Preliminary in vivo study of an intra-aortic impeller pump driven by an extracorporeal whirling magnet.

To achieve the aim of long-term heart-assist with a simple implantable device, we have been trying to develop a minimal intra-aortic impeller blood pump driven by an extracorporeal magnetic device. The purpose of the current study was to evaluate its feasibility by acute in vivo animal tests. The minimal intra-aortic pump was a cage-supported rotor-impeller, 17 mm in diameter with a total length of 30 mm. The driving magnet, mounted extracorporeally, was 55 mm in diameter and 50 mm in length. Seventeen dogs weighing from 28-34 kg were used in the study. After thoracic incision, heparin (50 U/k

Intra-aortic balloon counterpulsation for cardiogenic shock due to cardiac contusion in an elderly trauma patient.

Blunt thoracic trauma may cause cardiac contusion and cardiogenic shock resistant to inotropic support. The use of intra-aortic balloon counterpulsation (IABCP) as a mechanical means of augmenting cardiac function following cardiac contusion is rare with case reports largely limited to its use in young trauma patients. We describe the case of a frail, 80-year-old woman who suffered cardiac contusion in a motor vehicle crash. She developed cardiogenic shock with electrocardiograph changes, elevated troponin T and severe global dysfunction on echocardiography. She was successfully managed with i

Role of intra-aortic counterpulsation in cardiogenic shock and acute myocardial infarction.

IACP has played an important role in the evolution of the therapy of cardiogenic shock. Although IACP has not developed to an independent treatment modality, it is an essential adjunct to facilitate early catheterization and reperfusion strategies. With this combined approach, hospital and long-term survival rates have reached encouraging results. In acute myocardial infarction, IACP may evolve into an intriguing adjunct to thrombolysis and/or PTCA. Preliminary data suggest that IACP might decrease the rate of reocclusion and reinfarction after thrombolysis and/or PTCA and, thus, interrupt the

Paraplegia following intraaortic balloon assistance.

In a patient receiving intraaortic balloon counterpulsation following myocardial revascularization, paraplegia developed two days post-operatively. Postmortem examination demonstrated a dissecting hematoma of the thoracic aorta and spinal cord infarction. The neurological deficit is thought to be due to embarrassment of spinal cord blood supply, and mechanisms of injury are discussed.

Outcomes of mitral valve replacement with complete annular decalcification.

BACKGROUND: This study aimed to examine the clinical outcomes of mitral valve replacement (MVR) in patients with severe mitral annular calcification (MAC) who required extensive decalcification and mitral annular reconstruction. METHODS: We reviewed 15 patients with severe MAC who underwent MVR between January 2016 and May 2022. In all cases, the calcified mitral annulus was resected completely using a Cavitron Ultrasound Surgical Aspirator, and a new annulus was created using bovine pericardium. In the acute postoperative phase, strict afterload reduction therapy using an intra-aortic balloo

Normalization of Diffuse ST-Depression with aVR Elevation After Rehydration in a Patient with Severe Aortic Stenosis.

BACKGROUND Isolated ST elevation in lead aVR in combination with global ST depression with normalization after rehydration is a unique electrocardiographic pattern that is associated with a broad range of diagnoses. Its association with left main coronary artery disease and other acute coronary syndromes suggest the need for early and aggressive cardiac evaluation. CASE REPORT A 53-year-old man presented with altered mental status and loss of consciousness. He was unresponsive, hypotensive, tachycardiac, and diaphoretic. An initial ECG showed diffuse ST depression with isolated ST elevation in

Caecal leiomyoma detected by whole-body MRI in a patient with Hodgkin lymphoma: first case report.

INTRODUCTION: Neurological complications are, at the present time, considered among the most important causes of morbidity and mortality after heart surgery. We evaluated their importance and risk factors. PATIENTS AND METHODS: We retrospectively reviewed 2, 528 consecutive patients who underwent cardiopulmonary bypass in a single center. In each one, we attended to previous vascular risk factors, such as surgical and postoperative events. We considered four categories of neurologic outcome: (1) persistent neurological focal deficits, (2) stupor or coma, (3) temporary neurological focal defic

Acute gastrointestinal complications after cardiac surgery.

Gastrointestinal problems, with an incidence of about 1%, may complicate the postoperative period after cardiovascular surgery, increasing morbidity, length of stay, and mortality. Several risk factors for the development of these complications, including preexisting conditions; advancing age; surgical procedure, especially valve, combined bypass/valve, emergency, reoperative, and aortic dissection repair; iatrogenic conditions; stress; ischemia; and postpump complications, have been identified in multiple research studies. Ischemia is the most significant of these risk factors after cardiovas

Pulsatile cardiopulmonary bypass using a centrifugal pump reduces acute kidney injury after cardiac surgery: a propensity score-matched analysis.

OBJECTIVE: Acute kidney injury (AKI) affects approximately 30% of patients undergoing cardiac surgery with conventional non-pulsatile cardiopulmonary bypass (CPB). Pulsatile flow has long been proposed for mitigating this complication. While the 2019 EACTS guidelines recommend pulsatile perfusion for high-risk renal patients, most evidence is based on intra-aortic balloon pump (IABP) studies, with limited evidence for centrifugal pumps. We aimed to evaluate whether pulsatile CPB using a centrifugal pump reduces postoperative AKI in patients undergoing cardiac surgery. METHODS: This retrospect

The hemodynamic effect of different left ventricular unloading techniques during veno-arterial extracorporeal life support: a systematic review and meta-analysis.

BACKGROUND: Pulmonary edema and left ventricular thrombosis may arise during veno-arterial extracorporeal life support due to an increase in cardiac load. This mechanical stress can be reduced through different left ventricular unloading techniques. We set out to quantitatively summarize the hemodynamic effects of available methods in patients treated with veno-arterial extracorporeal life support. METHODS: Literature was systematically searched for studies reporting left ventricular unloading during veno-arterial extracorporeal life support as reflected by changes in left atrial pressure, pu

The Role of Mechanical Circulatory Support in Acute Myocardial Infarction Complicated by Cardiogenic Shock-Part Two: Getting with the Guidelines and the Future ahead.

Despite advances in percutaneous coronary intervention and expansion in cardiac critical care, randomized controlled trials investigating mechanical circulatory support in acute myocardial infarction complicated by cardiogenic shock (AMI-CS) have shown inconsistent results. These studies are limited by sample size, heterogeneity in patient selection, variability in intervention timing, and inconsistent definitions and staging of shock severity. Current evidence, in general, provides limited guidance on optimal device choice and timing. Ongoing future trials with the addition of next-generation

Short- and long-term adverse events in patients on temporary circulatory support before durable ventricular assist device: An IMACS registry analysis.

BACKGROUND: Patients with cardiogenic shock (CS) needing temporary circulatory support (TCS) have poor survival rates after implantation of durable ventricular assist device (dVAD). We aimed to characterize post-dVAD adverse event burden and survival rates in patients requiring pre-operative TCS. METHOD: We analyzed 13,511 adults (Interagency Registry for Mechanically Assisted Circulatory Support [INTERMACS] Profiles 1-3) with continuous-flow dVADs in International Society for Heart and Lung Transplantation Registry for Mechanically Assisted Circulatory Support (2013-2017) according to the ne

The contemporary role of Impella in a comprehensive mechanical circulatory support program: a single institutional experience.

BACKGROUND: The treatment of cardiogenic shock with percutaneous mechanical circulatory support (MCS) is attractive: however, at present it is not clear which is the best strategy, as no survival benefit has been demonstrated for any device as single therapy. Aim of this study is to describe the role of percutaneous Impella in a comprehensive MCS program. METHODS: Observational study on 22 patients supported with the Impella device in our hospital from May 2013 to June 2014. RESULTS: Four patients (18 %) were treated with Impella alone, 8 patients (36 %) were treated with Impella and IABP, 6

[Influence of ECMO and IABP on coronary blood flow. Valuable combination or waste of resources?].

BACKGROUND: The treatment of patients in severe cardiogenic shock with an intra-aortic balloon pump (IABP) or extracorporeal membrane oxygenation (ECMO) is a common procedure to achieve stabilization. Only limited data are available on the simultaneous use of both systems. The aim of the present study was to assess the effect of the concomitant use of IABP and ECMO on coronary blood flow. In addition, the influence of antegrade and retrograde perfusion was evaluated. METHODS: A median sternotomy was performed in adult pigs under general anesthesia. Arterial ECMO perfusion was realized through

[Anesthetic management for mitral valve replacement in a patient with mitral stenosis and dilated cardiomyopathy].

A 69-year-old man who has mitral stenosis concomitantly with dilated cardiomyopathy underwent mitral valve replacement. The preoperative left ventricular ejection fraction was 23%. Preoperative dobutamine stress echocardiography showed poor response. High dose fentanyl was administered for induction and maintenance of general anesthesia, and blood pressure was kept with vasoconstrictors. Circulatory assist with milrinone and intraaortic balloon pumping after the cardiopulmonary bypass enabled us to obtain stable hemodynamics.

Cardiogenic shock: therapy and prevention.

Cardiogenic shock is defined as profound circulatory failure resulting in insufficient tissue perfusion to meet resting metabolic demands. It occurs in approximately 7.5% of patients with acute myocardial infarction. Treatment strategies include inotropic agents, use of intra-aortic balloon counterpulsation, and revascularization. Current evidence supports the use of primary angioplasty. Surgery should be considered in patients with triple-vessel disease. If early catheterization is not available, thrombolytic therapy should be given to eligible patients and transfer to an interventional facil
