Secondary use of patient data within decentralized studies using the example of rare diseases in Germany: A data scientist's exploration of process and lessons learned.

OBJECTIVE: Unlocking the potential of routine medical data for clinical research requires the analysis of data from multiple healthcare institutions. However, according to German data protection regulations, data can often not leave the individual institutions and decentralized approaches are needed. Decentralized studies face challenges regarding coordination, technical infrastructure, interoperability and regulatory compliance. Rare diseases are an important prototype research focus for decentralized data analyses, as patients are rare by definition and adequate cohort sizes can only be reac

Multidisciplinary management of difficult-to-treat drug resistant tuberculosis: a review of cases presented to the national consilium in Uganda.

BACKGROUND: Patients with drug resistant tuberculosis (DR-TB) with comorbidities and drug toxicities are difficult to treat. Guidelines recommend such patients to be managed in consultation with a multidisciplinary team of experts (the "TB consilium") to optimise treatment regimens. We describe characteristics and treatment outcomes of DR-TB cases presented to the national DR-TB consilium in Uganda between 2013 and 2019. METHODS: We performed a secondary analysis of data from a nation-wide retrospective cohort of DR-TB patients with poor prognostic indicators in Uganda. Patients had a treatme

[Pulmonary Tuberculosis--Is Surgery still Necessary?].

BACKGROUND: Tuberculosis is still one of the most common infectious diseases along with HIV and malaria and therefore represents a serious problem in the health sector. Due to immigrants and refugees, the disease is also present in Europe. The global increase of multidrug resistant tuberculosis leads to a highly significant and current importance of sufficient therapeutic measures. In recent years, this fact has led to a reevaluation of surgical therapy in the context of an interdisciplinary and multimodal treatment of multidrug resistant tuberculosis. In addition, despite an effective treatme

[The future of vascular medicine].

In the future vascular medicine will still have a great impact on health of people. It should be noted that the aging of the population does not lead to a dramatic increase in patient numbers, but will be associated with a changing spectrum of co-morbidities. In addition, vascular medical research has to include the intensive care special features of vascular patients, the involvement of vascular medicine in a holistic concept of fast-track surgery, a geriatric-oriented intensive monitoring and early geriatric rehabilitation. For the future acceptance of vascular medicine as a separate subject

Bridging systems: oral-kidney connections - pathophysiological links, clinical implications, and health system integration - a narrative review.

Emerging evidence supports a bidirectional relationship between chronic kidney disease (CKD) and oral diseases, mediated by systemic inflammation, endothelial dysfunction, and immune dysregulation. Periodontitis and caries are prevalent yet underrecognized contributors to the inflammatory burden in CKD, particularly in low-resource settings where oral and kidney care remain fragmented. This reviewsynthesizes clinical, epidemiologic, and mechanistic evidence fromobservational studies, interventional trials, and meta-analyses examiningoral–renal interactions. Understanding the spatiality in neglected tropical diseases: An ethnographic study of scrub typhus in Puducherry, India.

Individuals from marginalised backgrounds are often "placed" in spatial planes marked by disadvantages and poor resource availability, impacting their health outcomes. This study explored the role of spatiality in shaping health outcomes, with scrub typhus-a neglected tropical disease (NTD) of zoonotic origin-as an illustrative case. Employing a spatial justice framework, the study investigated the intersection of geospatial and socio-spatial factors influencing health and illness experience in Puducherry, India. An interdisciplinary mixed-methods approach was employed, involving geospatial ho

Oral health as a modifiable risk factor for neurodegeneration: Prudent optimism and interdisciplinary action.

Neurodegenerative diseases, including Alzheimer's disease (AD) and Parkinson's disease (PD), represent a growing public health burden, necessitating exploration of modifiable risk factors. Emerging evidence suggests a link between poor oral health, particularly chronic periodontitis, and neurodegeneration, mediated by systemic inflammation, direct bacterial invasion, and disruptions in the microbiota-gut-brain axis. Periodontal pathogens, such as Porphyromonas gingivalis, have been suggested to contribute to neuroinflammation, blood-brain barrier dysfunction, and amyloid-beta aggregation, all

Implantable Neural Speech Decoders: Recent Advances, Future Challenges.

The social life of locked-in syndrome (LIS) patients is significantly impacted by their difficulties to communicate. Consequently, researchers have started to explore how to decode intended speech from neural signals directly recorded from the cortex. The first studies in the late 2000s reported modest decoding accuracies. However, thanks to fast advances in machine learning, the most recent studies have reached decoding accuracies high enough to be optimistic about the clinical benefit of neural speech decoders in the near future. We first discuss the selection criteria for implanting a neura
