Clinical profile of idiopathic normal pressure hydrocephalus with reduced myocardial sympathetic nerve function.

INTRODUCTION: Findings of reduced cardiac sympathetic nerve function and pareidolic illusions have been useful for identifying Lewy body disease (LBD). Therefore, we aimed to investigate whether these findings are associated with a high prevalence of LBD-related symptoms and poor shunt responsiveness in patients with idiopathic normal pressure hydrocephalus (iNPH). METHODS: We retrospectively analyzed 104 iNPH patients who underwent preoperative 123I-metaiodobenzylguanidine myocardial scintigraphy and the Pareidolia test. We divided the patients into the abnormal cardiac scan (AC) group and t…

Long-Term Outcomes in a Parkinson's Disease Minor Phenomena Cohort.

BACKGROUND: Minor phenomena, including illusions, feeling of presence, and passage hallucinations, are common in Parkinson's disease (PD) and may be a risk factor for the development of psychosis (hallucinations and delusions). OBJECTIVES: The objective of this study was to determine the 3-year cumulative probability of developing psychosis among individuals with PD minor phenomena. METHODS: We conducted a 3-year, prospective, observational study of individuals with PD minor phenomena with biannual study visits. The presence of psychosis was ascertained using the Movement Disorder Society-Un…

Impact of Focal Muscle Vibration on Flaccid Upper Limb Motor Paralysis following Acute Brain Disease: A Case Study.

Focal muscle vibration (FMV) is increasingly being recognized as a rehabilitative therapy for enhancing motor function in central nervous system (CNS) diseases, particularly in patients with fine motor control deficits stemming from CNS damage. Brain lesions from these diseases disrupt the motor networks, necessitating novel rehabilitation strategies. By applying vibrations to muscles, FMV stimulates sensory fibers to induce cortical activity and kinesthetic illusions. While initial studies have highlighted FMV's role in reducing spasticity, recent evidence points to its potential in treating …

Closed-Eye Visual Hallucinations Preceding Severe Alcohol Withdrawal.

Few existing cases of closed-eye visual hallucinations have been reported. These rare perceptual disturbances are distinct from open-eye vis … of agitation, space-time disorientation, open-eye visual hallucinations, and emotionally disturbing illusions. The patient was promptly transferred to the intensive care unit (ICU) to manage severe alcohol withdrawal, including administration of intravenous sedation. During follow-up evaluation, the patient retained a clear memory of the closed-eye hallucin …

Risk factors for minor hallucinations in Parkinson's disease.

OBJECTIVES: Minor hallucinations (MHs), including sense of presence, passage hallucinations, and visual illusions, have been reported in Parkinson's disease (PD). Here, we investigated the prevalence and associated risk factors for MHs according to appearance time. METHODS: Data on the clinical characteristics and the appearance time of MHs for 100 PD patients were collected using a questionnaire and analyzed. MHs were classified into two groups according to the time when MHs appeared: MHs appearing while awake during the daytime (dMHs) and MHs appearing at arousal from sleep during the night…

The prevalence and characteristics of hallucinations, delusions and minor phenomena in a non-demented population sample aged 60 years and over.

OBJECTIVE: Psychotic phenomena can occur in non-clinical subjects. The goals of this study were to assess the prevalence of delusions, hallucinations and minor 'psychotic' phenomena (visual illusions, feeling of presence and passage hallucinations) and to describe the characteristics of the latter in a non-clinical older population. METHODS: Three hundred and thirteen individuals aged 60 years and older, without cognitive deficits (according to mini-mental state examination scores) or patent psychotic disease, answered a structured questionnaire focusing on delusions, hallucinations and minor…

Visual complaints and visual hallucinations in Parkinson's disease.

BACKGROUND: Visual symptoms are common in Parkinson's disease (PD) and are frequently under-diagnosed. The detection of visual symptoms is i … exclusively found in PD patients. The same was true for recurrent complex visual hallucinations and illusions (PD vs. CONTROLS: both 17.0% vs. 0%; p < 0.001). Multiple RVC (43.2% vs. 15.8%) and multiple RVH (29.5% vs. 5.6%) were also more common in PD patients (both p < 0.001). RVC did not predict recurrent complex visual hallucinations; but double vision …

Pentobarbital anesthesia alters neural responses in the precedence effect.

The precedence effect (PE) is thought to be beneficial for proper localization and perception of sounds. The majority of recent physiologica … leading stimuli but can prolong the recovery time of lagging stimuli to paired sources produced PE illusions, which was gradually attenuated during recovery from anesthesia. Thus, extracellular recording immediately after administration of pentobarbital should be avoided in physiological studies of neural correlates of PE.

[Psychotic symptoms in Parkinson's disease].

About one third of patients with Parkinson's disease (PD) experience hallucinations, mostly of a complex visual type, less often auditory or tactile. Minor hallucinatory phenomena, including sense of presence, passage hallucinations and visual illusions are frequent. Hallucinations primarily occur in a context of clear sensorium in patients with longstanding PD. They are more frequent in the evening or during the night. Insight in the hallucinatory nature of the phenomenon may be retained, partial, fluctuating, or abolished. An altered insight is common when cognitive impairment is present, an…

Absence of short-term vestibular modulation of muscle sympathetic outflow, assessed by brief galvanic vestibular stimulation in awake human subjects.

The failure of GVS to elicit a change in muscle sympathetic nerve activity, as doc …

The hallucinations of widowhood.

227 widows and 66 widowers were interviewed to determine the extent to which they had hallucinatory experiences of their dead spouse. The people interviewed formed 80.7% of all widowed people resident within a defined area, in mid-Wales, and 94.2% of those suitable, through the absence of incapacitating illness, for interview.Almost half the people interviewed had hallucinations or illusions of the dead spouse. The proportion of men and women who had these experiences was similar. The hallucinations often lasted many years but were most common during the first 10 years of widowhood. Social iso…

Nociceptive sensitization and pain perception are dissociable in humans.

Although pain and nociception are conceptually distinct, whether nociceptive sensitization can occur without pain remains unknown. Here, we show that convergent innocuous thermal stimulation elicits primary mechanical and secondary hyperalgesia without pain perception. In a preregistered study, we compared thermal grill illusion (TGI) responders (n = 96) and nonresponders (n = 64) by quantifying primary mechanical and secondary hyperalgesia following thermal grill stimulation. In responders, we further compared thermal grill-induced sensitization with that evoked by its innocuous component tem

Changes in pupil diameter are correlated with the occurrence of pareidolias in patients with dementia with Lewy bodies.

Pareidolias are visual illusions of meaningful objects, such as faces and animals, that arise from ambiguous forms embedded in visual scenes. Pareidolias and visual hallucinations have been suggested to have a common underlying neural mechanism in patients with dementia with Lewy bodies (DLB). The aim of the present study was to find an externally observable physiological indicator of pareidolias. Using a pareidolia test developed by Uchiyama and colleagues, we evoked pareidolias in patients with DLB and recorded the resultant changes in the diameters of their pupil. The time frequencies of ch…

A role of 3-D surface-from-motion cues in motion-induced blindness.

Motion-induced blindness (MIB), the illusory disappearance of local targets against a moving mask, has been attributed to both low-level stimulus-based effects and high-level processes, involving selection between local and more global stimulus contexts. Prior work shows that MIB is modulated by binocular disparity-based depth-ordering cues. We assessed whether the depth effect is specific to disparity by studying how monocular 3-D surface from motion affects MIB. Monocular kinetic depth cues were used to create a global 3-D hourglass with concave and convex surfaces. MIB increased for station
