[Prevention in otology-the key to lifelong hearing health].

BACKGROUND: Hearing loss is a major global health issue with potentially severe consequences for speech development, social integration, and cognitive health. A significant proportion of this burden is preventable through targeted strategies applied across the human lifespan. OBJECTIVE: This narrative review synthesizes key evidence-based preventive measures in otology and provides practical recommendations for clinicians. MATERIALS AND METHODS: A selective review of the current literature was conducted, including national clinical guidelines, systematic reviews, epidemiological studies, and

Using an In Vivo Mouse Model to Determine the Exclusion Criteria of Preexisting Anti-AAV9 Neutralizing Antibody Titer of Pompe Disease Patients in Clinical Trials.

The efficacy of adeno-associated virus (AAV)-based gene therapy is dependent on effective viral transduction, which might be inhibited by preexisting immunity to AAV acquired from infection or maternal delivery. Anti-AAV neutralizing Abs (NAbs) titer is usually measured by in vitro assay and used for patient enroll; however, this assay could not evaluate NAbs' impacts on AAV pharmacology and potential harm in vivo. Here, we infused a mouse anti-AAV9 monoclonal antibody into Balb/C mice 2 h before receiving 1.2 × 1014 or 3 × 1013 vg/kg of rAAV9-coGAA by tail vein, a drug for our ongoing clinica

Parents' and Health Professionals' Attitudes to Advancing Primary MMR Vaccine Administration from Fifteen to Six Months of Age-A Qualitative Thematic Analysis Embedded in a Randomized Trial.

Declining levels and duration of passively acquired maternal antibodies prompted a Danish trial to test the feasibility of advancing administration of the first measles, mumps, and rubella vaccine (MMR1) from 15 to 6 months of age. A trial-embedded qualitative study aimed to understand parents' (N = 24) and health professionals' (N = 11) attitudes about the measles, mumps, and rubella vaccine (MMR) in general and about advancing MMR1 administration. Overly positive parent attitudes were contrasted by members of a vaccine-skeptical organization including parents considering that their child was

Chronic hepatitis B virus infection and rubella susceptibility in pregnant women.

Increased rubella susceptibility has been shown in subjects from the Asian-Pacific region where chronic hepatitis B virus (HBV) infection is endemic. This study was performed to explore the relationship between chronic HBV infection and rubella susceptibility in the obstetric population. We conducted a retrospective cohort study on 50556 pregnant women delivered in a university obstetric unit from January 1998 to June 2008. The incidence of rubella susceptibility according to maternal HBV carrier status was examined. HBV infection and rubella susceptibility were found in 5105 (10.1%) and 6102

DNA vaccines and their applications in veterinary practice: current perspectives.

Inoculation of plasmid DNA, encoding an immunogenic protein gene of an infectious agent, stands out as a novel approach for developing new generation vaccines for prevention of infectious diseases of animals. The potential of DNA vaccines to act in presence of maternal antibodies, its stability and cost effectiveness and the non-requirement of cold chain have heightened the prospects. Even though great strides have been made in nucleic acid vaccination, still there are many areas that need further research for its wholesome practical implementation. Major areas of concern are vaccine delivery,

Postnatal transmission of human immunodeficiency virus type 1: the breast-feeding dilemma.

Human milk has been considered only recently as a source of transmission for the human immunodeficiency virus. The estimated postnatal transmission rate from mothers who acquired human immunodeficiency virus infection while lactating is 26% (95% confidence interval 13% to 39%) and may be in the range of 8% to 18% from mothers who were infected before becoming pregnant. Risk factors for postnatal transmission include maternal immune deficiency and the presence of human immunodeficiency virus-infected cells in milk. Some milk factors may be protective against postnatal transmission such as speci

Medical aspects of pregnancy.

BACKGROUND: Physiologic changes during pregnancy can precipitate or exacerbate potentially serious maternal illnesses that may adversely affect the outcome for both mother and fetus. OBJECTIVE: To review the common medical conditions that may complicate pregnancy. SUMMARY: Venous thromboembolic disease is the most common medical disorder complicating pregnancy; heparin is the safest and most effective treatment and prophylactic agent in high-risk patients. Systemic lupus erythematosus may flare during pregnancy and may require therapy with corticosteroids, which may also be needed to treat i

Low maternal pertussis protection and quantified infant risk: supporting maternal Tdap vaccination in China.

BACKGROUND: Pertussis remains a significant threat to infants, particularly in settings like China, where maternal tetanus-diphtheria-acellular pertussis (Tdap) vaccination is not part of the National Immunization Program, and data on maternal pertussis immunity are limited. This study aimed to assess the seroprevalence of protective pertussis toxin (PT) IgG antibodies in pregnant women and quantify the potential risk of infant pertussis under varying epidemiological scenarios. METHODS: A cross-sectional serosurvey was conducted among 1,205 pregnant women in Jiangsu Province, China, from Janu

Maternal HIV infection and placental malaria reduce transplacental antibody transfer and tetanus antibody levels in newborns in Kenya.

BACKGROUND: In clinical trials, maternal tetanus toxoid (TT) vaccination is effective in protecting newborns against tetanus infection, but inadequate placental transfer of tetanus antibodies may contribute to lower-than-expected rates of protection in routine practice. We studied the effect of placental malaria and maternal human immunodeficiency virus (HIV) infection on placental transfer of antibodies to tetanus. METHODS: A total of 704 maternal-cord paired serum samples were tested by ELISA for antibodies to tetanus. The HIV status of all women was determined by an immunoglobulin G antibo

Effects of porcine epidemic diarrhoea virus vaccination timing and route of administration on colostrum antibody levels and prevention of diarrhoea in piglets on a pig farm with porcine epidemic diarrhoea.

Piglets rely on passive immunity acquired from maternal-derived antibodies obtained by ingesting colostrum and milk from immunised sows. This study evaluated the differences in colostrum antibody profiles (IgA, IgG, and neutralising antibodies) according to the timing, frequency, and administration route of the porcine epidemic diarrhoea virus (PEDV) vaccination in pigs with confirmed PEDV infection, and to assess the corresponding clinical protective effects of PEDV vaccination in neonatal piglets. Piglets of sows that received two or more vaccinations, including an oral administration of a l

Genetic loci responsible for atrioventricular block in children.

Atrioventricular block (AVB) represents a prevalent form of bradyarrhythmia in the pediatric population, broadly classified into congenital and acquired subtypes. Historically, the underlying mechanisms of AVB were predominantly attributed to structural cardiac anomalies, maternal autoantibody-mediated inflammatory processes, and pharmacological agents; however, the contribution of genetic factors remained largely underexplored. With the advent and progressive refinement of genetic sequencing technologies, a growing repertoire of arrhythmia-associated genes has been systematically identified.

Human Tregs at the materno-fetal interface show site-specific adaptation reminiscent of tumor Tregs.

Tregs are crucial for maintaining maternal immunotolerance against the semiallogeneic fetus. We investigated the elusive transcriptional profile and functional adaptation of human uterine Tregs (uTregs) during pregnancy. Uterine biopsies, from placental bed (materno-fetal interface) and incision site (control) and blood were obtained from women with uncomplicated pregnancies undergoing cesarean section. Tregs and CD4+ non-Tregs were isolated for transcriptomic profiling by Cel-Seq2. Results were validated on protein and single cell levels by flow cytometry. Placental bed uTregs showed elevated

Systematic review of the clinical development of group B streptococcus serotype-specific capsular polysaccharide-based vaccines.

INTRODUCTION: Vaccination against group B Streptococcus (GBS) during pregnancy could provide protection against disease in the mother, fetus, and newborn. Immunity through transplacental acquired antibodies in the newborns could persist through early infancy, reducing the risk of early-onset (<7 days age) and late-onset (7-89 days age) disease. We conducted a systematic review of clinical trials on GBS capsular polysaccharide (CPS) vaccine to assess its safety and immunogenicity in pregnant and nonpregnant adults. AREAS COVERED: We searched literature databases PubMed (Medline), Scopus, and t

Dynamics of pneumococcal transmission in vaccine-naive children and their HIV-infected or HIV-uninfected mothers during the first 2 years of life.

Pneumococcal vaccine-naïve mother-child dyads in South Africa had nasopharyngeal swabs taken 9 times within the first 2 years of the children's lives between January 2007 and May 2009. To quantify the strength of the association of serotype-specific carriage in mother-child dyads, a stochastic transmission model was fitted to the data. Children were more susceptible to individual serotypes included in the 7-valent pneumococcal conjugate vaccine (PCV7) transmitted by their mothers than vice versa; however, children infected their mothers with these serotypes more frequently than mothers infecte

Platelets, inflammation and tissue regeneration.

Blood platelets have long been recognised to bring about primary haemostasis with deficiencies in platelet production and function manifesting in bleeding while upregulated function favourises arterial thrombosis. Yet increasing evidence indicates that platelets fulfil a much wider role in health and disease. First, they store and release a wide range of biologically active substances including the panoply of growth factors, chemokines and cytokines released from a-granules. Membrane budding gives rise to microparticles (MPs), another active participant within the blood stream. Platelets are e

[Early recurrent spontaneous abortion: How to take care in 2006?].

More than 1% of the couples whishing children suffer from recurrent miscarriage, but investigations and treatment are not consensual. Most patients have several risk factors, and a minimum investigation of known factors has to be undertaken: karyotyping of the couple, hysteroscopy for searching uterine anatomic anomalies, evaluation for thrombophilias (anticardiolipin antibodies, lupus anticoagulant, protein C activity, Proteine S activity, factor V Leiden and factor II mutations, activated protein C resistance), antinuclear antibodies. Systemic diseases (like lupus) and endocrine abnormalitie
