncase = 767, Ncontrol = 375,610) and AD (Ncase/proxy = 111,326, Ncontrol = 677,663) in European individuals were used to screen genetic instruments for MR ana

Cerebral Microbleeds-Long-Term Outcome After Cerebrospinal Fluid Shunting in Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Cerebral microbleeds (CMBs) are common in idiopathic normal pressure hydrocephalus (INPH) and have been suggested as radiological markers of a brain prone to bleeding. The presence of CMBs might be relevant when selecting patients for shunt surgery. OBJECTIVE: To evaluate whether CMBs increases long-term risk of hemorrhagic complications and mortality or affects outcomes after cerebrospinal fluid shunt surgery in a cohort of patients with INPH. METHODS: One hundred and forty nine shunted patients with INPH (mean age, 73 years) were investigated with MRI (T2* or susceptibility-wei

Cerebral oxygen extraction fraction declines with ventricular enlargement in patients with normal pressure hydrocephalus.

OBJECTIVE: Normal pressure hydrocephalus (NPH) is a neurodegenerative disease that is potentially reversible by shunt surgery in approximately 60% of patients. Imaging may provide a means to investigate brain tissue viability and oxygen metabolism in NPH patients. METHODS: Oxygen extraction fraction (OEF) mapping was generated from 3D multi-echo gradient echo MRI (mGRE) data using QQ-CCTV algorithm and cerebral blood flow (CBF) using 3D arterial spin labeling (ASL) MRI data, thereby calculating the cerebral metabolic rate of oxygen (CMRO2 = CBF × OEF × [H]a) in 16 NPH patients. Regression ana

Chronic Alcohol Use Associated Encephalopathy With a Nearly Identical Presentation to Normal Pressure Hydrocephalus.

We present the case of a 52-year-old male who arrived at the Emergency Department after several ground-level falls in the past month. He complained of urinary incontinence, mild confusion, headaches, and appetite loss in the past month as well. Brain computed tomography (CT) and magnetic resonance imaging (MRI) were performed, which showed enlarged ventricles with moderately prominent cortical atrophy and no acute abnormalities. It was decided to conduct a cisternogram study with serial scans. The study showed a type IIIa cerebrospinal fluid (CSF) flow pattern at 24 hours. At the 48- and 72-ho

[Coincidence of normal pressure hydrocephalus and Alzheimer`s disease: therapeutic implications and open questions].

Normal pressure hydrocephalus (NPH) is prevalent in aging patient populations. Despite its clinical relevance, many patients with NPH may not receive adequate treatment. Because of the frequency of Alzheimer`s disease in these patients, there could be overlapping pathophysiological mechanisms that are as yet incompletely understood. Cerebral comorbidities seem to have negative effects on therapeutic response to ventriculoperitoneal shunting. In order to avoid unnecessary and unsuccessful surgery in highly vulnerable elderly patients, they have to be taken into consideration in the diagnostic p
