CONTEXT: Central diabetes insipidus (CDI) can be caused by several diseases, but in about half of the patients the etiological diagnosis remains unknown. Lymphocytic infundibulo-neurohypophysitis (LINH) is an increasingly recognized entity among cases of idiopathic CDI; however, the differential diagnosis from other pituitary diseases including tumors can be difficult because of similar clinical and radiological manifestations. Congenital absence of the internal carotid artery is a rare anatomical variant. The primarily reported origins of the ophthalmic artery associated with this variant are the posterior communicating, middle cerebral, and external carotid arteries. We report the case of a patient without an internal carotid artery whose ophthalmic artery originated from the contralateral internal carotid artery. The superior hypophyseal and prechiasmal arteries may contribute to collateral flow to the orbit from the contralateral internal carotid artery.

The German Society of Endocrinology (DGE) has a long-standing scientific and clinical focus on the pituitary. The pituitary working group 'Arbeitsgemeinschaft Hypophyse' is an interdisciplinary special interest group with a focus on advancing diagnosis and treatment of pituitary conditions. On the occasion of the English publication of the S2K clinical guideline Diagnosis and therapy of clinically hormonally inactive pituitary tumors, we present here a series of 12 articles from internationally renowned authors from inside and outside of Germany.

Craniopharyngiomas are benign cystic para-hypophyseal tumors often associated with hypopituitarism and visual-field abnormalities. Their therapy by surgery and external beam radiotherapy is imperfect. The intracavitary instillation of beta-emitting colloid radiopharmaceuticals into the cysts permits the delivery of far higher radiation doses to the cyst lining than is possible by external beam radiotherapy. This technique permits destruction of the lining epithelium with resultant elimination of cyst fluid formation and cyst shrinkage in up to 80% of cases.

Radioimmunoassay was used for the assessment of serum hormone levels. Dysfunctions in the fetoplacental system (decreased levels of progesterone and estrogens) were revealed as were elevated levels of somatotropic hormone, which were in inverse proportion to the fetal weight, in 20 pregnant females who abused alcohol during the entire pregnancy. It was suggested that higher production of somatotropic hormone could occur in the presence of hypophyseal tension which had been evidently developed before the pregnancy.

The experiments on adult (4-6 months) and old (28-30 months) rats revealed changes that develop in old age in various links of the regulatory system of aldosterone secretion: the hypothalamic-hypophyseal control of mineralocorticoid secretion decreased, and the sensitivity of the glomerular zone to ACTH, vasopressin, and K+, as well as that of the myocardium, skeletal muscle and kidney to aldosterone increased. Changes in tissue sensitivity to aldosterone may be due to the increased affinity of mineralocorticoid receptors to aldosterone in old age.

We report a case of pituicytoma, a rare primary tumor of the neurohypophysis. A 64-year-old man presented with progressive visual complaints, bitemporal hemianopsia, and headache. Imaging studies revealed distinctive features of a mass lesion that thickened the pituitary stalk with a bilobed protrusion extending into the hypothalamus. Angiography demonstrated tumor vascular supply from the superior hypophyseal arteries representing the diencephalic branches of the internal carotid arteries. We discuss the imaging and pathology of this unusual tumor.

Hypothalamo-hypophyseal metastases are still a relatively unfamiliar entity. Mostly they are confirmed by autopsy. Rear clinical manifestations are presented by diabetes insipidus. In general, patients with endocrine breast carcinoma metastases are young, premenopausal women, with great tumour burden and widely disseminated tumor. We report 54-year-old woman with diabetes insipidus caused by solitary breast carcinoma metastasis, eight years after primary tumour was diagnosed. The operative and radiation therapy were effective.

A radioimmunological examination (screening) of hypophyseal-thyroid function in 2400 newborns within the 1st week of life revealed 111 infants with transitory forms of hyperthyroidism that was observed during the 1st month of the life. The main causes of disturbed thyroid function were endocrine diseases in mothers, a complicated course of pregnancy and delivery. It made it possible to detect not only congenital thyroid diseases but also to evaluate the function of the most important hormonal system in the adaptation of an infant in the postnatal period.
