Volume regulatory influx of electrolytes from plasma to brain during acute hyperosmolality.

Brain volume is regulated during acute hyperosmolal states based, in part, on the tissue gain of Na, Cl, and K [H. F. Cserr, M. DePasquale, and C.S. Patlak, Am. J. Physiol. 253 (Renal Fluid Electrolyte Physiol. 22): F522-F529, 1987]. This study evaluates the contribution of influx from plasma to the volume regulatory gain of electrolyte. Blood-to-brain transfer constants, K1, were measured as a function of plasma osmolality, over the range 300-385 mosmol/kg, for sodium (22Na) and potassium (42K or 86Rb) and as a marker for nonselective changes in permeability for [14C]mannitol. Osmolality was

[Changes in water-electrolyte balance and central hemodynamics in hypertensive crises].

Sodium, potassium and water distribution, central hemodynamic parameters, and plasma renin activity and aldosterone levels were examined in 48 patients with stable essential hypertension, including 10 males and 11 females with hypertensive crises aggravating the course of the disease, and in 18 controls. The individuals showing signs of heart and renal failure were excluded from the analysis. In female patients, the crise pattern of the disease was associated with marked changes of sodium metabolism, namely, raised extracellular fluid and plasma sodium levels, the patients with the crises havi

Clinical Profile and Outcome of Young Infants With Hypernatremic Dehydration Presenting to the Emergency Department.

OBJECTIVES: The aim of this study was to evaluate the clinical profile and outcome of young infants presenting to the pediatric emergency department with hypernatremic dehydration. METHODS: A prospective observational study was conducted at a tertiary care teaching hospital over a period of 18 months. All outborn sick young infants aged 2 months or younger who presented to the emergency department with symptoms and signs of possible sepsis and/or dehydration were screened, and those with hypernatremia were enrolled in the study. Those infants born at less than 37 weeks of gestation and gross …

The muscular dystrophic chicken is hypernatremic.

1. The E3 ubiquitin protein ligase 1 (WWP1) gene, the mutation of which causes muscular dystrophy in chickens, is expressed not only in the pectoral muscle, but also in a number of tissues such as the kidney. Therefore, this study examined some parameters related to kidney function in muscular dystrophic (MD) chickens. 2. Plasma osmolality, Na+ and K+ concentrations, aldosterone levels, and the expression of aquaporin (AQP) 2, AQP3, and α subunits of the amiloride-sensitive epithelial sodium channel (αENaC) were analysed in the kidneys of 5-week-old MD chickens and White Leghorn (WL) chickens

Does monitoring newborn weight discourage breast feeding?

BACKGROUND: A policy of regular neonatal weight monitoring was introduced to a geographically defined population in 2000. This was combined with targeted breast feeding support for infants reaching specified intervention thresholds. AIMS: To look for evidence of compromise in breast feeding rates as a result of this policy change. METHODS: Breast feeding rates at 10 days and 6 weeks were compared for this intervention population and two local non-intervention groups for the years 1999 and 2001. The data were analysed using Poisson regression analysis and the Z-test. RESULTS: There was a 3.1

Neonatal hypernatremic dehydration associated with breast-feeding malnutrition: a retrospective survey.

BACKGROUND: Hypernatremic dehydration in neonates is a potentially devastating condition. Recent reports have identified breast-feeding malnutrition as a key factor in its pathophysiology. METHODS: Using a theoretical framework for breast-feeding kinetics, a retrospective chart review of all neonates less than 28 days of age who were seen at either British Columbia's Children's Hospital or the Vancouver Breastfeeding Centre between 1991-1994 was conducted to identify and classify possible causes of breast-feeding malnutrition among neonates who developed hypernatremic dehydration. RESULTS: T

Hypernatraemic dehydration in the Hail region of Saudi Arabia.

Infants presenting with hypernatraemic dehydration were studied prospectively in order to describe its incidence and the predisposing factors. Five hundred and twenty children with gastroenteritis were admitted to the Paediatric Unit of Hail General Hospital over a 1-year period from 1 June 1985 to 1 June 1986. Twenty-five children (4.8%) had hypernatraemia (Na+ greater than 150 mmol/l) and all 25 were under 1 year of age, 23 (92%) being under 6 months. Twenty (80%) came from families living in the villages and had a poor educational background. All the babies were bottle-fed. The majority of

Role of osmosis in biliary NaCl secretion and bile formation.

To challenge the osmotic hypothesis of biliary NaCl secretion and bile formation, experiments were performed in anaesthetized pigs. An increase in plasma osmolality of 7 +/- 1 mosm/kg H2O induced by intravenous sucrose infusion decreased NaCl secretion, NaHCO3 secretion, and bile flow by 36 +/- 3%, 34 +/- 2%, and 34 +/- 3%, respectively. There was no change in the biliary concentration of NaCl and NaHCO3. When bile acids were infused intravenously, the secretion of 1 mmol bile acids caused an osmotic flow of 12.0 ml bile containing 0.92 mmol NaCl and 0.30 mmol NaHCO3 in an isotonic solution. B

Common electrolyte disorders for the internist.

Electrolyte, acid-base, and mineral metabolism disorders are among the most common and clinically significant problems experienced in internal medicine patients. Their development reflects disturbances in regulated homeostatic mechanisms involving the kidneys, bones, endocrine systems, and cellular ion transport. Disorders such as hyponatremia, hypernatremia, hypokalemia, hyperkalemia, metabolic acidosis, metabolic alkalosis, and abnormalities of calcium and phosphorus metabolism are associated with substantial morbidity and mortality when not promptly recognized and appropriately treated. Acc…

Sublingual Administration of Desmopressin Oral Disintegrating Tablet in a Neonate With Central Diabetes Insipidus.

Precise titration of desmopressin (1-deamino-8-D-arginine vasopressin; DDAVP) is essential for managing neonatal central diabetes insipidus … semilobar holoprosencephaly, and a cleft lip and palate. The patient exhibited hypotonic polyuria, hypernatremia, elevated serum osmolality, and low plasma arginine vasopressin (AVP) levels within the first week of life. At 30 days of age, we dissolved a 60-µg DDAVP ODT in 5 mL of water and administered it sublingually. The initial dose of 3 µg/kg/day was titr …

Using old liver grafts for liver transplantation: where are the limits?

The scarcity of ideal liver grafts for orthotopic liver transplantation (OLT) has led transplant teams to investigate other sources of grafts in order to augment the donor liver pool. One way to get more liver grafts is to use marginal donors, a not well-defined group which includes mainly donors > 60 years, donors with hypernatremia or macrosteatosis > 30%, donors with hepatitis C virus or hepatitis B virus positive serologies, cold ischemia time > 12 h, non-heart-beating donors, and grafts from split-livers or living-related donations. Perhaps the most practical and frequent measure to incre…

Wilms tumor arising in a child with X-linked nephrogenic diabetes insipidus.

We report on a child with X-linked nephrogenic diabetes insipidus (NDI) who developed Wilms tumor (WT). Nephrogenic diabetes insipidus is caused by mutations of the arginine vasopressin receptor (AVPR2) or aquaporin-II (AQP2) genes. Wilms tumor is also genetically heterogeneous and is associated with mutations of WT1 (15-20%), WTX (20-30%) and other loci. The boy presented at 5 months with failure to thrive, polyuria, hypernatremia and abdominal mass. Analysis of leukocyte DNA showed a novel missense mutation (Q174H) of the AVPR2 gene, which was not present in his mother. In cells (WitS) isola…

Electrolytic disorders, hyperosmolar states, and lactic acidosis in brain-dead patients.

Despite the use of well-accepted protocols for donor maintenance, the severe electrolytic disorders are not infrequent with deleterious cons … dence of elevated lactate, and the relationship between lactate levels and bases deficit as well as hypernatremia. It can be concluded that, even following rigid protocols, the maintenance of brain-dead patients demands a close evaluation of electrolyte levels. Our results also suggest that the inclusion in the monitoring protocol of anerobic metabolism data in …
