Parathyroid Surgery for Primary Hyperparathyroidism in 2026.

Primary hyperparathyroidism (PHPT) is the most common parathyroid disease and one of the most common endocrine disorders. It results from autonomous overproduction of parathyroid hormone, which in the classic form of the disease causes hypercalcaemia. The ready access to biochemical testing in high-income countries has moved the disease spectrum from a classic symptomatic presentation to a biochemical anomaly that is asymptomatic or minimally symptomatic. Untreated PHPT, however, remains associated with significant consequences. This review summarizes up-to-date concepts in the diagnosis, imag…

Primary hyperparathyroidism in a child with tuberous sclerosis.

Primary hyperparathyroidism in patients with tuberous sclerosis (TS) is extremely rare: only three cases have been reported previously, two of them in pediatric patients. An 11-year-old male patient with a clinical and molecular diagnosis of TS presented with mild symptomatic hypercalcemia in routine laboratory tests. Biochemical evaluation confirmed hypercalcemia with hypophosphatemia and inadequate parathyroid hormone levels, with radiological signs of hyperparathyroidism. Ultrasound demostrated hyperplasia of the left superior parathyroid gland, and a sestamibi and technetium-99m scintigram…

Surgery for kidney-related hyperparathyroidism: A review.

Secondary hyperparathyroidism often occurs as a result of a disruption in calcium and phosphorus homeostasis caused by chronic kidney disease (CKD). While some individuals experience a remission of parathyroid gland disease subsequent to kidney transplantation due to the restoration of calcium homeostasis, a subset continues to exhibit elevated parathyroid hormone due to autonomous secretion, a condition otherwise known as tertiary hyperparathyroidism. This review aims to investigate the intricacies of diagnosis, presentation, medical treatment, state-of-the-art surgical options, and strategie…

What parathyroid imaging is required for hyperparathyroidism?

In over 80% of cases, primary hyperparathyroidism results from hypersecretion of PTH by a single parathyroid adenoma. Multi-glandular involvement, combining adenoma and/or hyperplasia in varying proportions, is also possible, although less frequent. When the diagnosis of hyperparathyroidism is certain and surgery is envisaged, imaging is useful for locating the hyperfunctioning gland or glands. First-line exploration is based on a parathyroid ultrasound and a nuclear medicine examination, which may be parathyroid scintigraphy, preferably double isotope I/12399m Tc-sestamibi, with planar and to…

Tertiary hyperparathyroidism in a dog - Case report.

Tertiary hyperparathyroidism (THPT) is rarely discussed in animals. This study aimed to report a case of tertiary renal hyperparathyroidism in a dog. A 16-year-old spayed female, Shih Tzu, previously diagnosed with stage 2 chronic kidney disease, presented increased levels of serum urea, creatinine, total calcium, ionized calcium, phosphorus and PTH, indicating THPT. Clinical treatment for the renal condition was initiated and cinacalcet hydrochloride (0.5 mg*kg-1, orally, once daily for 7 days, followed by 1 mg*kg-1, orally, once daily for 7 days) was administered to correct the hormonal diso…

Surgery for normocalcemic hyperparathyroidism is safe and effective.

BACKGROUND: The ideal management of normocalcemic primary hyperparathyroidism is controversial and is not discussed in recent guidelines as a result of limited published data. To help fill this knowledge gap, we aimed to report the outcomes of a large case series of patients with normocalcemic primary hyperparathyroidism who underwent parathyroidectomy. METHODS: A retrospective review of a prospectively maintained database was conducted to identify patients with normocalcemic primary hyperparathyroidism who underwent parathyroidectomy from 2000 to 2023. A small subset of patients participated…

Familial states of primary hyperparathyroidism: an update.

BACKGROUND: Familial primary hyperparathyroidism (PHPT) includes syndromic and non-syndromic disorders. The former are characterized by the occurrence of PHPT in association with extra-parathyroid manifestations and includes multiple endocrine neoplasia (MEN) types 1, 2, and 4 syndromes, and hyperparathyroidism-jaw tumor (HPT-JT). The latter consists of familial hypocalciuric hypercalcemia (FHH) types 1, 2 and 3, neonatal severe primary hyperparathyroidism (NSHPT), and familial isolated primary hyperparathyroidism (FIHP). The familial forms of PHPT show different levels of PHPT penetrance, dev…

Denosumab for Management of Hypercalcemia in Primary Hyperparathyroidism.

Denosumab is a completely human monoclonal high-affinity antibody that binds to the nuclear factor kappa-B ligand (RANKL) and is widely used to treat osteoporosis. Furthermore, it can potentially lower serum calcium levels by inhibiting osteoclast activation and preventing bone calcium from being released into the blood. This review aimed to provide evidence of the efficacy and safety of denosumab in treating hypercalcemia in primary hyperparathyroidism (PHPT). PubMed and the Cochrane Library were searched for published studies that described denosumab for hypercalcemia management in PHPT. Dat…

A Case of Parathyroid Carcinoma in Renal Hyperparathyroidism.

INTRODUCTION: Parathyroid carcinoma is a rare malignant endocrine tumor that is usually associated with primary hyperparathyroidism. The coexistence of parathyroid carcinoma and renal hyperparathyroidism is a rare phenomenon. Hence, we present a case of parathyroid carcinoma in a patient with tertiary hyperparathyroidism. CASE PRESENTATION: Our patient is a 31-year-old woman with a past medical history of end-stage renal failure (ESRF), on hemodialysis for the past 18 years. She was referred by her nephrologist to the endocrine surgery department for consideration of parathyroidectomy in view…

Phosphate: An underrated component of primary hyperparathyroidism.

Primary hyperparathyroidism (PHPT) is a systemic disease that affects all the systems of the body, specifically the bones and the kidneys. Its main action is on calcium homeostasis. It tries to preserve the body's calcium level at the cost of phosphate. The criteria for surgery in asymptomatic PHPT patients revolve around raised serum calcium levels, renal dysfunction or nephrolithiasis, and bone health. It does not take into account the serum phosphate levels. Depending on the serum level, Hypophosphatemia is divided into mild, moderate, and severe categories. In PHPT, several studies have su…

Sex differences in patients with primary hyperparathyroidism.

BACKGROUND: Differences in presenting symptoms of primary hyperparathyroidism and outcomes of parathyroidectomy between sexes have been described, but whether these can be assessed by perioperative use of a validated tool, such as the Pasieka Parathyroidectomy Assessment Score, is unknown. METHOD: All patients with primary hyperparathyroidism were asked to complete symptom assessment at the preoperative and postoperative visits. The assessment included a query for 13 Pasieka Parathyroidectomy Assessment Score parameters evaluated using a visual analog scale as described by Pasieka (summative …

Outcomes of subtotal parathyroidectomy for renal hyperparathyroidism.

BACKGROUND: Renal hyperparathyroidism due to end-stage kidney disease is associated with considerable morbidity, and when refractory is treated with parathyroidectomy. Recurrent renal hyperparathyroidism is a major surgical complication, yet initial target parathyroid remnant size and outcomes, including rates of recurrence are not well elucidated. METHODS: This is a single-institution retrospective cohort study of patients who underwent initial subtotal parathyroidectomy for renal hyperparathyroidism on dialysis, from 1990-2022. The subtotal parathyroidectomy was defined as resection of 3 pa…

Brown tumor of hyperparathyroidism with multiple lesions.

Brown tumors are rare focal giant-cell lesions that arise as a direct result of the effect of parathyroid hormone (PTH) on bone tissue in some patients with hyperparathyroidism. Browns tumor is a syndrome associated with an increase in PTH levels by parathyroid glands resulting in hypercalcemia. In the present case report, a 44-year-old female patient presented with a rare case of brown tumor with multiple lesions in the head-and-neck region. The recent advance in various diagnostic and biochemical tests helps in early diagnosis of hyperparathyroidism cases. The dentist should be aware of oral…

Changes in Imaging Utilization for Primary Hyperparathyroidism.

INTRODUCTION: Primary hyperparathyroidism is now largely managed surgically via minimally invasive techniques. This shift was aided by preoperative imaging, which saw drastic increases in utilization in the 1990s. Since then, it is unclear how the role of preoperative imaging has changed with regard to surgical management of primary hyperparathyroidism. This study aims to describe the trend in preoperative localization techniques for surgical management of primary hyperparathyroidism using career data from two endocrine surgeons over the last 20 years. METHODS: Parathyroid case data was obtai…

Epidemiology, Pathophysiology, and Genetics of Primary Hyperparathyroidism.

In this narrative review, we present data gathered over four decades (1980-2020) on the epidemiology, pathophysiology and genetics of primary hyperparathyroidism (PHPT). PHPT is typically a disease of postmenopausal women, but its prevalence and incidence vary globally and depend on a number of factors, the most important being the availability to measure serum calcium and parathyroid hormone levels for screening. In the Western world, the change in presentation to asymptomatic PHPT is likely to occur, over time also, in Eastern regions. The selection of the population to be screened will, of …
