An analysis of vascular pattern in modified P1 zone and relationship with recurrent laryngeal nerve.

PURPOSE: Modified P1 zone is defined as the zone related to the recurrent laryngeal nerve(RLN) 1 cm proximal to the nerve entry point to the larynx, where the vessels cross the nerve at a vertical plane. RLN injuries are common in this zone during thyroidectomy. This study aims to describe the vascular pattern at the modified P1 zone. METHODS: This prospective analysis included 131 consecutive subjects who underwent thyroid surgery between October 2023 and June 2024. Various clinical, radiological, histopathological and intraoperative parameters were studied. RESULTS: The study included 212

Interparietal Hernia Associated with Recurrent Inguinal Hernia: A Case Report.

INTRODUCTION: Interparietal hernia (IPH) is a rare subtype of inguinal hernia in which the hernial sac extends between the layers of the abdominal wall musculature. Due to its unusual anatomical location, preoperative diagnosis is often difficult. Reports of adult cases in the English-language literature remain extremely limited. IPH occurring in association with a recurrent inguinal hernia is even rarer. CASE PRESENTATION: A man in his 70s with a history of bilateral inguinal hernia repair presented with abdominal pain and constipation. Contrast-enhanced CT revealed herniation of the sigmoid

Abdominal wall abscess due to Mycolicibacterium neworleansense: a case report.

INTRODUCTION: Non-tuberculous mycobacteria (NTM) are widely distributed in the natural environment and can cause skin and soft tissue infections, typically through traumatic injuries or invasive medical procedures, especially in susceptible populations. Commonly encountered pathogens include Mycobacterium marinum and Mycobacterium abscessus, among others. However, cutaneous infections caused by Mycolicibacterium neworleansense are extremely rare. This report presents a case of abdominal wall abscess caused by Mycolicibacterium neworleansense, which was successfully managed with surgical debrid

Cavernoma of the left ventral striatum, anatomical and microsurgical implications of the ipsilateral transcallosal transrostral pathway.

BACKGROUND: Cerebral cavernous malformations are vascular abnormalities of the central nervous system with an incidence of 0.4-0.5% and an annual hemorrhage rate ranging from 0.7% to 1%. The most important and consistent risk factor for rebleeding is a prior hemorrhage, and deep lesions (within the thalamus, basal ganglia, and brainstem) characteristically carry a worse prognosis regarding annual hemorrhage rates (10.6% per patient per year vs 0.4% per patient per year for superficial cerebral lesions) and subsequent neurologic deterioration. The ventral striatum is a difficult region to appro

Effects of opioid-free anesthesia combined with iliofascial nerve block on perioperative neurocognitive deficits in elderly patients undergoing hip fracture surgery: study protocol for a prospective, multicenter, parallel-group, randomized controlled trial.

BACKGROUND: Perioperative neurocognitive dysfunction (PND), a prevalent complication affecting elderly surgical patients, poses substantial challenges to postoperative rehabilitation and long-term functional independence. Despite growing awareness of its clinical significance, current evidence regarding effective neuroprotective anesthetic strategies remains inconclusive. Where emerging evidence suggests opioid-free anesthesia (OFA) strategies could maintain analgesic efficacy while potentially attenuating opioid-associated neuroinflammatory mechanisms implicated in PND pathogenesis. This mult

Musculoskeletal Pain in Pediatric Surgeons: Prevalence, Impact, and Prevention-A Cross-Sectional Survey Study.

Recent studies show that at least three-quarters of surgeons who perform minimally invasive surgery suffer from musculoskeletal pain and discomfort. This problem may compromise surgical performance, patient outcomes, and career longevity. Because of small patient dimensions, pediatric surgery is associated with a different set of ergonomic challenges than adult surgery.A 59-item questionnaire was sent to all practicing pediatric surgeons in Germany. It contained a general assessment of demographics, practice setting, operative volume, types of operations performed, and compensatory interventio

Subarachnoid hemorrhage secondary to Chiari 1 malformation in a patient presenting with recurrent severe headache: A case report, imaging, and anatomic considerations.

BACKGROUND: Chiari 1 malformation (CM1) is a structural anomaly characterized by cerebellar tonsillar herniation through the foramen magnum. While typically associated with syringomyelia or hydrocephalus, its potential relationship with subarachnoid hemorrhage (SAH) remains poorly reported in the literature. CASE DESCRIPTION: We present the case of a 77-year-old woman who experienced recurrent episodes of thunderclap headache, which eventually progressed to altered consciousness following admission. Neuroimaging revealed a SAH involving the fourth ventricle and basal cisterns, alongside cereb

Primary giant hydatid cyst of the retroperitoneum: Diagnosis and therapeutic approaches.

INTRODUCTION AND IMPORTANCE: Primary hydatid cyst of the retroperitoneum is an exceedingly rare manifestation of hydatid disease. Diagnosis proves challenging due to nonspecific symptoms, and the condition is typically not suspected when facing a retroperitoneal cystic mass, necessitating awareness among clinicians and surgeons, particularly in endemic regions. CASE PRESENTATION: A 45-year-old male with a three-month history of progressive abdominal enlargement and pain. Living in a rural area, he exhibited a 30 cm, well-defined retroperitoneal cyst, with no guarding confirmed by CT-scan, wit

First Branchial Cleft Anomaly Mimicking Cholesteatoma: A Pediatric Case Study and Surgical Approach.

BACKGROUND First branchial cleft anomalies (FBCAs) are rare congenital disorders with clinical manifestations and anatomy that are variable and complex. Due to the anatomical proximity to the facial nerve and association with infection, FBCAs require accurate diagnosis and careful management. The purpose of this report was to share the diagnosis and treatment of an extremely rare case of a 4-year-old girl with an FBCA presenting as a painless mass of the left external auditory canal and an inconspicuous depression in the left mandibular angle, a presentation that can easily be misdiagnosed as

Extrahepatic abscess and dropped gallstones: a case report and a narrative review of an unusual delayed complication of laparoscopic cholecystectomy.

BACKGROUND: Laparoscopic cholecystectomy is the most common minimally invasive abdominal surgery procedure performed in Western countries; it offers several advantages over laparotomy but still carries some risks, such as intraoperative spillage of bile and gallstones. Diagnosis of dropped gallstones could be challenging, it is frequently delayed, and this can lead to further complications such as abscesses formation. METHODS: We report the history of a 51-year-old male with persistent dull abdominal pain in association to appetite loss, vomiting episodes and changes in regular bowel habits,

The structural connectivity of the human angular gyrus as revealed by microdissection and diffusion tractography.

The angular gyrus (AG) has been described in numerous studies to be consistently activated in various functional tasks. The angular gyrus is a critical connector epicenter linking multiple functional networks due to its location in the posterior part of the inferior parietal cortex, namely at the junction between the parietal, temporal, and occipital lobes. It is thus crucial to identify the different pathways that anatomically connect this high-order association region to the rest of the brain. Our study revisits the three-dimensional architecture of the structural AG connectivity by combinin

Incisional hernia appendicitis: A unique case report.

INTRODUCTION AND IMPORTANCE: Incisional hernia appendicitis is very rare. Very few papers have been published reporting about Pfannenstiel incision hernia with appendicitis. Here we report one such case of Pfannenstiel hernia with appendicitis. CASE PRESENTATION: A 20 years old lady presented with a tender mass in the right iliac fossa after Pfannenstiel incision. Ultra-sonography showed an incisional hernia with a 12.1 mm defect at the site of the incision. A provisional diagnosis of strangulated hernia was made. Acute appendicitis was diagnosed intra-operatively. Appendectomy followed by pr

Thoracotomy of an asymptomatic, functional, posterior mediastinal paraganglioma: A case report.

BACKGROUND: Paragangliomas in the mediastinum are rare, accounting for only 1%-2% of all paragangliomas and < 0.3% of all mediastinal tumors. Most paragangliomas are nonfunctional, therefore, asymptomatic functional paragangliomas in the left posterior mediastinum are extremely rare. Perioperative management including preoperative preparation, careful intraoperative procedures, and strict postoperative care is important, and one-stage surgical resection should be performed only after appropriate perioperative measures are undertaken. Because those tumors are rare, it is necessary to report kno

Thoracic Paraspinal Castleman Disease in a Young Mexican-American Man with Postcoital Hematuria.

BACKGROUND: Castleman disease (CD) is an uncommon disorder of deregulated lymphoproliferation with unicentric (UCD) and multicentric forms based on extent of nodal involvement. Gross resection with histopathologic analysis remains the gold standard for diagnosis of UCD and is curative in most cases. Symptomatic paraspinal UCD is a rare presentation with potentially dangerous complications, and its tendency to mimic more common spinal tumors presents a significant diagnostic challenge. CASE PRESENTATION: A 25-year-old Hispanic man with no past medical history was evaluated for a known left-sid

Defining Treatment Success After 2-Stage Exchange Arthroplasty for Periprosthetic Joint Infection.

BACKGROUND: Two-stage exchange arthroplasty remains the preferred surgical treatment method for patients with chronic periprosthetic joint infection (PJI). The success of this procedure is not known exactly as various definitions of success have been used. This study aimed at analyzing the difference in outcome following 2-stage exchange arthroplasty using different definitions for success. METHODS: A retrospective study of 703 patients with PJI who underwent resection arthroplasty and spacer insertion between January 1999 and June 2015 was performed. Chart review identified intraoperative cu

Organ Sparing Surgery for Penile Cancer: A Systematic Review.

PURPOSE: Although penile cancer represents only 1% of all male cancers, the traditional treatment, total or subtotal penectomy, carries devastating psychological and functional outcomes. Organ sparing surgery is an attractive option if it can provide satisfactory cancer control equivalent to or nearly equivalent to standard techniques. This approach is meeting increasing acceptance. We offer a timely comprehensive review to increase awareness of these procedures and their applicability, to evaluate the techniques objectively and to provide guidance to the practicing urologist. Developing a coaching mechanism for practicing surgeons.

BACKGROUND: While performance feedback and assessment are hallmarks of surgical training, they abruptly cease after training is completed. In their absence, performance may stagnate and poor habits persist. Our aim was to develop a coaching mechanism for practicing surgeons with feedback provision based on objective performance assessment. METHODS: Technical and nontechnical intraoperative video recordings from laparoscopic or robotic cholecystectomies, colectomies, and hysterectomies were assessed by a blinded surgeon and a human factors expert, respectively. Aspects of performance in need o

How much is too much? A guide to appropriately bending ball tip guide wires when using intramedullary nails for the treatment of lower extremity long bone fractures.

INTRODUCTION: This study was to determine the angle of bend that can be placed in a ball-tip guide wire while still allowing passage of reamers and nails of several commonly utilized nailing systems. METHODS: Ball-tip guide wires, opening reamer tips, femoral nails, and tibial nails were collected from several manufacturers. Guide wires were incrementally bent 3 cm from the tip and passed through the reamer tip, tibial nail, and femoral nail until unable to pass. RESULTS: All systems tested demonstrated that the reamer, with its relative smaller diameter cannula as compared to the nails them
