[Risk factors for the failure of the InSure method in very preterm infants with respiratory distress syndrome].

OBJECTIVE: To study the risk factors for the failure of the InSure method in very preterm infants with respiratory distress syndrome (RDS). METHODS: Seventy-one very preterm infants with RDS treated with InSure method were enrolled. These infants were categorized into two groups: InSure success (42 cases) and InSure failure (29 cases). The differences in basic information were compared between the two groups, and logistic regression analysis was used to identify the risk factors for InSure failure. RESULTS: The failure rate of the InSure method was 41%. The failure group were much lower in t

Insurer views on reimbursement of preventive services in the dental setting: results from a qualitative study.

OBJECTIVES: We explored insurers' perceptions regarding barriers to reimbursement for oral rapid HIV testing and other preventive screenings during dental care. METHODS: We conducted semistructured interviews between April and October 2010 with a targeted sample of 13 dental insurance company executives and consultants, whose firms' cumulative market share exceeded 50% of US employer-based dental insurance markets. Participants represented viewpoints from a significant share of the dental insurance industry. RESULTS: Some preventive screenings, such as for oral cancer, received widespread in…

Initial respiratory support of preterm infants: the role of CPAP, the INSURE method, and noninvasive ventilation.

This article explores the potential benefits and risks for the various approaches to the initial respiratory management of preterm infants. The authors focus on the evidence for the increasingly used strategies of initial respiratory support of preterm infants with continuous positive airway pressure (CPAP) beginning in the delivery room or very early in the hospital course and blended strategies involving the early administration of surfactant replacement followed by immediate extubation and stabilization on CPAP. Where possible, the evidence referenced in this review comes from individual ra

Private-sector vaccine purchase costs and insurer payments: a disincentive for using combination vaccines?

IMPORTANCE: Premature infants are not prepared developmentally, physically, or cognitively with the multiple survival skills of the typical newborn. OBJECTIVE: To determine whether physical activity generated spontaneously by premature infants stretching against the resistance of "prepod" garments is as effective as exercises provided by therapists in slowing bone density loss. DESIGN: Preterm very low birth weight infants were placed in two nonrandomized parallel groups according to birth age. SETTING: Infants routinely started in a traditional intense intervention setting and graduated to

Health care expenditures of children and adults with spina bifida in a privately insured U.S. population.

BACKGROUND: We provide new estimates of medical care utilization and expenditures over the lifespan for persons living with spina bifida in the United States. Updated estimates are essential for calculations of lifetime costs and for economic evaluations of prevention and management strategies for spina bifida. METHODS: We analyzed data from the 2001-2003 MarketScan database on paid medical and prescription drug claims of persons covered by employer-sponsored health insurance in the United States. Medical care utilization and expenditures during 2003 were analyzed for persons with a diagnosis…

Access to orthopaedic care for children with medicaid versus private insurance: results of a national survey.

BACKGROUND: It has been documented that children insured by Medicaid in California have significantly less access to orthopedic care than children with private insurance. Low Medicaid physician reimbursement rates have been hypothesized to be a major factor. The first objective of this study was to examine whether children insured by Medicaid have limited access to orthopedic care in a national sample. The second objective was to determine if state variations in Medicaid physician reimbursement rates correlate with access to orthopedic care. METHODS: Two-hundred fifty orthopedic surgeon's off…

The importance of health insurance as a determinant of cancer screening: evidence from the Women's Health Initiative.

BACKGROUND: Amid current changes in health care access across the United States, the importance of health insurance status and insurance type relative to demographic, actual, and perceived health variables as determinants of screening for breast, colorectal, and cervical cancer is uncertain. This analysis evaluates the hypothesis that health insurance independently predicts cancer screening in the Women's Health Initia tive Observational Study cohort. METHODS: Questionnaire data from 55,278 women en rolled in the Women's Health Initiative Observational Study between September 1994 and Februar…

Insurance-Related Disparities in Mental Health Status and Care Among Low-Income U.S. Adults, 2019-2023.

The COVID-19 pandemic affected mental health status and care among vulnerable Americans. We examined temporal changes in mental health status and assessed the associations between insurance types and mental health care among low-income U.S. adults. Using the 2019–2023 National Health Interview Survey, we analyzed data from 22,026 adults aged 18–64 with family incomes below 200% of the poverty threshold. Survey-adjusted descriptive statistics and linear probability models controlling for individual characteristics and time were used. Between 2019 and 2023, Medicaid enrollees consistently report…

Comparing endocrine therapy adherence across EHR and claims data by race and ethnicity, language, and insurance.

PURPOSE: We compared adherence measurements across electronic health records (EHRs) and claims data for 79 HR + breast cancer survivors, considering their race, ethnicity, language, and insurance status. METHODS: Adherence was evaluated using the medication possession ratio (MPR) and proportion of days covered (PDC). The correlation between EHR and claims data was assessed with Spearman correlation coefficients, while Kaplan-Meier curves were employed to visualize and analyze the time to the first instance of non-adherence and the time to discontinuation. RESULTS: A total of 75 breast cancer…

The Impact of Unemployment Insurance Benefit Cuts on Mental Health: Evidence From Early Pandemic Program Expirations.

This study is the first to estimate the effects of unemployment insurance (UI) benefit contractions on mental health. We leverage a unique quasi-natural experiment: early termination of the Federal Pandemic Unemployment Compensation (FPUC) and Pandemic Unemployment Assistance (PUA) programs across more than 20 states during the summer of 2021. Using high-frequency data from the Census Bureau's Household Pulse Survey (HPS), our difference-in-differences estimates suggest that early withdrawal from FPUC and PUA increases the probability of reporting anxiety symptoms among working-age adults with…

Machine Learning for Early Detection of Hidradenitis Suppurativa: A Feasibility Study Using Medical Insurance Claims Data.

Patients with hidradenitis suppurativa (HS) are often misdiagnosed and may wait up to 10 years to receive a diagnosis of HS. This study aimed to predict HS diagnosis prior to actual diagnosis on the basis of previous medical history using models developed with insurance claims data. Three machine learning models were compared with a model using features selected by a dermatologist (clinical baseline model). The study analyzed 5,900,000 United States individuals' insurance records over 13.5 years. The population included 13,886 patients with HS with at least 1 claim in each of the 2 years prior…
